In many ways, it is associated with a decreased volume of dynamic motor activity and an increased static loading. In patients with an active lifestyle, BP often develops when the technique of physical exercises or habitual movements are violated, thus often leading to secondary osteoarticular disorders. One of the most common causes of non-specific back pain (NBP) is the sacroiliac joint (SIJ) dysfunction. Makarenko","doi":"10.36425/rehab121733","DOIUrl":"https://doi.org/10.36425/rehab121733","url":null,"abstract":"Not all the patients who are diagnosed with COVID-19 can completely recover; some of them experience miscellaneous persistent symptoms that subsequently wax or wane. As the COVID-19 pandemic continues, the number of people with long-term symptoms is rapidly increasing, adding to the burden on healthcare and society. The prevalence of the consequences of COVID-19 varies between studies, with some reporting that more than half of hospitalized patients have prolonged symptoms for at least 6 months after acute SARS-CoV-2 infection, and others for more than 12 months. The overall prevalence of residual symptoms in patients infected with SARS-CoV-2 is currently estimated to be 1030%. This clinical syndrome is commonly referred to as post-acute COVID syndrome (PACS) or long COVID. This multifactorial syndrome is characterised by a variety of debilitating symptoms, including fatigue, brain fog, postural hypotension with tachycardia, and post-exertional malaise. Many of the observations of post COVID-19 condition, including changes in immune, cardiovascular, gastrointestinal, nervous and autonomic systems, are shared with the symptoms described in myalgic encephalitis/chronic fatigue syndrome (ME/CFS) patients. Comprehensive longitudinal symptom monitoring is required to confirm of diagnosis, uncover the mechanisms of post-COVID-19-associated ME/CFS, and develop prevention and treatment measures. Current absence of the effective treatment reflects the unclear causes of the post COVID-19 conditions which cannot be targeted properly until the mechanism is established and confirmed. The multisystem aspects of long COVID remain poorly understood. The COVID-19 pandemic has exposed a significant gap in knowledge about the post-acute consequences of infectious diseases and the need for a unified nomenclature and classification of post-COVID conditions, diagnostic criteria, and reliable assessments of these disorders. Unraveling the complex biology of PACS relies on the identification of biomarkers in plasma and tissue samples taken from individuals infected with SARS-CoV-2 that will allow classification of the phenotypes of patients who develop PACS. For the full treatment of patients with post-COVID syndrome, multidisciplinary therapy and rehabilitation are required. This required of change in the existing procedures of rehabilitation treatment while maintaining its staging. Particularly, a transformation of in-person rehabilitation to telerehabilitation is recommended. Previous experience allowed us to organize telerehabilitation for patients with COVID-19 in the early days of the pandemic. We summarized findings of our study in this article. \u0000Aims: The purpose of this article was to compare effectiveness of two technologies for the rehabilitation treatment in patients with post-COVID: in-person (day-stay hospital) and telerehabilitation. \u0000Materials and methods: This prospective, parallel group study included patients with post-COVID, who received day hospital or telerehabilitation treatment between May 2020 and February 2021. Efficacy points were physical activity intensity level measured by Borg scale and a measure of the generic quality of life by EQ-5 (European Quality of Life 5-Dimension Questionnare). The choice of these metrics is explained by the possibility of using in the form of teleinterview.Results: Overall 180 patients who received day hospital (DH) rehabilitation treatment (n = 97; 64 women and 33 men; 55,1 11,9 years) or underwent telerehabilitation (TR) (n = 83; 52 women and 31 men; 54,4 12,76 years) were included to study. In DH group mean patient-day was 12,6, mean number of sessions achieved 10. In these patients high compliance to treatment was noted: only 5 (4,9%) of participants visited 5 sessions. \u0000As a result of treatment in DH and TR groups, statistically significant improvement both on the Borg score and EQ-5 was observed (p 0,05 compared the treatment initiation). Mean improvement in Borg scale in patients of DH group achieved 3,65 and that in TR group was 1,43 (p = 0,001). Between-group differences in the effectiveness of therapy by EQ-5 were not statistically significant (p = 0,341). Form of COVID did not affect the effectiveness of rehabilitation treatment. We also assessed TR-patients' perception of the physical non-interaction during the therapy: all participants reported a high quality of medical care. Neither patients age nor treatment time did not affect the level of satisfaction: 12 patients expressed their desire to repeat the TR course. No stop-signs or adverse effects were registered during the rehabilition period in both treatment groups. \u0000Conclusions: The use of telerehabilitation in medical rehabilitation help of patients with post-COVID is not less effective than in-person (day-stay hospital) treatment. Given high risks of infection during the COVID pandemic, it is possible to use such a method of rehabilitation, without in-person medical examination. The lack of funding within the budget and the basic CHI program led to limited access for patients to the trajectory of the completed cycle of MR after surgical interventions, which reduced the effectiveness of the provision of profile HTMC. The aim of study is to demonstrate the effectiveness of the system for organizing rehabilitation care for patients after arthroplasty of large joints of the lower extremities in a stationary (second) stage in a medical rehabilitation center. Materials and methods. The data of the medical information system on patients treated for three months at the second (inpatient) stage after arthroplasty of the hip and knee joints in the conditions of the Medical Rehabilitation Center were used. The effectiveness of rehabilitation was assessed by tests and scales: RRS, VAS, Borg exercise tolerance scale, HADS, EQ-5D; MRS, index Leken. Results. 308 patients (306 operated) were treated after medical rehabilitation measures at the first stage with a RRS score of 4-5 points, of which about 80% - after primary and revision arthroplasty of large joints. Upon completion of rehabilitation measures, a statistically significant improvement in all functional indicators was noted. Discussion. Despite the ambiguous opinion of scientists about the need for inpatient rehabilitation after arthroplasty, according to our data, arthroplasty and the subsequent stage of medical rehabilitation using modern technologies in a hospital significantly improve the clinical status, reduce or eliminate pain, increase motor activity, improve the psycho-emotional background and the patient's quality of life. Stationary (second) stage of rehabilitation was effective in 100%. Conclusions. The results of the work of the CMR proved its organizational feasibility, clinical effectiveness and advantages: continuity and phasing of rehabilitation in a single institution; continuity of observation of the patient during the interaction of MDRT with the participation of the operating surgeon, an anesthesiologist-resuscitator. Modern guides for the medical treatment of chronic pain in cancer patients are not effective in all cases, especially when chronic neuropathic pain syndrome occurs. In this regard, there is a need for a multidisciplinary personalized approach to the tactics of chronic pain management in favor of surgical methods at earlier stages of this condition. Nowadays neuromodulation is widely used for the treatment of neuropathic chronic pain syndrome in patients with various pathologies, whereas it is not used routinely in cancer patients. \u0000CLINICAL CASE DESCRIPTION: We introduce a case report of successfully applied neuromodulation in patient with severe chronic pain syndrome due to rectal cancer. Epidural spinal cord stimulation at Th10Th12 levels allowed to stop the systemic use of opioids and achieve long-term remission. Lepikhina","doi":"10.36425/rehab112413","DOIUrl":"https://doi.org/10.36425/rehab112413","url":null,"abstract":"BACKGROUND: Solving the issues of rehabilitation of children after lower respiratory tract infections requires putting up rehabilitation diagnoses in the categories of the International Classification of Functioning, Disabilities and Health (ICF). \u0000AIMS: The study aimed to define the key problems in the ICF categories and make quantitative and qualitative assessments of the changes in the ICF domains in convalescent children with infectious diseases of the lower respiratory tract at the outpatient stage and in one month after the disease. \u0000MATERIALS AND METHODS: The assessment of rehabilitation was performed using the ICF categories for 201 convalescent children with lower respiratory tract infections. \u0000RESULTS: Most of the patients had significant impairments in the following functions: b134 Sleep functions; b152 Emotional functions; b280 Feeling of pain; b440 Respiratory functions; b450 Additional respiratory functions; b455 Exercise tolerance function; b460 Sensations associated with cardiovascular and respiratory functions; b730 Muscle strength functions; b740 Muscular endurance functions; b780 Sensations associated with muscles and motor functions, both at the time of discharge from the hospital and a month after the discharge. Environmental factors (e115 Products and technologies for personal use in daily life; e225 Climate; e260 Air quality; e310 Close relatives; e355 Medical workers; e410 Relationships with immediate family members; e450 Relationships with medical workers; e580 Medical services; e585 Services in education and training) were shown to have an influence on rehabilitation process. \u0000CONCLUSION: The identified range of functioning disorders and the influence of environmental factors indicates the need for medical rehabilitation for convalescent children with lower respiratory tract infections. The Classification proposed in 1994 does not provide a clear distinction between terms \"consequences\" and \"complications\" and currently, in our opinion, is outdated and was outdated long ago, since it raises more questions than it contains answers. \u0000AIM: to propose a new classification of spinal cord injury consequences and complications in children covering all periods of the injury course. \u0000MATERIAL AND METHODS: Children with severe spinal and spinal cord injury who have been admitted to the Clinical and Research Institute of Emergency Pediatric Surgery and Trauma in Moscow since 2014 were taken into the study. To diagnose their current state, the following diagnostic tools were used: clinical examination, trophological status, neurological assessment by ASIA scale, locomotor pattern, laboratory and instrumental testings; in addition, psychological examination so as to identify child's post-traumatic disadaptation and psychopathological state. \u0000RESULTS: Three years after the injury, spinal cord injury consequences were detected in 96% of injured children; spinal cord injury complications― in 56% of children who were treated at Clinical and Research Institute of Emergency Pediatric Surgery and Trauma in 20142020. The authors have developed a new classification of spinal cord injury consequences and complications in children. \u0000CONCLUSION: The early start of rehabilitation measures and subsequent long-term medical and psychological support of children with spinal cord injury delays the onset of spinal cord injury consequences, prevents complications, as well as encourages the child and his/her parents to comply with the recommendations of the rehabilitation team. The existing concept of the formation of pathological synergies considers them as a compensatory strategy in response to damage to the pyramidal tract, which, due to paresis and increased muscle tone, has acquired a pathological character. Recent studies in primates have shown that the contralateral hemisphere, in particular the reticulospinal and rubrospinal tracts, may be involved in motor control of the hand. The current hypothesis presents the corticoreticulospinal and corticorubrospinal tracts as a back-up system for neuronal reorganization due to injury. Due to the similarity of their symptoms, health care professionals often need to establish a differential diagnosis between these two disorders. This article is the first part of the literature review and is devoted to the discussion of the phenomenology and diagnostic methods of homonymous hemianopia and neglect. For the first time in the Russian literature, the use of the eytracking method in patients with homonymous hemianopia and neglect is highlighted. The article also provides criteria for the differences between the two disorders. The article contains useful information for health care professionals to make an appropriate diagnosis, which implies subsequent rehabilitation procedures.
