Commonly, the first movement of the sonata for two pianos KV 448 was played with many repetitions. At least a fifth of the patients achieved a reduction of seizure frequency of more than 50%. Patients with epileptic discharges from the occipital lobe responded very rarely. {"title":"[Ofatumumab in the treatment of multiple sclerosis - A summary of preclinical and clinical data].","authors":"Juliane Klehmet, Tobias Bopp","doi":"10.1055/a-2365-0966","DOIUrl":"https://doi.org/10.1055/a-2365-0966","url":null,"abstract":"<p><strong>Background: </strong> B-cell targeted therapies are highly effective in multiple sclerosis (MS). Most of these therapies are administered intravenously at long intervals. Ofatumumab, an anti-CD20 antibody that is administered subcutaneously at low doses on a monthly basis due to its high affinity to the target structure, became available for the treatment of MS in 2021.</p><p><strong>Methods: </strong> An overview of practice-relevant immunological and clinical data on ofatumumab is provided.</p><p><strong>Results: </strong> The high affinity of ofatumumab to the target structure allows low dose and low volume administration, with the release and absorption profile after subcutaneous application allowing for high concentrations in the lymph nodes and gradual depletion of B-cells. Rapid onset of action is achieved as well as B-cell repletion within a few months in case of discontinuation of therapy. Long-term data show stable IgG levels over up to four years and high efficacy with respect to relapse rate, progression, and cognition. According to current study data, the effect compared to teriflunomide is greater the earlier therapy is initiated. Ofatumumab has a specific B-cell depletion pattern. CD20 expressing B-cell progenitor cells in the bone marrow are preserved and therefore also the inducibility and differentiation of plasma cells. The formation of a humoral immunological memory is therefore possible. Four-year study data showed no abnormalities in the rate of severe infections or malignancies.</p><p><strong>Conclusions: </strong> Ofatumumab is an innovative B-cell targeted therapy. The aim of this study was to examine if and how many early career police officers had already experienced potentially traumatizing situations and how many met either subsyndromal or complete PTSD criteria. A further subject of interest was if the officers knew the concept of psychosocial emergency care for first responders (PSNV-E) and if such support was made use of.</p><p><strong>Method: </strong>Early career police officers (<i>n</i>=221) were assessed via an online survey about their posttraumatic stress symptoms.</p><p><strong>Results: </strong>More than half of the participants (<i>n</i>=121) reported having experienced at least one traumatic deployment. PTSD prevalence in this group was at 1.7%, an additional 14.9% showed partial PTSD. One-fifth did not know about the PSNV-E concept.</p><p><strong>Conclusion: </strong>Police officers are confronted with a range of extremely stressful events early in their career leading to first symptoms of PTSD in some of them. Early prevention strategies as well as the identification of those affected for secondary prevention is of very high relevance for long-term mental health.</p><p><strong>Hintergrund: </strong>Polizeibeamt:innen werden in ihrer beruflichen Karriere oft mit traumatischen Einsätzen konfrontiert und weisen im Vergleich zur Allgemeinbevölkerung ein erhöhtes Risiko eine posttraumatische Belastungsstörung (PTBS) zu entwickeln auf. Ziel der vorliegenden Studie war, zu untersuchen, ob und wie häufig Berufsanfänger:innen der Polizei bereits (potenziell) traumatische Einsätze erlebt haben und wie häufig eine PTBS oder eine subsyndromale Ausprägung der PTBS vorliegt. Ferner war von Interesse, ob ihnen das Konzeptzur psychosozialen Notfallversorgung für Einsatzkräfte (PSNV-E) bekannt ist und ob diese Angebote in Anspruch genommen werden.</p><p><strong>Methode: </strong><i>N</i>=221 Berufsanfänger:innen der Polizei wurden im Rahmen einer Online-Befragung zum Erleben potenziell traumatischer Einsätze befragt. Zudem wurde mit der PCL-5 das Ausmaß posttraumatischer Belastungssymptome erhoben.</p><p><strong>Ergebnisse: </strong>Über die Hälfte der Befragten (<i>n</i>=121) hatte mindestens einen (potenziell) traumatischen Einsatz erlebt. Die PTBS-Prävalenz lag in dieser Gruppe bei 1,7%, weitere 14,9% wiesen eine partielle PTBS auf. Einem Fünftel der Befragten war das PSNV-E Konzept nicht bekannt.</p><p><strong>Schlussfolgerung: </strong>Bereits in den ersten Berufsjahren sind Polizeibeamt:innen mit extrem belastenden Einsätzen konfrontiert, woraufhin ein Teil von ihnen erste Anzeichen einer PTBS aufweist. {"title":"[Intrajejunal levodopa in Parkinson's disease: Optimization of PEG application].","authors":"Karl-Ernst Grund, Annette Zipfel, Wolfgang H Jost","doi":"10.1055/a-2337-3433","DOIUrl":"https://doi.org/10.1055/a-2337-3433","url":null,"abstract":"<p><p>The various forms of Percutaneous Endoscopic Gastrostomy (PEG) are highly relevant in neurology, as pump-administered intrajejunal levodopa application is one of the indispensable forms of therapy in advanced Parkinson's disease. Optimal PEG placement and follow-up are therefore significant for the success of the therapy. However, the standard intrajejunal administration of levodopa gel via a JET-PEG, i. e. a PEG with an internal catheter inserted into the jejunum, is not without problems for various reasons. In particular, the considerable cumulative complication rates demand a reconsideration of the situation. The very limited absorption area of the drug in the region of the flexura duodenojejunalis must also be taken into account. Causes of complications are predominantly a non-optimal application technique of PEG and internal catheter as well as the frequent lack of an adequate follow-up. In this paper, the details of a modified and optimized application technique compared to the conventional techniques are presented. These new methods have proved their usefulness in clinical applications for years, and additionally a new application form, the Hybrid-PEG, is presented. However, many of the details derived from anatomical/physiological, surgical and endoscopic aspects must be strictly observed during the application in order to reduce or avoid minor and major complications. In particular, problems are caused by local infections in the area of the insertion point of the PEG including peritonitis, leaks and buried bumper syndrome (BBS). The relatively frequent dislocations of the internal catheter also prove to be particularly troublesome. These can ultimately be avoided by clip fixation of the catheter tip down in the jejunum. In particular, the use of the newly developed Hybrid-PEG, a combination of endoscopically controlled gastropexy with three sutures and subsequent central thread-pull-through of the PEG tube, can significantly reduce the complication rate and thus achieve a decisive improvement for patients. The aspects discussed here are highly relevant for all those involved in the therapy of advanced Parkinson's disease. The objective was to provide a scientific basis for developing psychological interventions tailored to this specific population.</p><p><strong>Methods: </strong> The research was conducted from May 2021 to July 2022, involving the recruitment of 200 women attending maternal and child health clinics for their late-pregnancy checkups. Data collection was carried out using a comprehensive approach, involving several validated tools. The participants completed a general demographic and sociological questionnaire along with four standardized psychological assessment scales: the 12-item General Health Questionnaire (GHQ-12), the Athens Insomnia Scale (AIS-8), the Generalized Anxiety Disorder 7 (GAD-7), and the 9-question Patient Health Questionnaire (PHQ-9). A total of 200 valid questionnaires were collected for analysis.</p><p><strong>Results: </strong> The study revealed that the overall prevalence of positive detection of general mental health problems in women during late pregnancy was 11%. Significant differences were observed in the positive detection rate of general mental health status based on various factors such as the quality of relationships with husbands, pregnancy intentions, insomnia, anxiety, and depression (p<0.01). Furthermore, participants with general mental health problems displayed notably higher scores on the AIS-8, PHQ-9, and GAD-7 scales compared to those without such problems (p<0.01). Regression analysis indicated that pregnancy intention and PHQ-9 scores were influential factors affecting the general mental health of women during late pregnancy (p<0.05).</p><p><strong>Conclusion: </strong> The study highlights high rates of general mental health problems during late pregnancy, with unplanned pregnancy and elevated depression scores as key risk factors. These 'psychosomatic' comorbidities increase with migraine severity. Severely affected, comorbid patients also often have a poorer response to specific migraine therapy. Interestingly, migraine and the comorbidities mentioned have a number of common aetiological or facilitating factors, e.g. genetic factors, and show a higher incidence in women and in people with previous traumatic experiences, as well as (in the case of pain syndromes) signs of central sensitization. Another common feature is the association with current or chronic stress. We propose an extended diathesis-stress model that takes into account interrelated but individually different vulnerabilities and, depending on the stress experience, can depict both the occurrence of individual disorders (e.g. an isolated migraine) and the joint occurrence of migraine with other pain syndromes and other psychosomatic comorbidities.
