Tynterova","doi":"10.36425/rehab77964","DOIUrl":"https://doi.org/10.36425/rehab77964","url":null,"abstract":"Aims: to assess the severity of cognitive, psychoemotional, behavioral disorders, the level of self-control and prevalence of certain coping strategies in patients with lesions of the right and left cerebral hemispheres after acute ischemic stroke. \u0000Materials and methods: The study included 60 patients with a diagnosis of ischemic stroke. Depending on the location of the stroke, patients were divided into two groups: Group 1 - 30 patients with lesions of the right hemisphere, Group 2 - 30 patients with lesions of the left hemisphere. The study was conducted on the 7th day of treatment and included: assessment of the cognitive status according to the Montreal scale (with additional scales for assessing the functions of the right hemisphere); assessment of the level of distress, depression, anxiety and somatization using the Four DimensionalSymptomQuestionnaire; assessment of mental and physical asthenia according to the MFI-20 scale, level of self-control using the Rotter'sLocus of Control Scale and coping strategies modality using E. Heim coping questionnaire. \u0000Results: The study established the prevalence of depression, a tendency towards non-adaptive coping strategies, ideational praxis related disorders understanding of metaphors and low level of self-control in relation to health in patients with lesions of the right hemisphere. Patients with ischemic stroke localized in the left hemisphere are characterized by a high level of mental asthenia, a choice of adaptive and relatively adaptive coping strategies, and a low level of self-control in respect to interpersonal relationships. \u0000Conclusions: The results of the study show that in the acute period of stroke there are qualitative and quantitative differences in neuropsychiatric symptoms of patients with lesions of the right and left cerebral hemispheres. The predominance of depression, cognitive impairments, and non-adaptive coping strategies in patients with lesions of the right hemisphere even with minimal or moderate motor deficits justifies early examination of the patient's behavioral, neuropsychiatric, and cognitive syndromes. In recent decades, instrumental methods have been used to improve the accuracy of diagnostics. The phenomenon of covert cognition and cognitive-motor dissociation have been demonstrated in a small proportion of patients in studies using instrumental methods in combination with different paradigms. This article describes the main features of the diagnostic paradigms used for such purposes. Currently, the development of its own complex of paradigms is held at the Research Center of Neurology collaboratively with group of neuropsychologists from Lomonosov Moscow State University. The general characteristics of this complex of paradigms are indicated. The complex is a flexible system that allows to train any movements of both the upper and lower extremities in any virtual environment with varying degrees of immersiveness. One of the most common complications of breast cancer treatment is postmastectomy syndrome (PMES). That is why the study of the latter and the development of modern methods of rehabilitation of patients with an established diagnosis of PMES remain extremely relevant. To date, the pathogenetic mechanism of the development of upper limb edema and a huge range of possible methods of prevention and treatment have been described in detail. It shows that Dutch rehabilitation is based on the International Classification of Functioning, Disability and Health (ICF) and organized in three different levels: rehabilitation medicine, geriatric rehabilitation, and monodisciplinary therapy. The author describes these three levels in their distinctive scopes and numbers. Together they operate as a coherent rehabilitation service system that reflects an important characteristic of the system of health care in the Netherlands: decentralized if possible and centralized if necessary. The system is accessible for all and it is organized in a way that simple medical problems can be dealt with close to home, whereas specialized treatment is concentrated in rehabilitation centers spread over the country. The rehabilitation facilities are embedded in the general health care system: it is easy for hospitals as well as general practitioners to refer patients to rehabilitation. The quality of life was assessed independently by the patients using the DemboRubinstein questionnaire. Before the start of immunotherapy, patients remained dissatisfied with their health, appearance, sexual relations, and mood. A repeated study showed that these spheres of life were significantly improved in all the subjects, and health satisfaction increased by more than 1.5 times compared with the initial one. In the course of immunotherapy, a significant increase in the scales of social functioning is noted: communication with others, opportunities for leisure and recreation. Early rehabilitation of patients after removal of neoplasms is an urgent task of modern rehabilitation. Clinical case description. This clinical example describes an example of rehabilitation and dynamics of recovery in a patient after removal of glioblastoma. After removal of a malignant neoplasm of the central nervous system, the patient underwent a course of rehabilitation measures within the framework of hospitalization at the Federal Research Center for Clinical Center of the Russian Federation. Conclusion. Current guidelines support its use in patients with stable chronic heart failure and coronary artery disease, after myocardial infarction, acute coronary syndrome, coronary artery bypass grafting, coronary stent placement, and valve surgery. Its use in these conditions is supported by a robust body of research demonstrating improved clinical outcomes. The significant clinical improvement obtained through the regular training in patients with cardiovascular diseases is the result of a complex interplay of different effects: 1) improved cardiopulmonary efficiency and pulmonary functional capacity; 2) amelioration of myocardial perfusion by reducing endothelial dysfunction and by inducing new vessel formation; 3) improved myocardial contractility; 4) counteract the muscle wasting and cachexia; 5) reduction of the systemic inflammation; 6) attenuation of the sympathoexcitation, a typical feature of CHF, even in the persistence of cardiac dysfunction.
