Due to the rarity of this condition, standardization of definition as well as diagnostic criteria and treatment outcomes are lacking. A non-systematic review was conducted exploring the available literature on PubMed and EMBASE databases to identify publications related to the management of FUS between January 2019 and June 2024. Overall, 22 studies were identified. Diagnostic work-up appeared heterogeneous among authors: uroflowmetry (UFM), post-voiding residual of urine (PVR), (video-) urodynamic study (UDS), voiding cystourethrogram (VCUG), urethral calibrations (UC) and urethrocystoscopy (UCS) are the mostly employed tests. Primary treatment consists of urethral dilatation (UD) or urethrotomy. A variety of different reconstructive techniques are currently available such as meatoplasty, flap urethroplasty and graft urethroplasty. Outcome measures varied widely among authors. Patient-reported outcomes (PROMs) were assessed, with a focus on sexual function (FSFI), symptoms and QoL (AUA-ss, IPSS, PGI-I, ICIQ-FLUTS, ICIQ-FLUTSsex, ICIQ-S, UDI, SF-36), demonstrating general improvement. Success rates reported in literature appear to fairly agree amongst authors, but the variability of outcome measures reduces the significance of comparisons between studies. Efforts should be put into pursuing a homogeneity of definitions and criteria. PROMs are promisingly being employed in post-surgical evaluation and should be integrated into standardized follow-up protocols.</p>","PeriodicalId":14068,"journal":{"name":"International Journal of Impotence Research","volume":" ","pages":""},"PeriodicalIF":2.8,"publicationDate":"2025-05-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143985309","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In this follow-up study, 539 adult participants from Belgium, Germany, and the Netherlands took part and were categorized as following a 'traditional' care pathway (i.e., undergoing all transition-related interventions), an 'individual' care pathway (i.e. any course of treatment deviating from the traditional pathway), or 'no care' pathway (i.e. not seeking transition-related medical interventions.). We analyzed differences in demographic (e.g., gender identity) and clinical variables (e.g., treatment satisfaction), conducting logistic regression analysis and descriptive subgroup analysis. Participants with a non-binary gender were 6.7 times more likely to follow an individual care pathway, while participants with higher treatment satisfaction were less likely to follow an individual care pathway (Odds Ratio: 0.6). We identified four patterns of individual transgender care pathways, some as a function of the sex assigned at birth. While various treatment modalities exist, penile prosthesis implantation offers a permanent solution. Surgery, however, carries inherent risks and potential complications. The aim of this study was to determine the incidence and risk factors of early postoperative complications after penile prosthesis surgery in a large, multicenter, prospective, cohort study. Data was analyzed from patients participating in the PHOENIX registry, an international study on penile prosthesis surgery conducted by the EAU Research Foundation. Patients were operated on from November 2021 until august 2024 in 30 centers from 8 countries. Our study was not limited to one device or brand, all available inflatable and malleable devices were eligible for inclusion. Postoperative complications were registered at different time points, up to 2 years post-surgery. For this analysis we studied complications that occurred within 2 weeks after surgery. Data on early complications after penile prosthesis surgery of 1072 patients were analyzed. Median age was 62 (range 24-88) years old at time of surgery. A total of 639 (59.6%) patients received a drain perioperatively. A total of 49 patients (4.6%) reported complications within 2 weeks after surgery. Infection occurred in 4 patients and penoscrotal hematoma occurred in 6 patients within 2 weeks post-surgery. A total of 5 patients underwent early revision surgery. There was no significant effect of diabetes mellitus on development of an early postoperative infection (0.4% vs. 0.4%, p = 1.00). We found no significant impact of drain placement on hematoma development (0.3% vs. 0.3%, p = 0.69). We did find that early hematoma occurred more frequently in low-volume versus high-volume centers (1.3% vs. 0.1%, p = 0.02). It is important to predict the functional recovery of erections for better patient counselling and timely treatment of ED. This systematic review looks at imaging parameters to predict the recovery of erectile function (EF) after laparoscopic or robotic prostatectomy. A systematic search was performed to capture publications from January 2000 up to December 2023 (PROSPERO; Registrations ID CRD 42022359557). The considered studies applied an imaging parameter obtained by any form of imaging modality and in any operative phase (pre- or intraoperative) to assess the potential impact on EF after surgery. An essential criterion was a formal EF assessment at both baseline and postoperatively, by means of a validated questionnaire. A total of 8 studies met our inclusion criteria. We categorised the studies based on the imaging modality into three groups: MRI (n = 4), diffusion tensor imaging (DTI) (n = 2), and intraoperative ultrasonography (n = 2). Preoperative MRI parameters were, firstly, dynamic contrast enhancement of prostatic tissue measured as ratio of change of contrast from baseline to 120 s (Ratio120) showing weak correlation to postoperative IIEF5 scores (r = 0.31; p = 0.044), and secondly, area of neurovascular bundle (NVB) was a predictor of EF recovery on univariate analysis (odds ratio = 1.30; P = 0.001). Bony pelvic dimensions, prostate surface area, and fascial thickness measured on MRI did not show correlation with EF scores. Two studies using DTI were included, with one showing the change in the number of periprostatic nerve fibres before and after surgery, which also demonstrated a correlation with the change in EF scores (r = 0.35; P < 0.05). While the other study using DTI showed the change direction of the periprostatic nerve fibres before and after surgery measured as a value of functional anistropy, it showed a weak negative correlation to postoperative EF scores on the left (r = -0.66120; p = 0.0006) and right (r = -0.420068; p = 0.0456). Additionally, intraoperative ultrasound assessment of the NVB, such as the number of visible vessels within the NVB, has also been shown to correlate (r = 0.34, p = 0.0001) with postoperative EF. Our systematic review could not identify an imaging parameter strongly correlated with EF recovery. Enhancement of the prostate on mpMRI and intraoperative ultrasound might be worth investigating through better-designed studies. More research is needed to establish which parameters can reliably predict EF post-prostatectomy to best inform the patient and mitigate the risk.</p>","PeriodicalId":14068,"journal":{"name":"International Journal of Impotence Research","volume":" ","pages":""},"PeriodicalIF":2.8,"publicationDate":"2025-05-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"143985867","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":3,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
