It is well known that some dermatological diseases are more common in children in comparison to other age groups. At least one out of 50 children have a kind of disability. The degree of their disability is crucial for their social life. We have researched if there are differences in the manifestation of dermatological diseases between children with disabilities and children without disabilities and the role of a disability in creating a dermatological disease. Methods: We have exanimated seventy-eight children with a dermatological disease. Half of them have a serious disability and the other half was the control group. Results: The most common dermatological diseases in the control group were the transmissible infections such as warts, impetigo, etc. The transmissible dermatological diseases were almost the 50% in the control group and only 10% in the group with the children with disability. The children with disability appeared more often with dermatitis and especially seborrhea dermatitis and less common with transmissible dermatological diseases. Conclusions: The social distance between the children with disability and the other children of the same age is well known. A proof of that fact is the limited percentage of the transmissible dermatological diseases in the group of disabled children. We have noticed that seborrhea dermatitis is more often in children with mental disability who take medical treatment. Seborrhea dermatitis in children with disability without medication is as common as in the control group.","PeriodicalId":13773,"journal":{"name":"International Journal of Clinical Pediatrics","volume":"102 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2021-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"77648876","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Seth, Praveen Kumar","doi":"10.14740/ijcp458","DOIUrl":"https://doi.org/10.14740/ijcp458","url":null,"abstract":"Background: Malnutrition in children < 5 years is a public health concern due to its associated high morbidity, mortality and serious long-term consequences. The World Health Organization (WHO) recommends 10 steps for the management of severe acute malnutrition (SAM) children which also involves correction of electrolyte disturbances. Treatment with diets having inadequate amounts of minerals like phosphate can result in refeeding syndrome and other abnormalities. We conducted the study among children with medically complicated SAM to evaluate their serum phosphate levels after receiving locally prepared therapeutic feeds and its associations of other factors. Methods: The study was conducted with 120 hospitalized SAM children aged 6 - 59 months with presence of other illnesses. All children were managed according to the WHO protocol and facility-based management of SAM guidelines of the government of India. Basic demographic details of the child were recorded. Weight of the child was recorded daily, while length/height and mid-upper arm circumference (MUAC) were recorded weekly. We evaluated changes in serum phosphate levels at three time points: on admission, during transition and at discharge. Binary logistic regression analysis was conducted to assess the association of hypophosphatemia with several other indicators. Results: Mean serum phosphate was 4.38 ± 1.07, 4.48 ± 1.16 and 5.13 ± 1.10 mg/dL on admission, during transition and at discharge, respectively. Hypophosphatemia was present among 30 (25%) children on admission, 28 (23.3%) children during transition and 10 (8.3%) children at discharge, which showed a decrease in the prevalence of hypophosphatemia from admission to transition and discharge using the current treatment. The finding also showed positive and significant changes in serum phosphate levels among children who were admitted without edema compared to those admitted with edema. Binary logistic regression estimates showed that older children, children with lower WAZ, hypokalemia, hyponatremia and anemia had higher odds of hypophosphatemia. Male children had lower odds of hypophosphatemia. Conclusion: The nutritional therapy with locally prepared starter and catch-up diet was effective in normalizing hypophosphatemia in the majority of children treated. Serum potassium and serum sodium levels were significantly associated with serum phosphate, indicating the need for careful monitoring of serum phosphate in the presence of hypokalemia and hyponatremia among SAM children. Clinical assessment of a child with a neck mass may need to be supplemented by diagnostic imaging. The optimal management of submandibular abscess is unclear, but an initially conservative approach is recommended. We report a case of a 10-year-old girl with a blunt injury 2 days following a fall who presented with a fever and a large lateral neck swelling. Inflammatory markers were raised and antibiotics were commenced. A magnetic resonance imaging (MRI) scan led to an initial working diagnosis of a submandibular abscess. Over time she remained systemically well, and an ultrasound could not radiologically differentiate infection from a collection. The final diagnosis was a submandibular post-traumatic hematoma with a secondarily infected collection. The initial management and decision to transfer to a higher level of service was significantly aided by the availability of MRI scanning. This case report illustrates the diagnostic and therapeutic dilemma in evaluating and treating acute mandibular swelling following blunt trauma. Tung","doi":"10.14740/ijcp422","DOIUrl":"https://doi.org/10.14740/ijcp422","url":null,"abstract":"Solitary rectal ulcer syndrome (SRUS) is a benign rectal disease that is rare in pediatric populations due to its underdiagnosis and misdiagnosis in children. It is often misdiagnosed as malignancy, Crohn’s disease, and ulcerative colitis due to its wide and varying clinical presentations. Both SRUS and inflammatory bowel disease (IBD) can present with rectal bleeding, constipation, diarrhea, and abdominal pain. Furthermore, macroscopic ulcers and inflammation can be seen in both diseases, making it difficult to diagnose without a biopsy. We present two cases in the pediatric population whose diagnoses of SRUS were delayed because the symptoms and macroscopic findings initially supported the differential diagnosis of IBD. These cases emphasize the difficulty and importance of differentiating between IBD and SRUS, and should encourage practitioners to include this differential diagnosis earlier on to improve diagnostic accuracy and begin implementing effective treatment. This can eventually decrease overall treatment time, unnecessary surgeries and diagnostic testing, and increase the emotional reassurance of the benign nature of the disease. A purulent drainage was observed in the left groin area. Staphylococcal scalded skin syndrome (SSSS) was suspected and the newborn was transferred to a pediatric intensive care unit where she started intravenous antibiotics and supportive care. During her stay in the intensive care unit, Staphylococcus aureus (SA) was isolated from the culture obtained from the left groin lesion. A culture from the mother’s milk, that did not show any signs of mastitis, was performed which yielded a pure growth of SA. The child improved after empirical treatment with antibiotic. SSSS is potentially life-threatening and thus its early recognition and prompt treatment are crucial to improve patients’ outcomes. Further studies are required to better understand the role of breast milk in the transmission of SA. During the assessment, she was observed to suffer repeated hyper-acute episodes of screaming associated with left-sided hemifacial flushing, lacrimation and rhinorrhea. This child was diagnosed with probable short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT), one of the trigeminal autonomic cephalalgias. We believe that this is the youngest probable case of SUNCT for which there is photographic and video evidence.
