Des erreurs peropératoires tel : le faux canal, le stripping ; peuvent se manifester durant cette étape. L’instrumentation endodontique à base d’acier inoxydable est souvent à l’origine de ces erreurs à cause de la rigidité de l’alliage à base duquel elle est fabriquée.Pour pallier ce problème, l’instrumentation NiTi endodontique a été conçue puis employée afin de permettre un meilleur respect de la trajectoire canalaire, grâce à la flexibilité de l’alliage NiTi.Matériel et méthodes : le présent travail avait pour objectif principal l’évaluation du respect de la trajectoire canalaire par les instruments NiTi du système ProTaper®universal rotatif. It is usually asymptomatic and discovery is incidental on routine radiological examination. Treatment is necessary if there is an infection due to exposure to the lesion in the oral cavity. In these severe cases, pain, cortical blowing, and facial deformity may be present. Thus, patients with cemento-osseous dysplasia have prosthetic bearing surfaces that are unfavorable for prosthetic rehabilitation.The management of cemento-osseous dysplasia requires a careful clinical examination to make a correct diagnosis and to develop the appropriate treatment plan. However, Bangladesh has achieved a significant reduction in childhood mortality from 173 per thousand. The high level of neonatal deaths in Bangladesh is a great concern among all health professionals. Objective: To determine the frequency and pattern of electrolytes imbalance in neonates with septicaemia and their impact on mortality and morbidity. Methods: All neonates suffering from septicaemia and admitted to BSMMU from 1st January 2011 to 15th September 2011 and fulfilled the inclusion criteria were evaluated. Results: During the study period total of 120 neonates were admitted with the diagnosis of septicaemia in the Department of Neonatology, BSMMU, Dhaka, Bangladesh. Among the neonates, mean age was 9.26±4.58 days, mean weight 2282.68±580.40 gm, mean gestational age 35.68±2.40 weeks, males were 66 (55%), and females were 54 (45%). Electrolyte abnormalities were found in 42 (35%) neonates with septicaemia. Among the electrolyte abnormalities, hyponatremia was found in 24 (20%) neonates, hypernatremia in 12 (10%), hypokalemia in 17 (14.2%), hyperkalemia in 19 (15.8%), and mixed abnormalities in 18 (15%) cases. Neonates with dyselectrolytemia were found significantly associated with prolonged hospital stay compared to those with normal electrolyte levels (p< 0.05). Hyponatremia, hypokalemia, and mixed electrolyte abnormalities were significantly associated with higher mortality (p<0.05). But hypernatremia and hyperkalemia were not significantly associated with higher mortality (p> 0.05). Conclusion: Electrolyte abnormalities are common in neonates with septicaemia. But the relationship between Metabolic Syndrome and Type 2 Diabetes Mellitus and the independent contribution of Metabolic Syndrome remained unclear. To evaluate the status of metabolic syndrome in the Type 2 diabetic population and for their ability to predict Type 2 Diabetes Mellitus, in parallel with conventional stroke risk factors, this study was planned. Methods: A hospital-based descriptive observational study was conducted in 100 patients with Type 2 Diabetes Mellitus who were selected by consecutive random sampling techniques. Patients were diagnosed through proper history, clinical examination, and necessary investigations. The outcome values were measured in anthropometric measurements, blood pressure, dyslipidemia, and blood glucose. Results: Among 100 patients with Type 2 Diabetes Mellitus, (64%) were females who were housewives (41%) with significant difference (p < 0.001) and Mean± SD was 58±12.24 yrs ranging from 32-85 years. About 30% had a positive family history of Type 2 Diabetes Mellitus and 28% tobacco abuse. Of the total, 31% of patients had high blood pressure, and 28% had low HDL with a significant difference (p<0.001), but 46% had high triglyceride levels, which was not significant. More than half (53.12%) females showed central obesity, while only 16.67% had central obesity among males, and it showed a significant difference (p=0.04) in both sex. There was a high frequency of Metabolic Syndrome in a patient with Type 2 Diabetes Mellitus (68%) with a significant difference, and 48% were female, and 20 % were male. In patients of Metabolic Syndrome, central obesity was found in 36(52.94%) patients where females were predominantly affected 32(88.89%), hypertension found in 28(4118%) patients where male were predominantly affected 16(57.14%) patients, hypertriglyceridemia found in 44(64.70%) patients where a female was predominantly affected 26(59.09%) patients and low HDL found in 22(32.35%) patients where a female was predominantly affected 16(72.73%). Conclusion: The frequency of Metabolic Syndrome was more than double in women (48%). Central obesity and low HDL were significantly more prevalent in women than men, where hypertension was significant in men. The purpose of this survey, divided into three parts, is to evaluate the knowledge, attitudes, and practices of dentists in private practice in the Rabat-Sale-Kenitra region regarding the design of removable partial dentures, to provide information on the means of communication with laboratory technicians, and to investigate possible correlations between the failure of the prosthetic project and certain adopted practices. Materials and methods The study concerned a sample of 101 dentists practicing in the region of Rabat-Sale-Kenitra to whom we sent an anonymous 4-page printed questionnaire containing 28 questions on the design of metal frames in PAPM. A descriptive and analytical statistical study was conducted to process the data.Results Following the results of the statistical study, only 8% of the practitioners performed more than 10 partial removable prostheses per month, 17% did not perform a clinical examination, 20% did not perform a study model, 69% did not perform the RPD design by themselves and entrusted this task to the dental technician while 89% did not use a Dental Surveyor. Conclusion This survey showed that a large number of practitioners do not follow the rules of good practice and that they lack knowledge of RPD design. Therefore, postgraduate training is envisaged to eventually help practitioners to implement these good practices and to improve this knowledge. The organism responsible for neonatal sepsis may vary across geographical boundaries even from institution to institution and with the time of illness thus periodic surveillance is a necessity. The present study was therefore carried out to determine the common pathogens and their antibiotic sensitivity pattern. Objectives: To isolate the bacterial agents causing neonatal sepsis determine the sensitivity pattern of the causative bacterial agents. Methods: This cross-sectional study was carried out in the neonatal intensive care unit of BSMMU from December 2012 to July 2013. Neonates (0-28 days) who were admitted in neonatal intensive care unit with suspected sepsis were included in this study. After admission written informed consent from parents were obtained and emergency management was given to the baby and then septic screening along with blood culture and antimicrobial sensitivity were done. All data were compiled, tabulated and then analyzed by SPSS V.12 according to the objectives of the study. Results: A total 94 newborn babies with suspected sepsis were included in this study. Most of the babies (54.3%) were admitted within 24 hours after birth, 86.17% babies were preterm and 81.92% babies birth weight was <2500 gm. There was preponderance of male babies over females comprising 53% male and 47% female. Among the suspected septic newborns 27.66% of babies had culture-positive sepsis and 73.41% of babies had culture-negative sepsis. Among the culture-positive cases, 72% of babies were developed late-onset sepsis and 28% of babies were developed early onset sepsis. The isolates from blood culture were Klebsiella pneumoniae in 9 (34%) cases, 6 (23%) cases were E. coli, Acinetobacter, Staphylococcus was found in 2 (8%) cases and Pseudomonas, Enterobacter, and Citrobacter were found in 1 (4%) case. Gram negative organisms predominated on Gram positive bacteria. Imipenem, Ciprofloxacin, Colistin, and Netilmicin were the most sensitive antibiotics to the commonly isolated organisms. Almost all organisms were resistant to Ampicillin and Gentamicin. Conclusion: Klebsiella pneumoniae is the commonest organism responsible for neonatal sepsis in BSMMU. Mbaye","doi":"10.15342/ijms.2022.619","DOIUrl":"https://doi.org/10.15342/ijms.2022.619","url":null,"abstract":"Objectives: The objectives of the present study were to assess the perceptions of physicians and midwives towards an Obstetric Electronic Medical Record and to apply the diffusion-of-innovation theory to the examination of factors that are perceived by users as influencing the implementation of Electronic Medical Records.Patients and methodsType of study: This is a mixed study, after the implementation of an Electronic Medical Record (EMR). Study location: Dakar, Senegal.Methodology: We designed an online questionnaire that was sent to all users of the software. The questionnaire included three parts: a section collecting the socio-demographic and professional characteristics of the participants, a section in relation to the usage of the software, the impact on the organization, the advantages, and limits, and finally the last section where the users reported their degree of satisfaction as well as the extent to which they would be willing to recommend the software-based upon their experience. The questionnaire was completely anonymized and accessible to physicians wishing to take the survey. Their consent was first requested.Results: A total of 51 physicians (85%) agreed to participate in this study among the 60 who were approached. Of the respondents, 62.7% were women and the average age and experience were 32.3 years and 22.1 months, respectively. Overall, the software was perceived as being simple to use, intuitive, and designed for an obstetrician. On the contrary, some physicians reported some drawbacks. Those were mainly the time needed to type in the information especially when they were on duty with a heavy workflow, lack of sufficient computers, and some bugs. More than half of the participants agree that there is no need to be young to use optimally the software. However, an ideal user should be open-minded and be able to cope with change. On a 0-10-scale, participants acknowledged that they were satisfied with the software, it impacted positively their department’s organization and they were willing to recommend the software; all medians above 8.Conclusion: Identifying the determinants of the adoption of the electronic medical record is crucial for its successful implementation and dissemination. Shen","doi":"10.15342/ijms.2022.617","DOIUrl":"https://doi.org/10.15342/ijms.2022.617","url":null,"abstract":"Objective: To investigate the efficacy and safety of chemoradiotherapy and radiotherapy followed by surgery in patients with locally advanced unresectable rectal cancer.Material and method: We reviewed records for 65 patients with locally advanced unresectable rectal cancer treated by preoperative chemoradiotherapy or radiotherapy followed by surgery between 2013 and 2016. Of these, 23 patients were treated with preoperative chemoradiotherapy (40 - 45 Gy) plus concomitant chemotherapy (5Fluorouracil + Calcium Folinate). For comparison, 42 similar patients, treated by preoperative radiotherapy (45 - 50Gy) plus surgery served as control. The primary end-point of the study was overall survival and local control rate.Results: No treatment plan was delayed because of toxicities in both groups. The radical resectability rate was 69.9 % in the chemoradiotherapy group and 33.3 % in the radiotherapy plus surgery group (P = 0. 024). The anal sphincter preservation rates were 26. 6 % and 3. 7 %, respectively (P= 0. 028). The anal sphincter preservation rates of the lower rectal cancer were 27. 3 % and 0. 0 %, respectively (P = 0. 014). Response rates of chemoradiotherapy and radiotherapy plus surgery groups were 82.6 % and 61.9 % (P = 0. 053). The tumor downstage rates were 16 (69.6%) and 24 (57.1%) in these groups (P = 0. 206). The 3-years overall survival rates were 66. 7 % and 55. 6 % (P = 0. 485), and the tumor-free survival rates were 40. 3 % and 33. 1 % (P = 0. 663) . The 3-years local recurrent rates were 26. 9 % and 48. 1 % (P = 0. 174) . No obvious late effects were found in either group. Conclusion: The results of this study suggested at least that acute side effects of preoperative chemoradiotherapy can be tolerated and a higher surgical resection rate can be achieved. However, the chemoradiotherapy did not improve the survival rate while it increased local recurrence due to the high rate of anal sphincter preservation.
