It is essential to have the ability to bring these symptoms under control in a way that is rapid, effective, and humane, and electroconvulsive therapy (ECT) provides this. Despite enormous advances in the safe delivery, regulation, and efficacy of ECT over the last 70 years, and its ability to bring patients from stupor to recovery over (on average) just 6–12 treatments, it remains a stigmatized and stigmatizing treatment in the public eye. {"title":"International perspectives on inpatient mental health care","authors":"A. These variations are most stark when based on economic factors, but stigma, prevailing societal attitudes, and the role of the family also play a significant part in the amount and quality of mental health care overall. This chapter begins by outlining global economic factors and their impact on provision. It then focuses on the concept of a whole systems approach, looking briefly at the evidence base for different components of services generally seen in high-income group countries. Alternative suggestions to ‘high-income group’ models are discussed as the evidence internationally emanates almost exclusively from a small number of wealthy westernized countries. Nursing leadership is identified as a significant factor in developing a positive culture in the inpatient environments and aspects of effective leadership are suggested along with ways to successfully manage, lead, and inspire a team in these roles. It also highlights the importance of challenging and addressing these issues, and suggests some points of best practice. Diversity is a complex issue and organizations are required to improve their services for patients and staff that fit the protected characteristics. There is a significant and enduring disparity in the life expectancy of the general population and people with severe mental illness (SMI). The majority of this inequality is due to preventable physical illness, particularly cardiovascular disease, respiratory disorders, and diabetes mellitus. Risk factors for these disorders such as smoking, obesity, and lack of physical activity are disproportionately represented in SMI populations and some psychotropic medications also contribute. Physical ill health impacts negatively on mental health outcomes. Psychiatric inpatient admissions represent a unique opportunity to address many of these illnesses and risk factors. It led to reduced conflict and ultimately reduced violence and aggression. The perceptions of patients on the impact of the Safewards project on their level of feeling safe are explored.
