This motivates rehabilitation specialists to search for new technologies and effective approaches. Timely resuscitation, early specialized and comprehensive rehabilitation care largely determine the outcome of a serious injury and prevent serious consequences. In this context, medical convergent technologies used to provide effective medical care become particularly relevant. This article of scientific, methodological and applied nature is devoted to the description of medical convergent technology implemented in the system of comprehensive rehabilitation care of children with severe neurotrauma. \u0000AIM: The aim of the study was to substantiate and develop an effective rehabilitation technology at the level of unified (convergent) interdisciplinary managerial and clinical-diagnostic medical solutions in rehabilitation of children with severe injuries. \u0000MATERIALS AND METHODS: Children, aged 2–17, (n=420) with consequences after severe traumatic injuries who were admitted to the Clinical and Research Institute of Emergency Pediatric Surgery and Trauma (Moscow) for treatment and rehabilitation in 2018–2022 with diagnoses: traumatic brain injury, consequences of spinal cord injury, combined trauma, mine-explosive trauma, severe skeletal trauma, infected extensive wounds. More than 300 of these patients had neurotrauma. The researchers used: literature sources on the discussed problem, findings of objective and subjective examinations (clinical examination, neuroimaging, neurofunctional, laboratory, radiation, instrumental as well as questionnaires, surveys, scales; electronic medical histories). To develop medical convergent technology, rehabilitation diagnosis, potentials and prognostic options according to the decision-making algorithm developed by physicians/specialists as well as differentiated personified rehabilitation programs were used. While developing these programs, the researchers assessed specific features of the injury, disease course, severity and level of functional impairment, activity and participation expressed in the criteria of the International Classification of Functioning, Disability and Health. \u0000RESULTS: The researchers have enunciated the essence of the medical convergent technology in the rehabilitation of children after severe injuries as convergence, the alliance of interests from different fields, process of integration of traditional and new approaches which are united by a single goal of managing patients after severe traumatic injuries. \u0000CONCLUSION: An interdisciplinary/multidisciplinary approach with medical convergent technology component in restoring children after severe trauma allows to optimize the rehabilitation process and the patient’s routing within this process. Due to emergence of new rehabilitation protocols, it is extremely important to know the time of occurrence of consequences of spinal cord injury and adequate diagnosis of complications, which allows timely identification of life-threatening conditions of one of the body systems. \u0000Clinical case description. We present a clinical case of autonomic dysreflexia in neurogenic lower urinary tract dysfunction after traumatic cervical spinal cord injury in a 17-year-old girl. In traumatic spinal cord injury above the level of the sixth thoracic vertebra, autonomic dysreflexia as a variant of autonomic dysfunction is a common complication. This clinical case will be of interest to clinicians of all specialties who encounter patients with spinal cord injury. It is shown that misinterpreted symptoms of increased blood pressure during bladder filling were the cause of observation and treatment by a cardiologist for arterial hypertension, as well as hospitalization in the cardiac surgery department for planned surgical treatment of renal artery stenosis, which was thought to be the cause of the episodic increases in blood pressure. Lack of knowledge about the presence of autonomic dysfunction in spinal cord injury patients may have led to unnecessary surgical treatment. During the joint management of the patient with a pediatric cardiac surgeon, signs of autonomic dysreflexia were detected against the background of traumatic spinal cord disease at the level of the cervical spine, hypotensive treatment and planned surgical intervention were canceled. \u0000Conclusion. At the same time, the arsenal of tools that make it possible to objectively assess the possibilities of cosmetic and aesthetic therapeutic measures is very limited. \u0000Aims: Evaluation of the effectiveness of the compression microvibration method for the correction of structural and functional skin changes in women with gynoid lipodystrophy of different age groups \u0000Materials and methods: A single-center, non-randomized, prospective study of the effectiveness of the compression microvibration method was carried out in 27 women with mild to moderate gynoid lipodystrophy aged 40 to 69 years under the control of ultrasonographic skin examination before treatment, after 12 procedures and after 2 months. after a course of therapy. Additionally, anthropometric parameters were measured, and the body mass index was calculated. \u0000Results: The dynamics of BMI values revealed a trend towards weight loss in most of the participants. The circumference of the hips and waist in participants aged 40-49, 50-59 and 60-69 years decreased by 3.01.1 and 5.31.1 cm, 2.20.9 and 4.1 1.3 cm and 2.50.9 and 6.31.5 cm, respectively. After the course \"Endospheres Therapy\" in patients of these age groups in the areas of the anterior, inner, posterior thigh and anterior surface of the abdomen, the thickness of the dermis decreased by 15.6, 9.5 and 18.1%; 10.9, 10.5 and 19.3%; 6.1, 15.7 and 13.8%, 23.4, 14.7 and 14.5% (p0.05); increased echogenicity of the dermis in the areas of the anterior, inner, posterior thigh and anterior abdominal surface by 2, 1.4 and 1.3 times; 1.2, 1.4 and 1.2 times; 1.4, 1.4 and 1.5 times; 1.3, 1.5 and 1.7 times (p0.05); the echogenicity of the hypodermis decreased by 30.8, 27.6 and 31.2% (p0.05); 20.1, 37.3 and 40.4% (p0.05); 43.2, 57.8 (p0.05) and 9.3%; 23.6, 15.2 (p0.05) and 9.3%, respectively. The occurrence and importance of the post-COVID-19 sequelae was realized when a sizable proportion of patients appeared to continue suffering from various symptoms for many months and years after having recovered from the acute phase of infection. These complications were observed in multiple organ systems and not only in the respiratory tract. Multidisciplinary efforts are required to manage these patients as the complications are variable in terms of location and severity. \u0000The post COVID-19 condition (long COVID) represents a number of different post-viral syndromes that require an appropriate classification. Collection of a large amount of data is required for all the physical and neuropsychiatric symptoms that persist for more than 12 weeks without an alternative explanation. The process of data collection and analysis should be controlled for all confounding factors including the consequences of intensive care hospitalization, social isolation, and other effects. \u0000The current absence of the effective treatment reflects the unclear causes of the post COVID-19 conditions which cannot be targeted properly until their mechanism is established. It has been experimentally proven that rehabilitation based on virtual technologies is able to recreate a realistic perception and corresponding reaction in a patient, thereby improving the quality of cognitive and motor rehabilitation with the least cost. However, the success of such rehabilitation depends mainly on the technologies and techniques used by doctors. \u0000This study is devoted to the important problem application of virtual reality technologies in patients with acute impairment of cerebral circulation at the stage of early rehabilitation. We focus on the neurophysiological aspects of the VR technology application and the mechanisms of the brains neuroplasticity during application of virtual reality. The characteristic features of modern approaches to application of virtual reality are highlighted. Also, a novel concept is proposed for the modern VR-technology application in intensive care unit patients. Patients with severe traumatic brain injury often develop severe physical, mental, emotional and cognitive impairments, which are often difficult to correct and require long-term treatment, including rehabilitation, relying solely on the body's potential, which is accompanied by a heavy financial burden for the patient, his family, and society in general. The introduction of new diagnostic methods, the improvement of resuscitation measures, the use of modern methods of neurosurgical intervention and timely rehabilitation can increase the proportion of children who survived after severe traumatic brain injury and their quality of life. \u0000AIM: to evaluate the effectiveness of medical rehabilitation of children with severe traumatic brain injury with the participation of a multidisciplinary rehabilitation team and relatives of patients. \u0000MATERIALS AND METHODS: The study included 50 children with severe traumatic brain injury. Two groups were distinguished by the randomization method: the main group 25 patients (50%) and the control group, which included 25 patients (50%). The follow-up period was 1 year after the onset of severe traumatic brain injury. In the main group, an individual plan for early rehabilitation was drawn up on the 2nd day after a severe traumatic brain injury in intensive care. At the first stage of rehabilitation, which is carried out in the intensive care unit and the neurosurgery department, an individual medical rehabilitation plan was developed for the children of the main group, depending on somatic indicators and the severity of the condition, and a rehabilitation diagnosis based on the International classification of functioning, disability and health was established. In the control group, after transfer from the intensive care unit to the specialized neurosurgical department, patients received basic rehabilitation therapy: articular gymnastics, paraffin therapy, and psychological support. \u0000RESULTS: In the main group, more effective changes were noted in domains associated with impaired structure and function of the nervous system, which led to improved activity and participation, self-service. In patients of the control group, there were no significant changes in the state of functions, activity and participation. \u0000CONCLUSION: The early start of complex medical rehabilitation of children with severe traumatic brain injury reduces disability and accelerates the process of the patient's socialization, which is more clearly demonstrated by the International classification of functioning, disability and health approach. The rehabilitation diagnosis according to the ICF and the rehabilitation programme are presented. \u0000After COVID-19, the 52-year old patient suffered from general weakness and forgetfulness, she dropped objects (e.g., broke dishes), could fall, had an impaired sense of smell and occasional shortness of breath. She could not attend to herself and could not leave her home without help. The multidisciplinary rehabilitation course helped the patient to cope with all domestic tasks after 3 weeks, with the psychological support from her husband. The rehabilitation results according to the corresponding estimation scales show an improvement of the physical tolerance, cognitive functions and asthenia. Sheveleva","doi":"10.36425/rehab345206","DOIUrl":"https://doi.org/10.36425/rehab345206","url":null,"abstract":"The literature review covers an analysis of the typical protective and adaptive reaction mechanisms that develop in small rodents that spontaneously hibernate under the cold snap, together with warm-blooded animals and humans during circadian fall of the body temperature at night time and in a course of a slow-wave sleep, along with induced artificial therapeutic hypothermia. \u0000The general features of neuroprotection states development in natural endogenous and induced hypothermia are highlighted, which include metabolic, epigenetic and biophysical reactions that ensure the formation of nonspecific tolerance of the brain to potentially damaging effects. Significant attention has been devoted to the participation of hibernation proteins, opioids and antioxidant systems in the processes of safe exit from state of torpor in animals and in implementation of sleep restoration functions. Taking into account the circadian nature formation of endogenous brains hypothermia at night and in the phases of slow sleep, it is suggested that periodic temperature exposure on the cerebral cortex can be applied in order to restore the disturbed circadian rhythms. A successful treatment of chronic impairment of consciousness is formed from a comprehensive medical and multidisciplinary rehabilitation efforts aimed at restoring the consciousness, including a combination of various pharmacological and non-pharmacological methods. Modern rehabilitation techniques, with a competent approach to their development and application, not only are characterized by a significant potential in the context of improving the quality of life, but are also capable of providing a significant progress in alleviating the severity of the disease. At the same time, there are still no reliable methods in this area, which is a significant medical and social problem. \u0000AIM: Approbation and evaluation of the effectiveness of integrative sessions by a neuropsychologist and a speech therapist in the rehabilitation of persons in chronic critical condition, by the example of the Federal Research and Clinical Center of Intensive Care Medicine and Rehabilitology (FRCC ICMR). \u0000MATERIALS AND METHODS: 40 patients with diagnosed chronic impairment of consciousness were examined 20 patients in the main group and 20 patients in the control group. The rehabilitation work was carried out with the main group patients for 12 months, based on a developed system of integrated sessions of a neuropsychologist and a speech therapist. The evaluation of the rehabilitation work effectiveness was carried out at the time points of 6 and 12 months by monitoring the dynamics of improvement at the level of consciousness according to the CRS-R scale. \u0000RESULTS: We describe in detail the methodology and practical experience of integrated sessions of a neuropsychologist and a speech therapist in the rehabilitation of persons in chronic critical condition provided at the FRCC ICMR. A stable improvement in the level of consciousness, according to the CRS-R scale, was shown in patients with chronic impairment of consciousness, who underwent the integrated sessions of a neuropsychologist and a speech therapist. \u0000CONCLUSIONS: We have proven the effectiveness of an integrative approach to the rehabilitation of patients with chronically impaired consciousness, based on combined sessions of a neuropsychologist and a speech therapist. The patients with fractures of the bones of the lower extremities reaches for 8.525% of the total number of patients with fractures. Fractures of the bones of the lower extremities are characterized by long period of reparation, persistent contractures, a violation of the congruence of the articular surfaces, and changes in the biomechanics of walking. Only full course of rehabilitation can eliminate the these complications and restore the previous motor activity. \u0000АIMS: The study aimed to analyze the structure of injuries in patients of the traumatology department and to present and the results of the recovery of patients with fractures of the bones of the lower extremities using and algorithm of organization of medical rehabilitation in the case study. \u0000MATERIALS AND METHODS: A retrospective analysis of statistical data on 995 patients of the traumatology department № 1 of the Kazan City Clinical Hospital, who underwent 981 operations, was carried out. Randomly selected two groups of patients ― I main (n=45) and II control (n=45). Patients of group II after the treatment at the trauma center underwent outpatient medical rehabilitation, group I received inpatient medical rehabilitation in the rehabilitation department of the Kazan City Clinical Hospital № 7. In a hospital setting, a comprehensive rehabilitation program included physiotherapy exercises, CPM-veloergometry, exercises on rehabilitation simulators, the PNF kinesiotherapy technique, etc. \u0000RESULTS: A more pronounced statistically significant positive dynamics at the end of the course of medical rehabilitation was noted in patients of group I (reduction of pain syndrome, increased joint mobility, degree of dependence on others, increased mobility index, the ability to dress independently, maintain body position in space, reduced dysfunction in lifting and carrying objects, walking disorders, and caring for body parts). The dynamics of motor function and psycho-emotional sphere also improved to a greater extent in patients of group I. \u0000CONCLUSIONS: The effectiveness of the presented algorithm of organization of medical rehabilitation of patients with fractures of the bones of the lower extremities is confirmed by the results of present study.
