Karpinska","doi":"10.15674/0030-59872024217-23","DOIUrl":"https://doi.org/10.15674/0030-59872024217-23","url":null,"abstract":"Hip arthroplasty is a reliable and quick way to relieve the patient of pain, restore movement in the hip joint and improve the function of the affected limb, but the question of how effective this intervention is in dysplastic or idiopathic coxarthrosis requires further study. The objective of the study was to evaluatethe effectiveness of treatment of patients with idiopathic and dysplastic coxarthrosis, and the effect of endoprosthetics on the work of the pelvic muscles. Methods. A comparative analysis of the treatment of 121 patients was conducted, which were divided into two groups — with dysplastic coxarthrosis(63 patients) and with idiopathic coxarthrosis (58 patients). The functional results of the treatment were studied according to the Harris scale, the data of stabilographic studies and the evaluation of the work of the muscles of the pelvic girdle, which are responsible for maintaining the horizontal balanceof the pelvis. Results. In general, the results of endopros thesis were evaluated as good in both groups. At the same time, the average results in the group of patients with idiopathic coxarthrosis are significantly better (t = 2.08; p = 0.001) than in the group of patients with dysplastic coxarthrosis. However, the indicators of the muscles that ensure the horizontal balance of the pelvis, in most cases, remain reduced within 6–8 months after the endoprosthesisoperation. This is evidenced by clinical data and statistical research data. Conclusions. Endoprosthesis of the hip joint is an effective method of coxarthrosis and allows to significantly improve both the patientʼs support and kinematic function and his quality of life. The results of treatment of patients withidiopathic coxarthrosis according to the Harris scoring scale are significantly better than the results of treatment of patients with dysplastic coxarthrosis. Objective. To analyze the results of the use of Symeʼs operation in patients with gunshot injuries of the foot, to highlight the important points of surgical technique, rehabilitation and prosthetics. Methods.The results of treatment of 12 men with gunshot wounds to the foot aged 23–54 years (mean age (36.1 ± 2.7) years) who underwent Syme surgery were analyzed. All patients were military personnel at the time of injury. Clinical, instrumental and microbiological examinations were used. The severity of the injury and the prospects of reconstructive treatment were assessed by the Hannover Fracture Score (HFS-97), the anatomic defect present and the severity of static-dynamic disorders. The results were evaluated according to VAS (pain) and ALAC (prosthesis using). Results. Healing of postoperative wounds in all cases occurred without complications. The observation period was from 8 months. up to 7 years (3.5 years on average). All 10 prosthetic patients use a prosthesis: 1 — walking on a prosthesis outside the home (ALAC IV), 5 — independent mobility (ALAC V), 4 — mobility does not differ from the norm (ALAC VI). In two cases (15,4 %) due to pain syndrome (7,3 and 6,8 according to VAS) prosthetics was impossible — reamputation at the level of the leg. Conclusions. Syme’s operation provides a good functional result of prosthetics in case of gunshot injuries of the foot, is a real alternative to transtibial amputation in cases with preserved hindfoot. Comorbidity of one of these diseases is directly and significantly associated with an increased risk of developing another. Objective. Carrying out a critical analysis of the results ofstudies related to the relationship between CVD and OA, as well as an assessment of the possibilities of their joint prevention and treatment. Methods. Publications from the Google search system, electronic databases PubMed, Scopus, Web of Science and other relevant sources of scientific and medical information were analyzed. The results. The main pathogenetic explanation of the relationship between CVD and OA is the presence of systemic,slowly progressing inflammation, which becomes especially important in patients of older age groups. The similarity of the composition of pro-inflammatory cytokines in the development of both CVD and OA enhances pathological changes in the structure of comorbidity. CVD and OA share common pathological mechanisms, such as oxidative and metabolic stress, molecular factors of endothelial dysfunction, hyperlipidemia, and systemic and local vascular remodeling. At the same time, it was established that OA develops against the backgroundof CVD risk factors and progresses along with their accumulation. Special care should be taken when prescribing non-steroidal anti-inflammatory drugs (NSAIDs). The appointment of systemic forms of NSAIDs is not recommended for patientswith high and very high cardiovascular risk. Conclusions. In patients with both conditions, the risk of one is directly related to an increased risk of the other. Further study of the role of comorbiditiesin the pathogenesis of OA will expand the understanding of the integration of cardiovascular risk factors. Violation of muscle function in the case of a limb injury can be both a consequence of denervation — as a result of damage to peripheral nerves, and ischemia with subsequent necrosis and fibrosis — as a result of direct or indirect damage to muscles, «tenotomy syndrome» — in case of traumatic damage to tendons and muscles yams, with the simultaneous combination of denervation and ischemia. Markers for assessing the state of muscle tissue — levels of Na+, K+ and Ca++ electrolytes, indicators of kidney function (urea, creatinine); glucose and the products of its transformations — lactate (lacticacid) and pyruvate (pyruvic acid) and, finally, muscle enzymes, namely creatine kinase (CK) and lactate dehydrogenase (LDH). Objective. To study the biochemical indicators of blood (activity of energy metabolism enzymes and electrolyte content) of military personnel with ischemic contracture and muscle denervation after a mine-explosive injury. Methods. The blood serum of 45 people, who were divided into IV groups, was analyzed. The average age of patients of the II group was 33.75 ± 3.46; III — 37 ± 3.66; IV — 35.86 ± 2.35. The results. The activity of LDH and CK and the content of electrolytes Na+, K+, Ca++ were determined in blood serum. Conclusions. LDH plays a key role in lactate metabolism and maintenance of energy balance in skeletal muscle. Its increased activity indicates the presence of hypoxia due to ischemic contracture of the limb and damage to peripheral nerves. KK enzyme protects cell membranes from damage. The tendency to decrease the activity of CC is a negative sign that reflects the initial process of destruction of the cell membranes of muscles damaged by a mine-explosive injury. Results. The final is established diagnosis: gunshot fragment blind penetrating wound chest on the right with a gunshot fracture of the right scapula,III–V ribs on the right, damage to the right lung with the presence of a metal fragment paravertebrally on the right at the level ThV vertebra, right-sided hemopneumothorax. right-sided post-traumatic pneumonia, spinal cord injury, lower paraplegia. The operation was performed: video-assisted thoracoscopy on the right, revision of the pleural cavity, removal of a metal fragment from the body of the ThV vertebra, sanitation and redrainingof the right pleural cavity according to Bulau, which lasted 40 min. In the future, after 2 weeks, the stitches and signs were removed neurological deficit regressed, the wounded was sent to the military medical commission and discharged to parts. Conclusions. Gunshot wounds to the chest with spinal injuryare rare. The use of modern magnetic surgical instrument allows to remove ferromagnetic foreign bodies during vertebral injury. Fixation of the AEC to the acromial process of the scapula is a priority. Competing metal structures are the hook plate and the Weber method. The significant disadvantages of using the Weber method of fixation are the migration of the tips and the violation of their integrity andthe wire. Objective. To conduct a comparative analysis of the fixation of the acromial end of the clavicle according to Weber, hook plate and the proposed construction, by studying the stressed-deformed state of the clavicular-acromial joint in case of damage to the lig. acromioclavicular superior and various methods of fixation. Methods. A finite-element model of the clavicular-acromial joint was constructed. Damage to the ligaments was modeled. claviculo-acoacromiale superior, as well as fixation of AEC in three ways: according to Weber, hook plate and the proposed construction. Results. The best results, from the point of view of reducing the level of stresses in intact ligaments, are provided by fixation of the AEC according to Weber, but its use leadsto an increase in the level of stresses on the AEC and the acromial process of the scapula, which can cause destruction of the latter. The hinge-type fixator provides the best stress distribution, both in the bony elements of the model and in intact ligaments. The hook plate holder occupies an average position,both in terms of the level of stress and the magnitude of the relative deformations in the ligaments. Conclusions. Weber fixation provides the best results for reducing the level of stresses and relative strains in the intact ligaments, but leads to a several-fold increase in the level of stresses on the AEC and acromial process of the scapula. The emphasis on optimal sizing and alignment of the components has led to an increase in the use of tools that allow for preoperative planning and verification of intraoperative steps. Computer navigation and robotic surgery have emerged as valuable tools for planning and performing surgery with greater precision and consistency. Objective. The aim of this paper is to organise information on the use of robotic systems in total knee arthroplasty based on own personal experience and analysis of contemporary literature sources. Methods. This study analysed professional articles that discussed the advantages and disadvantages of using robotic systems during knee arthroplasty. The information was obtained from electronic databases including PubMed, Scopus, Web of Science and Google Scholar, with a search span of over 20 years. Computerised or navigation devices allow the surgeon to enter anatomical data via an interface and receive feedback on the alignment of the implant and the knee as a whole, but cannot be programmed to perform additional tasks. Currently, several patented systems are available, and rapid technological advances in computer processing power have allowed for the rapid development of robotic surgical systems. Robotic systems usually provide feedback similar to navigation systems, but they can also be programmed to assist in specific surgical tasks. It is expected that these systems will become more reliable and accurate in the future, potentially leading to a reduced role for physicians in certain aspects of the surgical process, limiting their involvement to supervision, and thus improving the workflow of the operating room. The integration of new technologies, such as mixed reality, which overlays simulated images on real-life images, is expected to further expand the range of capabilities of these devices. Materials and various techniques for replacing bone defects of the limbs are described. Goal. Conduct an analysis of surgical technologies and materials for the replacement of bone defects, by analyzing literary sources. Methods. In three reputable databases (PubMed, Scopus, and Web of Science), an analysis of the latest scientific studies devoted to the treatment of defects of long tubular bones in the period from 2015 to 2022 was carried out. The search was carried out using the keywords \"bone defects\", \"autograft\", \"allograft\", \"Ilizarov method\", \"Masquelet method\", \"biocomposite materials\". The results. Analysis of osteoinductive and osteoconductive properties of auto- and allografts, modern biocomposite materials. Established advantages and disadvantages. The contemporary analysis of literary sources does not provide an objective comparative assessment of the effectiveness of the treatment of bone defects according to the Masquelet and Ilizarov method due to the lack of a sufficient number of randomized studies, which is the basis for further targeted research. Conclusions. Treatment of bone tissue defects of various genesis is an urgent problem of modern orthopedics and traumatology. The wide range of treatment options is proof that no single strategy works for every patient, just as there is no perfect universal material to fill and ensure bone regeneration in the defect site.
