Delay in the appropriate treatment may result in acute airway diseases and fatal complications. The aim of this manuscript is to report a clinical case of dual migration of an organic foreign body from left to right and then back to the left bronchus in a 13 months old child. The case was initially diagnosed as left sided severe pneumonia which recurred after initial partial response to medical management and thus raised the suspicion of a foreign body. Computed tomography of the chest revealed a foreign body in the right bronchus contrary to clinical findings on the left side suggesting the first migration. They usually present as obstructive submandibular sialadenitis. The diagnosis can be challenging because of rarity of such an event, and inability of traditional diagnostic methods to detect the foreign body. Due to its protected location under the mandible and only penetrating injuries to the floor of the mouth or trauma underneath the mandible can reach and damage it. Hence, submandibular gland injury is a very rare incidence that has been documented only in case reports.To approach an early diagnosis, a patient history needs to be believed. The second aim of the study is to compare the speech intelligibility between hemiglossectomy patients who have undergone tongue reconstruction using pectoralis major myocutaneous with those hemiglossectomy patients who had undergone tongue reconstruction using radial forearm free flap.Fourteen patients who have undergone tongue reconstruction following hemiglossectomy as a treatment for oral cancer between the age of 30 to 60 years were taken up for this study. Tamil Articulation Test was used for assessing the articulatory functions of patients. The speech intelligibility of each patient was assessed using the Ali Yavar Jung National Institute for the Hearing Handicapped intelligibility rating scale. Analysis of articulatory errors revealed linguoalveolar consonants were more impaired when compared to other consonants. Patients with radial forearm free flap had somewhat better speech intelligibility compared to patients with pectoralis major myocutaneous flap reconstruction.The type of reconstruction also impacts the speech intelligibility.
