Motivated by this, in this article, a robust prediction model called COVID-SDL has been proposed using Sequential Deep Learning (SDL) for predicting the total positive cases per day. In order to evaluate the performance of COVID-SDL, data samples used in the model have been collected from Italy’s COVID-19 situation reports. Besides this, the dataset has gone through the processes of cleaning, filtering, formatting and visualization. COVID-SDL utilizes the correlation information among the features that have strengthened the prediction capability. Also, the exploratory survey showed that 5 most salient features (Home Confinement, Deaths, Recovered, Current Positive Cases and Tests Performed) results better which is obtained from the mentioned dataset primarily composed of 17 features. In addition, to assist the prediction ability of COVID-SDL, ReLu (Rectified Linear Unit) activation function has been used which enhanced the robustness of the model. With a view to making the predictions highly accurate, Adam optimizer has been adopted which works by reducing the cost function and making further updates of the weights. Moreover, COVID-SDL has successfully obtained accuracy parameters such as MAE- 0.00037316, MSE- 0.00000018, RMSE- 0.00043476 and R2 Score- 0.99999 with providing the best fit curve of predicted data which covers 99.999% of the actual data. Hence, the current study evaluated sodium status and its correlation with indices of pulmonary dysfunctions among Nigerians of Negroid race.\u0000\u0000\u0000Methods: This was a retrospectively-designed observational study. Data, all obtained at presentation, were acquired from medical records of 480 RT-PCR-confirmed COVID-19 patients managed at a COVID-19-designated treatment facility in Port Harcourt, Southern Nigeria. Analysis of acquired data was done by COVID-19 clinical grades and sodium status using descriptive and inferential statistics.\u0000\u0000\u0000Results: At presentation, hyponatremia and hypernatremia were observed in 47.7% and 1.0% of the entire studied cohorts (n=480), respectively. Both disorders (hyponatremia/hypernatremia) were mostly observed among the moderate, severe, and critical cases. Most hyponatremic cases (n=154;67.2%) and the entire hypernatremic cases (n=5;100%) were of mild grades. Hyponatremics had higher proportions of fever, breathlessness, confusion, and a higher burden of inflammatory markers which increased with worsening hyponatremic grade. Etiologically, hyponatremia was mostly associated with the syndrome of inappropriate anti-diuretic hormone secretion (SIADH) (n=132;56.7%). Among the hyponatremics, an inverse correlation existed between sodium and respiratory rate (RR), while a correlation existed between sodium and oxygen saturation (SpO2). Compared to mild hyponatremics, the moderate/severe hyponatremics had a greater risk of having RR>30 and SpO2 <95%. \u0000\u0000\u0000Conclusion: Hyponatremia, mostly of mild grade, was common among the studied COVID-19 patients and was associated with indices of pulmonary dysfunctions, including disease severity, inflammatory markers, and SIADH. Hence, hyponatremia should be utilized to triage COVID-19 patients at presentation. This degenerative condition is progressive with functional impairment and a severe decrease in quality of life. Pain management with Adductor Canal Block (ACB) and Genicular Nerve Block (GNB) can be an alternative treatment option for chronic knee pain. This study aims to analyze the difference between clinical & laboratory outcomes of the ACB and GNB groups. \u0000\u0000\u0000Material & Methods: An experimental study with consecutive sampling was conducted on 34 patients with chronic knee pain divided into 2 groups of ACB and GNB procedures. The parameters measured were VAS score, pain-free duration, range of motion, and interleukin-6 (IL-6) levels. \u0000\u0000\u0000Results: Predominance of chronic knee pain patients found in older women. Chest radiography and CT-enhanced studies help in diagnosis with pulmonary angiography which is considered the gold standard study for diagnosis. Management was done with embolization of the feeding artery or arteries. \u0000\u0000\u0000Case: A 31 year old female with no previous history of medical illness, presented with sudden onset left sided weakness and was diagnosed as a case of right sided ischemic stroke. After reviewing her case by multidisciplinary team and multiple imaging studies, pulmonary angiography showed that the patient had large right lower lung lobe PAVM in addition to another right middle lung lobe and left lower lung lobe smaller PAVM. After that, patient underwent embolization of with no complications.\u0000\u0000\u0000Conclusion: PAVM, although it is rare, should be considered in differential diagnosis as the cause of neurological manifestation as ischemic stroke in young patients by paradoxical emboli. Classic migraine (with aura) and common migraine (without aura) are clinically more common in the population. Migraine mainly has symptoms such as throbbing and throbbing headache attacks accompanied by nausea, fear of light and sound, and recovery with rest. This study investigated the migraine pattern in 90 patients referred to Walfajr Health Center in Shiraz through questionnaires and statistical analysis and compared it with the global pattern in terms of epidemiological characteristics. According to the results, migraine is more common in women and ages before 40. The results also indicated that migraine without aura is more frequent than migraine with aura. All these findings are in line with the global pattern. Pressing Three Yin Intersection point was effective for diseases caused by cold feet and waist and was also effective for gynecological diseases. Pressing Leg Three Miles point was effective for all gastrointestinal diseases and could regulate immunity. Pressing Junction Valley point could improve blood circulation above the shoulders, effective for shoulder and neck discomfort. Results shown the 30 patients obtained good medical treatments without pain. Currently there is no definite parameter to determine the necessity of surgery in CTEV. This study aimed to examine the association between Pirani score and the elasticity of achilles tendon with the need for Achilles Tendon Lengthening (ATL) in CTEV patient. \u0000\u0000\u0000Methods: This study is a prospective study which begins with calculating Pirani score and examining achilles tendon elasticity using Shear-Wave Elastography. The subject was performed ponseti serial casting and followed up for up to 6 weeks. After 6 weeks, an assessment was made to determine the necessity of ATL procedure based on the ability to perform 150 dorsiflexion. Data analysis was performed using SPSS version 22 to calculate receiver operating characteristic curve, spearman correlation test, and chi-square test.\u0000\u0000\u0000Results: There were a total of 33 feets included. Pirani score above 4.25 and achilles tendon elasticity above 19.39 kPa predicted the need of ATL. Correlation test showed moderate positive correlation between the Pirani score and the Achilles tendon elasticity score (r= 0.447; p = 0.009). There is an association between Pirani Score with surgery necessity (RR 4.52; 95%CI 1.52-13.45; p=0.001) and achilles tendon elasticity with surgery necessity (RR 9.625; 95% CI 2.54-36.35; p<0.001). \u0000\u0000\u0000Conclusion: There is an association between the Pirani score and achilles tendon elastography with the need of ATL procedures in CTEV patients. Cases of osteoporosis are often found in patients with CKD who are commonly known as having Renal Osteodystrophy, which is defined by the presence of disorders or pathological conditions in the bones associated with CKD conditions. FGF-23, CRP, ALP are markers that have been associated with the occurrence of a renal osteodystrophy. By conducting this research, it is expected to know the relationship between the three markers and the occurrence of renal osteodystrophy. This study used a case-control design with a cross-sectional method to determine the relationship between FGF-23, CRP, ALP and renal osteodystrophy. From the CKD population with or without renal osteodystrophy, the sample was selected by consecutive sampling. After that, statistical tests were carried out in the form of descriptive tests, normality tests, risk factor assessment using 2x2 cross-tabulations, and predictive analysis of proportions using logistic regression tests. FGF-23 levels > 30 pg/ml (p-value <0.001; CI = 1.4-6.7), CRP > 3 mg/L (p-value <0.001; CI = 1.3-4.8), ALP > 120 U/L (p-value <0.001; CI = 1.7-10.6) in patients with stage IV-V CKD significantly increased the risk of renal osteodystrophy. In the multivariate analysis, ALP had the most significant value compared to these three variables. Abkari","doi":"10.24018/ejmed.2023.5.2.1661","DOIUrl":"https://doi.org/10.24018/ejmed.2023.5.2.1661","url":null,"abstract":"\u0000The Luxatio erecta humeri is the inferior dislocation of the glenohumeral joint and the poterior hip dislocation are rare cases in the emergency department.\u0000\u0000\u0000The association of the two presentations is a rare entity and similar cases are very little found in the literature, hence the purpose of this work.\u0000\u0000\u0000We report the case of a 35-year-old taxi passenger victim of a public road accident who presented with both luxatio erecta humeri and pure posterior hip dislocation. An orthopaedic surgeon reduced the luxatio erecta humeri and the posterior hip dislocation under procedural anesthesia as soon as the patient’s vital signs were stable. \u0000\u0000\u0000Different concomitant injuries and various injury mechanisms have been described in regard to inferior shoulder dislocation in the literature. However, pure posterior dislocation of the hip as a concomitant distant region injury for luxatio erecta humeri is being described for the first time in this case report. \u0000\u0000\u0000Some authors suggested that a two-step manoeuver can be easier to practice than a one-step manœuvre, in our specific case, luxatio erecta was easily reduced by a single operator in a single attempt. \u0000\u0000\u0000Luxatio erecta humeri and posterior hip dislocation are lesions that occur at high energy and complex injuries can accompany them in patients with multiple traumas.
