The systematic annual process of starting the development of new nursing clinical guidelines and of leading and coordinating the adaptation as well as preparing the trainings of national trainers for each set of guidelines has slowed down or even stopped and needs immediate attention. In addition, the development of corresponding evidence-based Standardized Operational Procedures must be organized. Our recommendation is that the Ministry of Healthcare and the Republican Center of Health Development would strengthen the national coordination and create a systematic roadmap for translating nursing clinical guidelines into nursing practice. In addition, nurses’ knowledge and skills in evidence-based nursing must be increased through capacity building activities. The formation of social health insurance and developments in medical education was followed by a Social Health Insurance Project to increase the accessibility, quality, and efficiency of health service delivery and reduce financial risks to the population caused by severe health problems. As a part of the Project, Semey Medical University and Başkent University became strategic partners to develop not only the education of the future medical doctors but also the management of the University Hospital. The project last for 21 months. Although pandemic restrictions caused some obstacles during the implementation of the project, all the tasks and activities were executed with a successful cooperation of the experts of both parties. The development of the curriculum, implementation of the Başkent University Medical Education Model, the modernization of the defined clinics as well as the development of the financial and strategic decision making of the governance were the main tasks of the project. Methods.The research was carried out according to the traditional scheme and included the classical stages: drawing up a plan and program, collecting material, processing and analyzing it. Results. To improve the quality of medical care, targeted measures are being implemented. Thus, the design capacity of Primary Healthcare: polyclinic – 500 people per shift, actual attendance – 703 people per shift. The hospital has 330 beds. However, the rate of bed coverage per 10 000 people is low at 28.4. The overall incidence has decreased, and in 2019 it was 75 652. It was found that the number of visits to doctors has increased, thus, this figure in 2019 reached 66.5%. It was found that the volume of provision of free medicines increased and reached 47 420. The indicator of provision with medical technical means as a whole also increased and amounted to 82%. Conclusions. A set of measures aimed at improving the hospital's activities using the existing organizational structure will allow us to respond in a timely manner to changes in all areas of the hospital's activities, and the KPI system will help assess the effectiveness of managerial actions in the field of quality management to improve the quality of medical care and improve the weaknesses of healthcare facilities. Evaluate the clinical pharmacologist's contribution to the rational use of medications at the outpatient level.Methods. Clinical and economic analysis of budgetary expenditures on drugs purchased as part of the Guaranteed amount of free medical care and in the system of mandatory social health insurance (MSHI) for the period from 2019-2021. Assessment of the effectiveness of spending budgetary funds for drugs was conducted using data on the number and cost of drugs purchased to provide outpatient care in the medical organization of Nur-Sultan.Results. A three-year analysis showed a positive trend of a 7.1% decrease in the proportion of spending on unproven drugs (index N) by 2021 compared with the previous year 2020, which was 29.54% of spending on 13 drug items, despite the expansion of lists under insurance medicine. Before the introduction of mandatory social health insurance in 2019, this figure was 8.18% of all outpatient drug coverage costs in the Guaranteed amount of free medical care, indicating an average use of drugs not included in international clinical guidelines.Conclusions. The introduction of clinical pharmacology services into clinical practice has optimized the cost of drug therapy and promoted rational use of medications. The result is a 7.1% decrease in the share of spending on unproven drugs compared to the previous year 2020, which amounted to 29.54% of spending on 13 drugs (index N) not included in the Kazakhstan National Medical Formulary and the WHO Model List of Essential Medicines. However, due to the long-term training of clinical pharmacologists, including basic education in therapeutic medicine (6-7 years) and additional education at the level of residency or doctoral studies (2-3 years), there is decreased motivation of clinical pharmacologists to work in budget medical organizations due to the \"low\" level of wages.
