Silantyeva","doi":"10.36425/rehab50236","DOIUrl":"https://doi.org/10.36425/rehab50236","url":null,"abstract":"Background: rehabilitation of patients after severe pneumonia associated with the new coronavirus infection requires the searching for effective tools to restore impaired functions, including methods of hardware physiotherapy. \u0000Aims: to evaluate the effectiveness of the application of high-intensity electromagnetic field (HIEF) in rehabilitation of patients after pneumonia associated with COVID-19. \u0000Materials and methods: 40 patients were examined and treated at the outpatient stage of rehabilitation after severe pneumonia associated with COVID-19. All patients received a set of rehabilitation measures, including daily sessions of therapeutic exercises (No. 15) and magnetotherapy procedures (No. 15). Patients were randomly separated into 2 groups - 20 patients in the treatment group (TG) who received HIEF therapy (BTL-6000 Super Inductive System) and 20 patients in the control group (CG) who received low-intensity magnetotherapy (BTL-4000 Premium device). \u0000Results: during the course of therapy, there were no patients who dropped out of the study, and no undesirable side effects and complications were identified. The high clinical effectiveness of the complex of rehabilitation measures has been proven, more pronounced in the group of patients receiving HIEF therapy. In Japan, a legislative and normative-methodological base for rehabilitation has been formed, staged rehabilitation has been organized (acute period, rehabilitation, outpatient). Equipment standards, staffing and other standards for rehabilitation services have been developed, depending on the type of disease. Great importance is given to patient routing and treatment quality assessment. Specialists are being trained: rehabilitologist, physical therapists, occupational therapists, speech therapists, nurses, social workers. All specialists are included in the list of staffing standards for medical institutions necessary for obtaining accreditation for medical rehabilitation. Payment for medical care in Japan is based on the insurance principle. The cost and standard term for receiving rehabilitation assistance depending on the disease is determined by insurance standards. Rehabilitation approaches of patients with another pathology also require to be reviewed under the conditions of the SARS-CoV-2 epidemic. It is necessary to develop clinical recommendations for the rehabilitation care delivery in patients with COVID-19 and other diseases during a pandemic. A group of experts analyzed the data of current reviews on rehabilitation with COVID-19, as well as previous studies on rehabilitation in patients with Post-intensive care syndrome (PICS) and acute respiratory distress syndrome of non-coronavirus etiology, and developed thereupon the main positions of clinical recommendations. We discuss the main rehabilitation problems in patients with COVID-19, in particular structures and functions impairments, leading to limitations and restrictions of self-care, mobility, domestic life, communication, interpersonal interactions and relationships, professional activities and those determining care requirement. The article also provides general recommendations on medical rehabilitation management in a pandemic at all three stages, including patients routing, ensuring the infectious safety of medical personnel and patients. The necessary individual rehabilitation program components for patients with COVID-19 at all stages of rehabilitation have been identified, including patients examination, nutritional deficiency correction, restoration of respiratory function, exercise tolerance, muscle strength, self-monitoring and training in new movement conditions, psychoemotional state disorders and cognitive functions, as well as independence in everyday life recovery. Along with the measures to restore the basic body functions and prevent secondary brain injury, one of the key areas of modern rehabilitation is nutritional and metabolic support. \u0000Aims: to improve the results of the treatment of the patients by optimizing the system of nutritional and metabolic rehabilitation using enteral, parenteral, combined parenteral-enteral nutrition methods, who are undergoing rehabilitation after recovering from Coma resulting from injuries and diseases of the central nervous system. \u0000Methods. In a prospective cohort study, the results of the study and complex treatment of 113 patients after recovering from Coma as a result of brain injury of various etiologies who were undergoing rehabilitation in the period from 2017 to 2019 in the FNRC RR were analyzed. \u0000Results. Indirect calorimetry has proven to be the most accurate method for creating an effective and adequate program of nutritional and metabolic support in patients in chronic critical conditions who recovers from the coma. In patients who recovers from the coma, due to the excessive process of hypercatabolism, increasing protein requirements: the average protein requirements for a patient who recovers from the coma are 1.261.5 g of protein per kg/weight. Conducting optimal nutritional support for patients recovering from Coma involves determining the functional state of the gastrointestinal tract. To build an effective and adequate program of nutritional and metabolic support where accurate determination of energy needs is of great importance. \u0000Conclusions. Median of remission interval was 2 years (from 3 month to 10 years). MS was evaluated by Bruininks-Oseretsky Test of Motor Proficiency. NS was assessed using anthropometry and bioimpedance analysis. As a result, it was found that children after cancer treatment have motor and nutritional deficiencies. Low values of NS indicators (lean body mass, cell mass, mid-arm muscle circumference (MAMC)) are associated with a decrease in MS parameters. There was no dependence between the value, dynamics of body fat mass and MS. According to the initial hypothesis, the sera of patients with malignant tumors of different localization and different histological nature, contain different sets of autoantibodies (auto-Ab) of IgG class to many cancer-associated antigens (CA-AG). The content of such auto-Ab differs in cancer patients and patients with non-malignant chronic diseases, which determines the difference in serum immunoreactivity profiles of cancer patients and patients with non-malignant diseases. Confirmation of this hypothesis opens up prospects for the creation of simple and cheap laboratory methods of mass preventive examination of the population for the early detection of different cancers. The confirmation of the hypothesis was obtained.
