While it is generally managed by neurologists, many of the presenting symptoms and most of the challenges in management are psychiatric. The description of psychiatric symptoms as part of the presenting symptoms of encephalitis dates back to the first descriptions of the disorder. The more recent discovery of autoimmune causes of encephalitis has particular relevance for psychiatry: there is significant overlap between the initial presentation of encephalitis with other core psychiatric disorders such as depression and psychosis. The particular challenge for inpatient psychiatry is to screen and detect these disorders early. The current evidence indicates that the rapid detection and delivery of immunotherapy, rather than the use of psychiatric treatments, is associated with better long-term outcomes for patients. Conversely, the risks of not detecting encephalitis are of long-term disability, or even death in a proportion of patients. Using an authentic, compassionate approach towards understanding risks with service users is important to support meaningful collaborative risk assessment, and involving carers wherever possible is recommended. Reflective peer review can be a helpful way to consider decision-making and enhance clinical skills, as well as providing valuable support to staff. Beyond the macro level, it aims to provide a handbook for anyone wishing to set up such a ward from scratch: it explores what a ward designed with older adults in mind would look like (design) and what resources and staffing are necessary. PICU psychiatry concerns the management of acute psychiatric illness, usually of a manic and/or psychotic nature, associated with behavioural disturbance. Such disturbance often involves violence and aggression towards others. It discusses the physical risks that may be posed by a restraint, including prone restraint concerns, and reviews the public health model in relation to care planning around the management of disturbed behaviour—looking at the use of primary, secondary, and tertiary level interventions. In addition, the chapter discusses risk assessment and the decision-making process involved in a restraint.
