Adipokines include fat-secreted proteins such as leptin or adiponectin and fat- or liver-derived cytokines such as tumor necrosis factor-α (TNF-α) and interleukin 6 (IL-6) are considered to play an important role in the pathogenesis of the metabolic syndrome, including NAFLD.Material and Methods:One hundred Saudi patients with NAFLD (45 males and 55 females) with NAFLD diagnosed by ultrasonographic findings, our group includes one gender, age and body mass index (BMI) matched hundred healthy volunteers. Adiopkines and parameters of glucose control of all participants were detected.Results:Serum glucose, insulin, aspartate aminotransferase(AST), alanine aminotransferase (ALT), aspartate aminotransferase /alanine aminotransferase ratio (AST/ALT), serum levels of total cholesterol, triglycerides, low density lipoprotein cholesterol (LDL-C), serum TNF-α and L-6 levels were significantly higher in patients with NAFLD when compared to control group. However, serum high density lipoprotein cholesterol (HDL-C) and adiponectin levels were significantly lower in patients with NAFLD when compared to control group. It can be used to systematically evaluate the cardiovascular anatomy, morphologic features of thoracic vessels such as aorta, systemic veins and pulmonary vessels and relationship between the upper lobe bronchi and pulmonary arteries, coronary artery, valves, systemic veins. In diffuse lung diseases, this technique can increase nodule detection and help differentiate between small nodules and vessels. MDCT with three-dimensional (3D) volume rendering provides a unique perspective on thoracic anatomy and disease. MDCT allows shorter acquisition times, greater coverage and superior image resolution. In vascular imaging, this technique provides image quality that equals or surpasses that of conventional angiography. Its use has expanded to aid in diagnosis and surgical planning. The diagnosis of the ambulance crew was conversion or seizure. The patient has all of dystonic reaction symptoms, including facial, neck, back, and extremity spasms, opisthotonus, oculogyric crisis, torticollis, trismus. The history revealed 40 mg of metoclopramide intake. Biperiden (5 mg) was infused in 100 ml saline. Symptoms were completely resolved. She was discharged from the ED.\u0000\u0000Drug-induced dystonic reactions can be confused with conversion, seizures, encephalitis, tetanus and hypocalcemic tetany. Gruber, is an autosomal recessive disorder that is characterized triad of occipital encephalocele, bilateral renal dysplasia and polydactyly. Because of the genetic heterogeneity, multiple organs can be affected. The worldwide incidence varies from 1 in 13,250 to 1 in 140,000 live births. We have reported a male baby born at 37-week gestation, who has occipital encephalocele, polydactyly and cystic dysplasia of both kidneys. As a result of these clinical findings, Meckel-Gruber Syndrome is suspected for his baby. However, there is a high need for noninvasive methods for assessment of hepatocellular damage.Material and Methods:One hundred patients with chronic hepatitis C virus infection(64 males and 36 females, their age ranged from 28 to 53 years with circulating anti-HCV antibodies were equally categorized into two study groups: patients with CHC and patients with liver cirrhosis (LC). Also, one fifty healthy subjects were included as healthy controls. Serum alanine aminotransferase (ALT), soluble intercellular adhesion molecule1 (sICAM-1); Soluble vascular adhesion molecule 1(sVCAM-1); Soluble E-selectin(s-E-selectin) and Tumor necrosis factor-alpha (TNF-α) were assayed for all participantsResults:We observed elevation with regard to the healthy controls group in the parameters of ALT, sICAM-1, sVCAM-1, s-E-selectin and TNF-α for patients with CHC and patients with liver cirrhosis (LC). Also, a significant positive correlation between serum TNF-α, sICAM-1, sVCAM-1 and ALT values was detected.Conclusions:In conclusion, our results confirm that, in patients with chronic virus hepatitis and liver cirrhosis there is a significant positive correlation between serum TNF-α, sICAM-1, sVCAM-1 and ALT values. Hemophagocytic syndrome secondary to Adult-onset Still’s disease was reported rarely in literature. However, 10% of myxomas arise from atypical locations. This report presents a rare case of atypical location of extracardiac myxoma in a case of sudden death.\u0000\u0000A 43-year old female suddenly dies at home. At autopsy, three masses attached each other were observed in pericardial fat tissue localized between right atrium, vena cava superior wall, and right ventricle. Three masses, as a lobulated single mass, were 7x2x1 cm, 5x5x2 cm, 3x2x0.5 cm in size, respectively. Masses were observed to surround right coronary artery for a segment of 7 cm. Cardiac dissection revealed that mass infiltrates epicardial site of right atrium and ventricle wall but not infiltrated to myocardium and heart cavities. The secondary tuberculosis of oral cavity represents 0.2-1.5% of all cases with extrapulmonary tuberculosis. Although incidence of oral lesions secondary to pulmonary tuberculosis ranges from 0.4% to 1.5%, primary tuberculosis of the tongue occurs very rare, and it is published only as isolated case reports. A male patient presented to our dermatology clinic with complaints of painful ulcerated white plaques on the tip of his tongue lasting for three months, and an excisional biopsy showed granulomatous inflammation with caseification necrosis. Pulmonary and systemic examination showed no evidence of pulmonary or other extra-pulmonary tuberculosis, and he was initiated therapy for diagnosis of primary tuberculosis of the tongue. We found that the lesion completely resolved during the follow up visit after 1 month.
