This study was a cross sectional quantitative study carried out to investigate the knowledge, attitude and perception of self-harm in pregnancy among 369 women attending antenatal clinics in a government owned hospital in southwest Nigeria. Respondents were selected using multi-stage sampling technique from ante natal clinics with the aid of semi-structured questionnaire to elicit information on their socio-demographic, knowledge, perception, and their attitude towards self-harm. Data were entered using SPSS version 25 and were analysed using descriptive and inferential Chi-square statistics at p =0.05. Respondent’s knowledge on self-harm in pregnancy was measured on 7 items and 7-point scale. Knowledge score of <2 was rated as poor and ≥7 was rated as good. Perception of self-harm in pregnancy was measured on 16 items and 16-point scale, score <8 was rated as negative and scores ≥8 was rated as positive. While respondents’ attitude toward self-harm was measured on 11 items and 11-point scale, score <6 was rated as negative and scores ≥6 as positive. Most respondents (85.4%) had a good knowledge of self-harm in pregnancy, good perception of (79.1%) and positive attitude of (85.9%). There should be public awareness and education campaigns on early detection, screening and counselling of pregnant women on the effect and risks of self-harm in pregnancy. Platelets play important role in the pathogenesis of thromboembolism. Large size platelets are more metabolically and enzymatically active also hemostatically more reactive and prone to the development of thrombosis. This study aims to determine the association between platelet index and the occurrence of DVT in patients with acute ischemic stroke. Method: Prospective study with a total of 42 samples acute ischemic stroke patients who were treated ≤ 3 days in the stroke corner of RSUP. H Adam Malik Medan. Compression ultrasonography to diagnose DVT was performed on the lower extremities. Platelet indices assessed were mean platelet volume (MPV), platelet volume distribution width (PDW), and plateletcrit (PCT) based on the results of a complete blood test on admission. Results: The mean age was 59.7 years with male gender as much as 66.7%. The majority of research subjects had a history of hypertension (42.9%) The mean body mass index was 28, 18 kg/mm2. 97.6% were found to have asymptomatic DVT with Well's score of 1 (moderate probability of DVT occurrence). 54.8% of the study subjects with positive DVT results based on compression ultrasound results. There is an association between increased MPV and the incidence of DVT in acute ischemic stroke patients (p = 0.002). Conclusion: There is an association between an increase in MPV value and the incidence of DVT in acute ischemic stroke patients. Moyamoya disease can manifest as a stroke in young adult. Methods: This report presents a young female patient with Moyamoya disease in Siloam Hospital Denpasar, Bali, Indonesia, diagnosed using MRI and Digital Subtraction Angiography. Case Report: A 29-year-old woman with recurrent stroke, presented with right-side hemiparesis, urinary incontinence, disinhibition syndrome, hypertension, and a history of left hemiparesis from a previous stroke. The MRI brain showed hyperintensity throughout the sulci (ivy sign) in the bilateral frontoparietal with acute infarct in the left frontomedial region, chronic infarct in right frontomedial, occlusion of bilateral carotid arteries (supraclinoid segment), bilateral MCA and ACA. The cerebral DSA study showed stenosis in the right ICA, stenosis in the M1-M2 junction with poor collateral to ACA territory with a puff of smoke appearance, occlusion in the left ICA (supraclinoid segment), and severe stenosis in M1. The patient had undergone direct bypass in the right MCA and right superficial temporal artery. Unfortunately, the bypass did not succeed because of infection of the bypass site and the patient got oral medication with a single antiplatelet (asetosal) and angiotensin receptor blocker (valsartan). Discussion: Moyamoya disease is a rare chronic cerebrovascular disorder that can manifest as an ischaemic stroke in young adult. This patient had a recurrent ischaemic stroke due to Moyamoya disease. According to a global multicenter stroke database, 90% of Moyamoya disease patients initially presented with an ischemic stroke, 7.5% with a transient ischemic attack, and 2.5% with a hemorrhagic stroke. DSA showed that the intracranial collateral system failed to fulfill the demand for brain circulation when the stroke occurred. The patient had failure to grow collateral after direct bypass surgery from the right superficial temporal artery to MCA and single oral antiplatelet as a secondary prevention for ischaemic stroke. Conclusion: Moyamoya disease is a rare disease that needs further studies for its treatment to halt the progression and prevent secondary complications. Young patients with ischaemic stroke needed to be asses with brain MRI, MRA, and DSA to evaluate the etiology of ischaemic stroke. This study aimed to describe a T2DM patient with extrapyramidal symptoms and hyperglycemia who was also on several antipsychotic therapies and to determine whether omission of all antipsychotics along with modification of antidiabetic therapy led to better glycemic control. Case Report: An uncontrolled T2DM female subject presented with tremor in both hands as well as in the right corner of her lips. She also having comorbid hypertension and hypothyroidism. On examination, she showed no signs of pallor, icterus, clubbing, cyanosis or edema. Central nervous system examination revealed a GCS of 15 and neurological examinations including motor and sensory responses were unremarkable in Nerve Conduction Study and MRI. her blood glucose levels showed poorly controlled diabetes (HbA1C – 9.5%, FBS – 278mg/dl, PPBS – 234mg/dl) in spite of taking OHAs. Her initial treatment plan included omission of all antipsychotics and reduction in dose of glimepiride to 4mg. This led to a drastic fall in the blood glucose levels. Her CBG levels were between 100mg/dl- 150 mg/dl. Soon after this, the dose of metformin was reduced to 1000mg, vildagliptin was omitted and a SGLT-2 inhibitor was added. This caused gradual improvement in her symptoms and subsequently the blood sugar levels and blood pressure were also within acceptable limits. Discussion: Diabetes and hyperglycaemic emergencies are linked to APDs. There may be several processes that mediate this. In insulin-sensitive cells such muscle cells, hepatocytes, and adipocytes, APDs can block the insulin signalling pathway, resulting in insulin resistance. When choosing a course of treatment, one must consider this elevated risk. Conclusion: Patients receiving multiple antipsychotics therapy are more likely to experience negative metabolic alterations. This case findings also highlight the importance of glycemic control in patients with acute or subacute onset movement disorders, irrespective of their past glycemic status. Criterions evaluated for each scheme are active and reactive power losses in whole system, voltage value at all buses, installation and operation cost and accumulative profit saving. Standard with and without transformer branches in distribution systems and Newton-Raphson method are introduced to make clear about the way to analyze power flows in whole system. Optimal placements and sizes for capacitor banks are determined by using a proposed algorithm and optimal capacitor placement tool in ETAP software. Simulation results can help to show the comparisons, evaluations and make decision about optimal capacitor placements and sizes. The contribution shows the best scheme that install capacitors at low voltage side of transformers because it has the lowest implemented capacity, highest economic factors while voltage quality is always in allowable range. Achieved results can be applied in any distribution system, help managers have overall review and decide optimal placements and sizes for capacitor banks.
