Preoperative diagnosis is usually difficult, but it remains of favorable prognosis. In this study, the authors report a case of a 33-year-old Moroccan man with a history of left pyelolithotomy who presented with a 19 cm left renal mass associated with two kidney stones in the lower calyx. He underwent an open radical nephrectomy, and pathology revealed a renal SFT. To the best of our knowledge, our patient is the first case of renal SFT with a history of urolithiasis and presenting with concomitant ipsilateral calculi. {"title":"A Rare association of Mauriac syndrome and Van-Wyk Grumbach syndrome found in a young Saudi girl with poorly controlled type 1 Diabetes and Hypothyroidism: A Case Report and Brief literature review","authors":"Aida Al Jabri, Aeshah Al Johar, M. Yacoubi","doi":"10.15342/ijms.2022.679","DOIUrl":"https://doi.org/10.15342/ijms.2022.679","url":null,"abstract":"Type 1 diabetes mellitus (T1DM) is an autoimmune disease characterized by insufficient insulin production of the pancreatic beta-cells. Patients with T1DM will have a higher risk of other autoimmune disorders like celiac and thyroid diseases. Hypothyroidism is the failure of the thyroid gland to secrete an adequate amount of thyroxine, which is required for physical growth, brain development, and cellular metabolism. Most studies reported that children with T1DM have a higher incidence of hypothyroidism than normal children, with 9.6% having hypothyroidism and 19% having positive anti-TPO antibodies. Hypothyroidism will aggravate the condition in a child with T1DM and vice versa. Uncontrolled diabetes for a long time might increase insulin resistance due to complete depression of the hypothalamus-pituitary thyroid axis. A rare complication of poorly controlled T1DM is Mauriac syndrome, characterized by elevated liver enzymes, hyperlipidemia, cushingoid features, growth retardation, and hepatomegaly due to glycogenic hepatopathy. Van Wyk–Grumbach syndrome is also a rare complication of long-standing, untreated hypothyroidism, manifested by breast development, multicystic ovary, uterine bleeding associated with lack of pubic and axillary hair growth, and delayed bone age. {"title":"Attitudes and Beliefs of Physical Therapists in Saudi Arabia Regarding Direct Access and Scope of Practice","authors":"Safwan Alosaimi, R. This study aimed to better understand what practicing physical therapists know and believe about direct access (DA) and scope of practice (SOP) for physical therapy in Saudi Arabia (SA). Methods. A pilot study was first performed to ensure the clarity of the questions. Then, a cross-sectional survey was sent through emails and social media platforms including three main sections: demographics, opinions, and beliefs questions. Results. A total of 150 respondents met the inclusion criteria. About 55.3% of the participants reported never or rarely using an SOP document as a reference for knowing their practice. Moreover, nearly 48% learned about the SOP through an informal discussion. Only 24.7% of the participants correctly identified which practice setting DA is permitted. Most of the participants felt confident or strongly confident of their abilities to assess (67.3%) and treat (72%) patients without physicians’ referral. Around 84% of the participants agreed or strongly agreed that DA should be expanded to include all healthcare settings. Conclusion. We found that physical therapists are confident about their ability to treat and assess patients without physicians’ referrals. Hypospadias occurs in 1 in 125 live male births. Classically three abnormalities are found in the hypospadias penis: 1) an ectopic opening of the urethral meatus at the ventral surface. 2) A ventral curvature of the penis (Chordee), and 3). A hooded foreskin on the dorsum of the penis with a lack of skin on the ventral due to a V-shaped defect referred to as urethral delta. There are anterior (65%), middle (15%, and posterior penile hypospadias (20%). Objective: The objective of surgical repair of hypospadias is to provide complete straightening of the penis, placing the meatus at the tip of the glans, forming a symmetrical conical-shaped glans, and constructing a urethra uniform in caliver and acceptable skin coverage. Materials and Methods: This is a Cross-sectional variety of descriptive type of observational study conducted in the department of pediatric surgery, Dhaka Medical College & Hospital, Dhaka, Bangladesh, from 1st January 2009 to 30th June 2010. The total number of cases is 50. Patients having anterior penile hypospadias were selected by simple random sampling. The mean age of the patients was 5.22 years (2 to 10 years). Surgical correction was done with the Snodgrass technique. The mean operative time was 90 minutes. Post-operative hospital stay was10 days. Results: Satisfactory results were obtained in 35 (70%) patients, and only 14 (28%) had urethrocutaneous fistula, wound infection 5(10%), and meatal stenosis 2 (4%). The final outcome was evaluated based on urethrocutaneous fistula, meatal stenosis, and cosmesis. Out of 50 patients, 35 (70%) did not have a urethrucutaneous fistula and were cosmetically acceptable. Conclusion: Early detection and correction are essential to minimize the complications and also to lessen the psychological trauma of the patient. For this, a thorough examination of the neonates following delivery in different maternal and child care clinics is mandatory. Early and timely measures also give the best cosmetic and functional results. Objective: This is to evaluate the hemodynamic modifications precipitated by oxytocin given as an I/V bolus or infusion to limit uterine bleeding in cesarean section. Methods: This prospective, interventional learning was once carried out at the Department of Anaesthesiology, Abdul Malek Ukil Medical College & Hospital, Noakhali, Bangladesh, from January to December 2020. It is a very common medical problem affecting 5-20% of patients attending otolaryngologists in the tropical and subtropical regions of the world. Objective: To assess the Pattern of Otitis Externa in the ENT Outpatient Department. Methods: A prospective study was carried out in the outpatient department of Bangabandhu Shelkh Mujib Medical University among 200 patients (220 affected ears) with the aural symptoms suggestive of otitis externa and 110 healthy control (220 ears), from July 2002 to December 2003, with the aims and objectives to find out the pattern of disease among the patients suffering from otitis externa in Bangladesh. Results: It is found from the study that the maximum number (35%) of patients were within 21-30 years of age group. Most patients were male (56%), with a male-female ratio is 1.3:1. Higher numbers of patients were fallen within lower and middle-income family groups (up to 2,500 and 3,501-4,000 taka per month, respectively). Most of the patients (30%) were laborers, and most patients were literate. The majority lived in a paka (44%) or semi-paka (38%) house and used tap water for bathing (53.5%). Most patients used cotton buds (49%) for cleaning of ears. Otitis externa was mostly found in the summer and rainy season (69%), with maximum occurrence during June (18.2%) and July (16.4%). In most of the patients, symptoms persisted for less than two months (76.3%), of which itching, pain in the ear, and sense of blocking are the commonest triad of symptoms. Itching is the predominant symptom (98.2%) in fungal, and pain is the dominant symptom (92.8%) in bacterial otitis externa. Acute infection is commonly painful, whereas chronic disease is mostly itchy. The regarding involvement right ear was more involved (60%) than the left one and the ear canal was more commonly diffusely involved (84.5%). Among the predisposing factors for the causation of otitis externa, ear cleaning either by self or others ranked the top 42, 7% of the list. It was mainly associated with acute cases. The fungus was isolated from the affected ear in most instances (49.1%), followed by Isolation of bacteria (31.8%), of which bacteria were often associated with the acute cases, and fungus was found more in the chronic cases. Aspergillus was found in 89.7% of the fungal species, and candida was found in 7.5% of cases. Pseudomonas was the predominant organism (40%), and Staphylococcus was the second most common (37.1%) among the bacterial species isolated. The microbiological finding among the healthy control showed that the external ear canal is less commonly harboring pathogenic organisms. Bacteria were isolated from 18.2% of cases, and fungus was isolated from only 6.3%. Methods: 121 cases, Benign prostatic hyperplasia (BPH) patients were assessed retrospectively and were divided into good prognosis (group A) and poor prognosis (group B) according to the degree of improvement in maximum urinary flow rate, and the changes in ultrasound and non-invasive urodynamic parameters between the two groups were explored. Results: The PV, IPP, and PVR of group A were lower than those of group B (P<0.05), and Qmax of group A was greater than group B (P<0.05). The difference was statistically significant (P<0.05); There was no significant difference in DWT, age, and IPSS(P>0.05). The ROC curve analyzes the diagnostic efficacy of each parameter in the diagnosis of the efficacy of TURP, and the results are ranked in order of 1/Qmax (AUC=0.777), PV (AUC=0.715), PVR (AUC=0.642), IPP (AUC=0.629), Of which 1/Qmax has the best diagnostic efficiency, and it AUC=0.777, the best cutoff value is 0.12, the sensitivity is 0.81, and the specificity is 0.571.
