Mandreoli, Rossella Giannini, Sara Dominjanni, Silvia Porreca","doi":"10.33393/gcnd.2024.3026","DOIUrl":"https://doi.org/10.33393/gcnd.2024.3026","url":null,"abstract":"Chronic kidney disease (CKD or CKD [Chronic Kidney Disease]) is present in approximately 7% of the world population: several studies have highlighted socio-cultural discrimination, to the detriment of women, in referral to specialist nephrological care and access to dialysis and transplantation. Globally, gender discrimination limits the possibility of access to education, medical care and involvement in clinical trials. Women on dialysis have different comorbidities than men; the choice to follow a predialysis process and the subsequent orientation towards dialysis treatment are certainly influenced by gender as is the choice of dialysis access. As regards kidney transplantation, women are more likely to offer themselves as donors rather than to be beneficiaries. Moreover, it allows adherent patients to delay the onset of dialysis, leading to improved quality of life and cost savings for both patients and the community. Through a survey, we aimed to evaluate how personalized diets were assessed, administered, and experienced by CKD patients. A questionnaire was administered to 180 patients from 4 nephrology Centers in Lombardy, (Italy,) regarding their CKD and nutritional therapy. 73% of patients received dietary prescriptions. In 40% of cases, dietary prescriptions were administered in dedicated clinics and were valued as much as pharmacological ones. Most diets prescribed were low-protein (0.8g protein per kg of body weight), although some included very low protein diets supplemented with keto analogs. Unfortunately, post-initial prescription, monitoring adherence to nutritional therapy is not particularly frequent. In January 2023, Dapagliflozin became the first SGLT2 inhibitor approved by the Italian Medicines Agency Administration (AIFA) for the treatment of chronic kidney disease (CKD) regardless of diabetic status. However, although these agents have received considerable praise for their cardiovascular and nephroprotective effects among patients with native kidney disease, the safety and efficacy of SGLT2i in the kidney transplant setting are not well-known as safety concerns have led to the exclusion of transplant recipients from all large RCTs. Although there are conflicting data, among males affected by SLE a higher proportion develops lupus nephritis, with a more severe histological and clinical pattern than in females. More frequently males also present an antiphospholipid syndrome. Males are less likely to achieve clinical remission of lupus nephritis. A sex and gender disparity is also present in all the phases preceding the kidney transplant from the referral for eligibility to the registration on the active waiting list. There is a perception among healthcare staff that women have a greater degree of fragility and are therefore less eligible for kidney transplantation. Women also encounter obstacles in completing the tests necessary for inclusion in the waiting list and in many Health Care contexts they are less likely to have a transplant. In the field of living transplantation, women represent over half of all donors, while males are predominantly recipients. This requires a complex process of psychological adaptation which often leads to the manifestation of two important expressions such as anxiety and depression. Music in support of conventional treatments promotes the care of the human being in all its aspects: physical, psychological, emotional, cognitive, social and spiritual as it represents a sort of “escape”, and allows a detachment from reality, albeit momentary. It acts as a regulator of emotions and mood, also creating beneficial effects on hemodynamic parameters such as blood pressure and heart rate. Furthermore, musical stimulations are able to stimulate pleasure by acting directly on the neuroanatomical structures responsible for attention, learning, thinking and behavior.
