Although transurethral resection of the prostate (TURP) remains an effective treatment for symptom relief, its negative impact on sexual function has driven increasing interest in minimally invasive surgical therapies (MISTs) that aim to preserve sexual outcomes. This narrative review summarizes contemporary evidence on urinary and sexual outcomes following major MISTs, including iTIND™, UroLift™, Rezum™, Aquablation, transperineal laser ablation (TPLA), prostatic artery embolization (PAE), and the drug-coated balloon system Optilume BPH™. Across studies, most modalities demonstrated reductions in the International Prostate Symptom Score of 7.8-17 points and improvements in maximum urinary flow rate of 3-12 mL/s, though the magnitude of effect and durability varied. UroLift™ and TPLA consistently demonstrated strong ejaculation-sparing effects, owing to their non-ablative or highly selective mechanisms. Rezum™ and Aquablation provided durable symptom relief with low rates of de novo erectile dysfunction and mostly transient ejaculatory changes. PAE offered excellent sexual preservation but comparatively less pronounced urinary improvements. Early data on Optilume BPH™ suggest sustained LUTS/BPH improvement with reliable maintenance of sexual function. This prospective cohort pilot study aimed to determine optimized settings in preparation of a randomized controlled trial to further asses its effects. Men without prior erectile dysfunction were instructed to use a mechanical medical vibrator daily for six months following surgery and to adjust settings according to preferences. The primary objective was the optimized parameters defined by the time spent using the vibrator, along with the most frequently used amplitude and frequency settings. Secondary outcomes included patient acceptance and settings most often inducing erections and orgasms. Twenty men were included, 19 of whom used the device on a median of 40.1% of days (IQR: 28,8-51,4%). PVS was most often performed for 5 min (IQR: 5-10 min) and the most frequently used amplitude and frequency were 1 mm and 90 Hz. These parameters were also the ones to induce erections and orgasms most often. At 6 months, 12/19 men were able to engage in penetrative sex. Fourteen patients reported satisfaction with the device, and no adverse effects were observed. Altogether, patient-optimized parameters consisting of 5 min daily stimulation at an amplitude of 1 mm and a frequency of 90 Hz were the most used and showed the strongest association with both erections and orgasms. This 15-year retrospective single-centre cohort study included 42 women who underwent surgical intervention for non-obstetric genital injuries at a tertiary center. Consensual sexual activity accounted for 85.7% of cases, and most of these occurred during first intercourse (80.9%). All the intercourse-related injuries were reported as consensual. Vaginal injuries comprised 78.6%; the median tear length was 2 cm. A hemoglobin drop of ≥2 g/dL was observed in 40.0%. For women with sex-related compared to trauma-related injuries, the median age was younger (26.5 vs. 49.0 years, p = 0.045) and the median parity lower (0 vs. 3.5, p = 0.003). The vagina was more often involved in sex-related than trauma-related injuries (83.3 vs. 50.0%, p = 0.065), and the vulvar less often (16.7 vs. 50.0%, p = 0.065). Arab women were generally younger, less frequently postmenopausal, and more often presented with a partner compared with Jewish women. Among sex-related injuries, Arab women more commonly sustained combined vaginal and vulvar injuries, whereas Jewish women presented predominantly with vaginal injuries. The findings highlight the potential importance of trauma-informed, culturally sensitive, and individualized clinical care. We conducted a cross-sectional patient survey of men who underwent penile prosthesis implantation at a tertiary centre between December 2023 and May 2025. The survey addressed glans engorgement, sexual satisfaction, ejaculatory function, cosmetic perception, and treatment interest. Associations were analysed using chi-square tests and univariable logistic regression. Eighty men participated (mean age 59.7 years (SD 9.3), median erectile dysfunction duration of 36 months (IQR 48). Soft glans was reported by 63.3% (n = 50/79), with another 17.7% (n = 14/79) experiencing it half the time or less. Most respondents (79%,n = 62/79) expressed interest in treatment. Soft glans was significantly associated with impaired ejaculatory function (p = 0.019), reduced sexual enjoyment (p < 0.001), and treatment interest (p < 0.001). Older age increased risk (OR 1.07, 95% CI 1.01-1.13, p = 0.014), while BMI, comorbidities, implant type, surgeon seniority, indication, and cosmetic satisfaction showed no association. Based on our single-center cohort analysis, soft glans appears to be a relatively common finding following penile prosthesis surgery. There is a paucity of available literature on glanspexy treatment as a definitive solution and patients' post-operative satisfaction. We conducted a retrospective review of patients who underwent glanspexy for real glans hypermobility (RGH) following penile implant insertion at a quaternary referral centre in 2012-2024. Demographic features, surgical techniques and post-operative outcomes were logged. We contacted patients via telephone to conduct a modified Erectile Dysfunction Inventory of Treatment Satisfaction questionnaire. Out of 3210 patients who underwent penile prosthesis insertion from 2012-2024, 40 underwent glanspexy. Glanspexy was performed at the time of primary implantation in 22.5% patients and as a secondary procedure in 77.5%. 67.65% of patients were satisfied and 77.78% felt erection shape was better post glanspexy. 76.47% felt the treatment met their expectations. 94.12% of patients agreed they would still choose to undergo the surgery again. Glanspexy done as a secondary procedure, in comorbid patients, after multiple implant revisions or genital/pelvic surgeries results in lower satisfaction. Four major device companies have successfully manufactured and marketed IPP devices worldwide, and these devices have pre-determined cylinder sizes and unique properties. It is recommended that the cavernosal body be dilated to at least 12 mm to accommodate a standard IPP cylinder and 10 mm for smaller diameter or narrow-base devices. The selection of cylinder size and type is often based on intraoperative total corporal length and dilation measurements; however, other factors such as the surgeon's expertise and preference, the presence of corporal fibrosis and/or the use of a rear-tip extender can influence the final cylinder selection. A properly sized cylinder that sits nicely within the corporal body and cycles well is the prerequisite for an optimal IPP function and a satisfied patient. Based on existing knowledge, a multimodal novel concept, the 3Ts Protocol, is presented, involving Tadalafil daily, Tunneling with intralesional biologics, and daily Traction therapy. It introduces a comprehensive, regenerative strategy that integrates pharmacologic, biologic, and mechanical interventions in a single sequence. Daily low-dose tadalafil improves cavernosal oxygenation and arterial inflow, counteracting hypoxia-driven fibrosis. Tunneling injections of biologics deliver growth factors directly into and around the plaque, promoting regeneration and remodeling, while traction therapy provides continuous mechanotransductive stimulation for structural realignment. Together, these components act synergistically to modulate inflammation, restore microvascular perfusion, and induce tissue regeneration. Importantly, this work provides, to our knowledge, the first fully specified, step-by-step description of a Peyronie's disease management protocol, enabling reproducible clinical implementation and facilitating meaningful comparison across future studies employing different biologic agents. Prospective randomized studies examining the efficacy of the 3Ts Protocol are needed.
