The incidence of listeriosis seems to be increasing. In the present study cases of listeriosis from two hospitals are analyzed with the aims of studying changes in its incidence, clinical forms of presentation and possible factors associated with mortality.</p></div><div><h3>Methods</h3><p>Retrospective multicentric study of patients with culture-proven listeriosis in two university hospitals in Madrid between 1977 and 2021. Epidemiological and clinical variables, as well as factors for immune depression, complementary studies and treatments were registered. Factors associated with mortality were analyzed.</p></div><div><h3>Results</h3><p>A total of 194 cases of listeriosis were analyzed. The incidence of listeriosis among in-patients increased through the study period, with a significant drop in the number of cases in 2020. The most common clinical presentations were isolated bacteriemia (37.1%) and central nervous system involvement (CNS) (36.6%). Symptoms of gastroenteritis occurred in 21% of cases. Other focal infections were present in 16.5% of patients, the most frequent were spontaneous bacterial peritonitis (8.2%), cholecystitis (2.1%), respiratory infection (1.5%) and vascular prothesis infection (1.5%). In-hospital mortality was 24.7%. Independent factors associated with mortality at admission were age (odds ratio [OR] 1.027, 95% confidence interval [95% CI]1.003–1.056) and a diagnosis of a solid tumor (OR 3.525, 95% CI1.652–7.524).</p></div><div><h3>Conclusions</h3><p>This study confirms an increasing incidence of listeriosis in our millieu. The most common clinical presentations were isolated bacteriemia and central nervous system involvement. {"title":"Relevancia de los enterovirus en la meningitis neonatal","authors":"Elisenda Moliner-Calderón , Núria Rabella-Garcia , Eulalia Turón-Viñas , Gemma Ginovart-Galiana , Josep Figueras-Aloy","doi":"10.1016/j.eimc.2022.07.010","DOIUrl":"10.1016/j.eimc.2022.07.010","url":null,"abstract":"<div><h3>Introduction</h3><p>Enterovirus (EV) infections are the most frequent infections in the neonatal period and in many cases lead to hospital admission of the newborn (NB).</p><p>The aim of this study was to determine the incidence of EV in the etiology of neonatal meningitis and to define the clinical characteristics of newborns with EV meningitis.</p></div><div><h3>Material and method</h3><p>Retrospective observational cohort study. Including 91 NBs with meningitis and gestational age greater than 34 weeks gestational age (GA) attended in our center over a period of 16 years.</p></div><div><h3>Results</h3><p>The percentage of NBs with EV meningitis was higher than that of NBs with bacterial meningitis (BM) and accounted for 78% (<em>n</em> <!-->=<!--> <!-->71). Half of the NBs with EV infection had a history of epidemic environment among their caregivers. Fever was present in 96% of cases as a clinical sign and, in general, sensory disturbances represented the main neurological alterations. Antibiotics (ATB) were given to 71.4% of patients with EV infection.</p><p>Detection of EV in CSF samples showed a high sensitivity for the diagnosis of EV meningitis. The most frequently implicated EV types were echovirus 11, coxsackievirus B5, echovirus 18, 25 and 7.</p></div><div><h3>Conclusions</h3><p>The results of this series show that enterovirus infection is a common cause of neonatal meningitis. These data underline the importance of rapid EV testing of infants with suspected meningitis. Rapid diagnostic tests (RDTs) have been proposed as alternatives, although data about their potential impact in non-endemic areas is scarce.</p></div><div><h3>Methods</h3><p>We performed a cost-effectiveness analysis comparing the costs of dengue RDTs to the current standard of care for the management of febrile returning travelers in Spain. Effectiveness was measured in terms of potential averted hospital admissions and reduction of empirical antibiotics, based on 2015–2020 dengue admissions at Hospital Clinic Barcelona (Spain).</p></div><div><h3>Results</h3><p>Dengue RDTs were associated with 53.6% (95% CI: 33.9–72.5) reduction of hospital admissions and were estimated to save 289.08–389.31€ per traveler tested. Additionally a systematic literature review of articles published between 1975 and 2021 in PubMed/MEDLINE was conducted.</p></div><div><h3>Results</h3><p>52 studies with 65 cases were found; five were excluded because of incomplete data. In total 64 cases were analyzed including our four cases: 55 with meningitis and 9 with intracranial focal infections. Both infections were frequently associated with underlying conditions (70.3%) such as immunosuppression (32.8%) or cancer (10.9%). In 23 cases a biotype was identified, with biotype <span>II</span> being the most frequent (69.6%) and <em>S. pasteurianus</em> the most common within this subgroup. Intestinal diseases were found in 60.9% of cases, most commonly neoplasms (41.0%) and <span><em>Strongyloides</em></span> infestation (30.8%). Overall mortality was 17.1%, with a higher rate in focal infection (44.4% <em>vs</em> 12.7%; <em>p</em> <!-->=<!--> <!-->0.001).</p></div><div><h3>Conclusions</h3><p>CNS infections due to <em>S. bovis</em><span> are infrequent and the most common clinical form is meningitis. Compared with focal infections, meningitis had a more acute course, was less associated with endocarditis and had a lower mortality. WGS was performed using Ion torrent GeneStudio™ S5 system for <em>emm</em> typing and identification of superantigen genes in <em>S. pyogenes</em> isolated during the 2022–2023 UK outbreak alert.</p></div><div><h3>Results</h3><p>During 2023, there were more cases of GAS-BSI compared to the same period of previous year with a non-significant increase in children. Fourteen isolates were sequenced. The <em>emm1</em> (6/14, 42.9%) and <em>emm12</em> (2/14, 14.3%) types predominated; 5 of 6 (75%) <em>emm1</em> isolates were from the M1<sub>UK</sub> clone.
