Ghayour-Mobarhan","doi":"10.22038/JCTM.2021.58525.1338","DOIUrl":"https://doi.org/10.22038/JCTM.2021.58525.1338","url":null,"abstract":"Introduction: Metabolic syndrome (MetS) is characterised by a clustering of metabolic cardiovascular disease (CVD) risk factors that include: central obesity, impaired glucose metabolism, dyslipidaemia (hypertriglyceridemia, increased low-density lipoprotein [LDL], decreased high-density lipoprotein [HDL]) and hypertension. Paraoxynase-1 (PON1) is a plasma HDL associated protein that inhibits the oxidation of other lipoproteins. There is some evidence of reduced enzyme activity in subjects with metabolic syndrome. Curcumin is a natural polyphenol, it has been reported that curcumin has beneficial effects on several metabolic syndrome associated parameters. The aim of this study was to evaluate the effect of phospholipid complex of curcumin as an antioxidant on the activity of this enzyme in subjects with metabolic syndrome.Materials and method: A double-blind randomised control study was undertaken in 80 patients with metabolic syndrome. Subjects in the intervention (n = 40) were given capsules of phospholipidated curcumin (1 g/day) for a period of 6 weeks. The control subjects (n = 40) received a placebo. Fasting blood samples of each person were obtained during the start and the end of the study. Paraoxonase activity was measured using a PON1 fluorescence Paraoxonase Assay Kit and Arylesterase activity was measured by photometeric method using a UV-Visible spectrophotometer.Results: Serum paraoxonase enzyme activity did not change significantly between the curcumin treated group and the control group before the intervention (0.54 ± 0.18 U/µl versus 0.49 ± 0.14 U/µl; P > 0.05), nor did serum arylesteras activity change significantly between two study groups (152.67 [46.60 to 916.05] U/µl versus 129.77 [55.34 to 344.78] U/µl; P > 0.05). Reintubation is described as the failed extubation or patient get intubation after extubation who had been initially tracheal intubated.The purpose of the study was to determine the Spiro metric parameters among post Extubated patients and the application of Non-Invasive Ventilation for - prevention of reintubation. Materials and Methods: Quantitative research approach with purposive sampling technique was adopted to include 38 participants. Data were collected by providing spirometry immediately after extubation. The data were analyzed using descriptive statistics. Results: The result shows that 36.84% of participants required non-invasive ventilation after extubation who were having FEV1 between 0.38-1.48, FVC between 0.44-1.75, PEFR between50-70. Hemodynamic variable like saturation (0.01), PCO2 (0.00),PO2 (0.03), and HCO3 (0.00) were highly significant at the level of p < 0.05. Proving that patients whose saturation level& ABG profile like PCO2,PO2, and HCO3 are not maintained required NIV after extubation. We presented 1 cases of traumatic carotid–jugular fistula. A 52 years old man that was referred to us because of an expanding neck hematoma. There was a 1×1 cm ulcer with a clot in zone II right neck and a thrill was palpated. We clamped the proximal and distal site of injury (Arteriovenous fistula), then divided the fistula and primarily repaired the artery (transversely) and the vein with 7-0 prolene suture. We placed two hemovac drains and then repaired the subcutaneous and skin and covered the site with gauze and then the patient was transferred to ICU. He was extubated the next day and physical exam was completely normal without any neurologic deficit. The Percutaneous Coronary Intervention is the gold standard approach, but in many patients this procedure is not successful. Cardiac Rehabilitation and Secondary Prevention Programs has showed improve the patient's ability to perform physical effort by its positive effect on endothelial function and promote angiogenesis, increasing the ischemic threshold. We evaluate the Cardiac Rehabilitation and Secondary Prevention program effect on myocardial performance and ischemic profile in successful and unsuccessful Chronic Total Occlusion Percutaneous Coronary Intervention patients.Method. A non-randomized clinical trial was conducted in patients with CTCO underwent to Percutaneous Coronary Intervention (PCI). Patients were divided into two groups: 1) With successful PCI and 2) With unsuccessful PCI. All patients underwent a Sestamibi-Dipyridamole cardiac scan and stress test before and after of CRH&SP. The cardiac rehabilitation program considered 4-6 weeks of 5 weekly 30-minute training sessions with aerobic at 70% of Heart Resistance Reserve (HRR), with interspersed 3 weekly strength training sessions, as well as nutritional and Psychiatric group interventions.Results. We evaluated 25 patients with successful PCI (n = 13) and unsuccessful PCI (n = 12). For both groups, the CRH&SP showed significant improvement (p <0.05) in myocardial performance parameters, ischemic profile, and physical effort tolerance, with a Cohen's Delta ≥ 80% in Nuclear Medicine Risk, NYHA functional class, METs reached, oxygen consumption, Myocardial Efficacy Index and Duke Score. Conclusions. We have examined the effects of curcumin on HDL cholesterol uptake capacity in subjects with obesity. Methods: 30 obese individuals received curcumin and placebo 1 g per day for a period of 30 days. The subjects were crossed over to the alternative regimen after a 2 week wash-out period. A modified cholesterol uptake capacity (CUC) assay was used to determine serum HDL functionality.Results: The study groups had similar base line characteristics. We did not find significant effects (p>0.05) of curcumin on serum HDL CUC levels. Material and Methods: In this retrospective study, all patients who underwent Stenting in the catheterization department of ShahidMadani Hospital between 2015 and 2018, were included. Angiographic and angioplasty findings were recorded. The primary end point, which includes total mortality, myocardial infarction, revascularization (adverse events) were recorded. Results: The mortality rates were similar in both groups. Moreover the myocardial infarction and repeated revascularization did not differ in both groups (p>0.05).The rates of adverse events wasn't significantly different between the two groups. Therefore, once diagnosed, it should be surgically resected. Atrial myxomas are present about 75% in left atrium (LA) and about 15% in right atrium (RA). Early diagnosis is a challenge because of nonspecific manifestations, and sometimes is asymptomatic and may be discovered accidentally during transthoracic echography (TTE).Minimally invasive cardiac surgery (MICS) has benefits over sternotomy include cosmetically, less pain, and shorter total hospital stay. Materials and Methods:Between January 2011 to December 2020, (50) patients [30 Sternotomy, 20 MICS] underwent surgery for isolated resection of cardiac myxoma. We reported outcomes; cardiopulmonary bypass (CPB) time, aortic cross-clamp (ACC) time, conversion to median sternotomy (ST), total hospital stay, complications (stroke, renal failure, respiratory failure, reoperation, and infection), pain, patient¢s satisfaction, recurrence and survival. Follow-up time was from 6-months to 3-years. Results: There is no significant difference in CPB or cross-clamp time between groups. No minimal invasive (MI) cases required conversion to a median ST. Total hospital stay is shorter in the MI group by 2.2 days (p-value = 0.045). No differences present in morbidity or mortality between two groups. The improper implementation of inhaler techniques thatare used in such patients leads to the reduced effect of medicines. This study was conducted to evaluate the correct use of various inhalers among COPD patients. Materials and Methods:This observational, cross-sectional study was carried out on 96 patients with COPD aged over 40 years. The samples were selected using asystematic random samplingmethod from patients with COPD referring to the clinics of Ghaem and Imam Reza hospitals, Mashhad, Iran, from March 2018 to March 2019. The subjects were informed that their participation in the study was voluntary. These cases were under the treatment of using at least one inhaled medicine for a month or more. The adopted technique of applying four types of inhalers was evaluated by a standard checklist. The patients' performance scores of all procedures were recorded, and the collected data were analyzed in SPSS software (version 16). Results: Our study revealed that more than 98% of patients used metered-dose inhaler (MDI) spray (P=0.05). The patients' scores on the correct use of MDI, Diskus, Turbuhaler, and HandiHaler inhalers were estimated at 68, 77, 87, and 90%, respectively. The most common mistakes in using MDI and HandiHaler inhalers were related to the 'holding the breath' and \"taking a deep inhale' steps after using the inhaler, respectively. Conclusion: Physicians must evaluate and modify the use of inhalers in every COPD patient.
