Limited research has investigated the patterns of bone remodeling in 360 degrees during the initial stages of implant healing. This study aimed to classify peri-implant marginal bone remodeling patterns at the implant uncovering stage. A total of 74 patients with 106 implants were included. At uncovering stage under an operating microscope, peri-implant bone conditions were recorded and categorized into four types: Type 1A (facial or lingual remodeling ≤ 2 mm with intact interproximal bone), Type 1B (facial or lingual remodeling ≤ 2 mm with interproximal remodeling ≤ 2 mm), Type 2 (circumferential remodeling ≤ 2 mm), and Type 3 (bone remodeling > 2 mm at any site). Results showed that 43% of implants exhibited no bones loss, while 57% presented various degree of bone remodeling, of which 58% Type 1A, 18% Type 1B, 17% Type 2, and 7% Type 3 in this specific cohort. Multinomial logistic regression identified ridge preservation history, simultaneous guided bone regeneration, and age as significant predictors of the bone remodeling variations. This new classification provides a framework for assessing early bone remodeling pattern and could serve as a foundation for future studies exploring the relationship between early remodeling, peri-implantitis risk, and long-term implant outcomes.
","PeriodicalId":14076,"journal":{"name":"International Journal of Implant Dentistry","volume":" ","pages":""},"PeriodicalIF":4.7,"publicationDate":"2026-04-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13187071/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147623003","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Three strategies were compared: IR, surgical endodontic treatment, and extraction with implant placement. The model used 6-month half cycles over 39.3 years with 3% annual discount rate. Transition probabilities were derived from systematic reviews and clinical studies. Costs were obtained from the American Dental Association (ADA) 2022 survey adjusted to 2025 USD. Dynamic computer-assisted implant surgery (d-CAIS) enhances surgical precision through real-time feedback; however, its accuracy and procedural efficiency when used by novice operators remain insufficiently investigated. This study evaluated how different registration methods in d-CAIS influence implant placement accuracy and procedural time when performed by novice operators.

Materials and methods: In this in vitro study, three registration methods were assessed (surface-based registration: ND, marker-based registration: XC, and pair-point registration: XM groups). Five novice operators, defined as dentists with less than five years of implant experience, placed 25 implants per group (75 total) in partially edentulous maxillary models. Postoperative cone-beam computed tomography scans were used to measure deviations at the implant entry point, apex, vertical depth, and angle. Statistical analyses were performed to compare group differences.

Results: Mean three-dimensional deviations at the implant entry point were 0.97 mm (ND), 0.68 mm (XC), and 0.72 mm (XM); at the apex: 1.33 mm, 0.78 mm, and 0.90 mm, respectively. Vertical depth deviations at the apex were comparable across groups: 0.62 mm (ND), 0.51 mm (XC), and 0.54 mm (XM). Angular deviation was highest in the ND group (3.16°) compared to the XC (1.18°) and XM (0.97°) groups, with significant differences observed between ND and both XC and XM. Average procedural time was longest in the ND group and shortest in the XM group, with statistically significant differences between ND and the other two groups.

Conclusions: In novice operators, d-CAIS enabled stable control of implant entry point position and insertion depth regardless of the registration method used. This study analyzed current treatment concepts, clinical practices, decision-making factors, and the adoption of advanced technologies in implantology in OMFS in Germany.

Material and methods: A dynamic online questionnaire with up to 38 questions was sent to 1391 OMFS members of the German Association of Oral and Maxillofacial Surgery (DGMKG). The questionnaire collected general and specific data, such as implantological experience, time points of implant placement (immediate vs. delayed), implant systems and designs, imaging modalities, digital planning, guided surgery, healing, (immediate) prosthetic restorations, follow-up, pre- and postoperative management, and the use of platelet-rich fibrin (PRF). Data analysis was descriptive and anonymous.

Results: 276 OMF surgeons participated in the study, with an average of 20 years of experience in implantology. Most worked in private practices without inpatient facilities (66.3%). Most placed 201-500 implants per year (34.78%). 78.99% performed immediate implant placement, mainly to shorten treatment time and preserve alveolar bone. Cone beam CT (88.42%) and panoramic radiographs (68.34%) were the most common imaging modalities. Virtual planning was used by 73.08%, and guided surgery was used by 66.54%, mainly with externally produced guides. Most participants preferred closed implant healing (83.53%). Immediate prosthetic restorations (PR) were rarely performed (57.83% never), and 70.92% did not carry out definitive PR. As the β-TCP content increased, the compressive strength decreased significantly. The average compressive strength C30 was 1.5 MPa. Penetration tests confirmed good permeability for C30 and C50. In the histological study, better new bone formation was observed in the C30 and C50 at 4 and 8 weeks.

Conclusion: These results suggested that the composite material made in this study had a shorter setting time and improved penetration than that in the previous study and confirmed its biocompatibility and osteoconductivity. Its compressive strength was low; therefore, it is necessary to examine whether the material can withstand the vertical and horizontal augmentation of the jawbone.
","PeriodicalId":14076,"journal":{"name":"International Journal of Implant Dentistry","volume":" ","pages":"8"},"PeriodicalIF":4.7,"publicationDate":"2026-02-04","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12901794/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146119013","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to evaluate and compare these two approaches to support clinical decision-making.

Methods: In this prospective, randomized controlled trial, 46 patients received a total of 48 implants, assigned to either an open healing group (n = 25) or a closed healing group (n = 23). Clinical and radiological assessments were conducted at three time points: T1 (pre-extraction), T2 (3 months post-op), and T3 (12 months post-op). The primary endpoint was the Pink Esthetic Score (PES), which assesses seven soft tissue parameters. Secondary endpoints included implant survival and volumetric tissue changes. PES was analyzed using a linear mixed-effects model.

Results: The open healing group showed significantly higher PES outcomes compared to the closed healing group (mean difference: -1.49; 95% CI: [-2.36, -0.62]; p = 0.0014). A higher baseline PES was also significantly associated with better esthetic outcomes at follow-up (0.3638, 95% CI: [0.1890, 0.5386], p = 0.0002). Smoking had no significant effect. Volumetric analysis revealed soft tissue volume loss from T1 to T2, with partial recovery by T3. Although the open healing group showed slightly less volume loss, this was not statistically significant. No implant losses occurred in either group.

Conclusion: The study demonstrated esthetic advantages of the open healing technique compared to the closed healing technique in immediate implant placement.
","PeriodicalId":14076,"journal":{"name":"International Journal of Implant Dentistry","volume":"12 1","pages":"2"},"PeriodicalIF":4.0,"publicationDate":"2026-01-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12847548/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"146052002","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
