It is clinically presented as depigmented macules or patches that occur due to malfunction and loss of melanocyte. Objective To evaluate the impact of severity of vitiligo evaluated by Vitiligo Area Scoring Index (VASI) score on quality of life of Egyptian patients using the Arabic translation of the Dermatology Life Quality Index (DLQI) and to evaluate the treatment effect on quality of life of Egyptian vitiligo patients. Patients and methods In all, 25 vitiligo patients with more than or equal to 18 years old were enrolled into the study. Corticosteroid cream was topically applied and was combined with Ultraviolet B-Narrow band (UVB-NB) phototherapy. The session of UVB-NB was done 2 days a week for at least 4 months duration. The VASI score was used before and after treatment to estimate the severity of vitiligo. The effect of vitiligo on patients' quality of life was evaluated before and after treatment using the Arabic version of the DQLI scale. Results Vitiligo negatively affected the Egyptian patients' quality of life. The mean of the VASI score was significantly decreased after treatment (P = 0.040). Moreover, the mean of the DLQI scale after treatment (6.48 ± 2.89) was significantly decreased than that before treatment (7.44 ± 3.11) (P = 0.003). There was significant positive correlation between VASI score and DLQI of patients before treatment (r = 0.736; P = 0.000) and after treatment (r = 0.667; P = 0.000). Conclusion The effect of vitiligo on patients' quality of life was correlated positively with the severity of the disease. Vitiligo patients before treatment have worse quality of life than after treatment. This study was performed to evaluate renal resistance index (RRI) and Cystatin C (Cyst C) as predictors of HRS. Patients and methods The included 100 adult patients with liver cirrhosis and ascites were divided into three groups; group I with HRS (30 patients), group II with normal serum creatinine and increased RRI (40 patients), and group III with normal serum creatinine and normal RRI (30 patients). International normalized ratio, total bilirubin, albumin, creatinine, and Cyst C were measured in all enrolled candidates. Also, abdominal ultrasound was done with duplex Doppler examination of the kidneys and RRI was calculated. Group II was randomly divided into subgroups IIA and IIB where group IIA were received prophylactic therapy against HRS and group IIB did not. Both subgroups were followed for 6 months to assess their outcome and possibility to develop HRS. Results Serum Cyst C and RRI were significantly higher in those with HRS in comparison with other enrolled patients. There were significant correlation between serum Cyst C with RRI and estimated glomerular filtration rate. Progression to HRS and death was frequently higher in those patients who did not receive prophylactic therapy against HRS. With multivariate regression analysis; low serum albumin, increased RRI, and increased Cyst C are independent risk factors for progression to HRS while prophylactic therapy is protective against HRS in patients advanced liver cirrhosis. Early AKI diagnosis can help in treating this complication. Neutrophil gelatinase-associated lipocalin (NGAL) is a recent marker for the diagnosis of contrast-induced acute kidney injury (CI-AKI). This research targeted to evaluate the early diagnosis of CI-AKI and predictive value of NGAL and study the correlation between renal role tests and serum NGAL in cases with coronary artery disorder. This research was conducted on 45 cases with coronary artery disorder. Serum NGAL, urea, and creatinine (SCr) were evaluated. The estimated glomerular-filtration rate (eGFR) was measured 2 and 48 h after PCI. Results In total, 11 (24.4%) patients had AKI, while 34 (75.6%) patients had no AKI. Serum urea NGAL was significantly greater in AKI cases either 2 or 48 h after PCI, while SCr was significantly greater in AKI cases 48 h after PCI. eGFR 48 h after PCI was significantly decreased in AKI patients. Albumin/creatinine (A/C) ratio was significantly greater in AKI cases. Serum NGAL 2 h after PCI positively correlated with A/C ratio and SCr 48 h after PCI, but is negatively correlated with eGFR 48 h after PCI. After 2 h, serum levels of NGAL had 90% sensitivity and 55% specificity; after 48 h, they had 81% sensitivity and 61% specificity. SCr after 2 h had 63% sensitivity and 82% specificity, and after 48 h, had 90% sensitivity and 88% specificity. The objective of this study was to determine the prevalence of ORD in managed hypertensives in a tertiary health care center in South Nigeria. Participants and methods A cross-sectional retrospective was carried out in 708 managed hypertensives attending the medical outpatient clinic. Data from 826 patients were analyzed, and 706 (85.7%) of these patients were found to have a creatinine value of 1.5 mg/dl and less. These 708 managed hypertensives with a normal serum creatinine value were analyzed, and their estimated glomerular filtration rate (eGFR) calculated with the Chronic Kidney Disease Epidemiology (CKD_EPI) prediction equation. Results A total of 218 (30.9%) had an eGFR lower than 60 ml/min,244(34.6%) had an eGFR of 60–89 ml/min, and the remaining 244 (34.6%) had an eGFR of 90 ml/min and above. Conclusion ORD has a high prevalence of 31% in treated hypertensive patients with normal serum creatinine values of less than 1.5 mg/dl. There is a need to use eGFR values, in this case, the CKD-EPI to assess GFR, to enable physicians detect chronic kidney disease at an earlier stage than the current practice of using serum creatinine alone. Aim To assess the degree of adherence of pediatricians in pediatric intensive care unit (PICU) at Assiut University Children Hospital to the American protocol of management of VAP. To assess the demographic and clinical characteristics of admitted children. Results The study included 50 cases admitted in PICU at Assiut University Children Hospital who developed VAP. There were 32 males and 18 females with age ranging from 1 month to 17 years. VAP was detected more commonly among age group 1 month to less than 5 years (76%) followed by age group 5 to less than 10 years (16%) followed by age group 10 to 17 years (8%). Late-onset VAP was observed in 86% of cases, while early-onset VAP was observed in 14% of cases. VAP with risk factors for resistant organisms was observed in 94% of cases, while VAP without risk for resistant organisms was observed in 6% of cases. In our study, the treatment that was given for the studied cases was in the form of empirical therapy. According to the American Protocol of Management, VAP in PICU was given in 86% of cases. Changing antimicrobial therapy after 48–72 h according to culture results and clinical response was done in 78% of cases. Proper broad-spectrum empirical therapy and de-escalation of antibiotics according to culture results and clinical response was done in 76% of cases. It can easily detect injuries to different abdominal viscera. Aim The current work aimed to evaluate the role of MDCT in the diagnosis of splenic injuries in patients with blunt abdominal trauma. Patients and methods Over a period from March 2019 to June 2019, 40 patients with different forms of splenic injuries were reviewed. The splenic injuries were diagnosed based on abdominal ultrasound, CT, and/or laparotomy if done. Results Of the studied patients, 85% were males. The mean age of all patients was 21.40 ± 14.39 years. The most frequent form of trauma was motor car accidents (42.5%). Based on CT, 17 (42.5%), 16 (40%), seven (17.5%), and two (5%) patients had splenic laceration, hematoma, shattered spleen, and vascular extravasation, respectively. All patients had intraperitoneal fluid collection. Moreover, grades I, II, III, IV, and V splenic injuries were presented in seven (17.5%), six (15%), 19 (47.5%), two (5%), and six (15%) patients, respectively. All patients with grades I, II, and III were conservatively managed, whereas those with grades IV and V were managed with splenectomy. Diagnosis by MDCT in 20% of patients was consistent with the final diagnosis in laparotomy, so diagnostic accuracy of MDCT was 100%. In 80% of patients, the management plan was conservative and follow-up ultrasound showed improvement of splenic injuries, which suggests that the diagnosis of MDCT was correct. It could be determined by computed tomographic (CT) measurement in Hounsfield units. In this study, we tried to establish an accurate way of stone density measurement by computed tomographic. Patients and methods In this prospective study, 18 patients with renal or upper ureteric stones were scheduled to perform ESWL. Stone densities were measured from different parts: central and peripheral measurements as well as stone size, location, and skin-to-stone distance. Results The patients' mean age was 40.4 ± 16.5 years. A total of 14 (77.8%) patients were male and four (22.2%) patients were female. The central density showed a statistically significant difference between the two groups with higher densities in the failed group (P = 0.001). The analysis proved that center density less than 935 HU predicts the successful outcome with a sensitivity of 71.4%, a specificity of 90.9%, a positive predictive value of 83.3%, a negative predictive value of 83.3%, and accuracy of 81.8%. Pelvic venous insufficiency is one of the causes of chronic pelvic pain, especially in females of child-bearing age. Embolotherapy is now a developing-adopted treatment method to pelvic venous insufficiency. Patients and methods We accomplished a prospective study on 20 females (mean age, 33.8 years; range between 20 and 45 years) with pelvic congestion syndrome treated between April 2017 and April 2018 by Trans-Cutaneus Foam Sclero- therapy (TCFS). Pelvic pain and atypical lower limb varices were associated with dyspareunia in 16 (80%) patients, urinary urgency in 12 (60%) patients, and tender ovarian point sign in 17 (70%) patients. Diagnosis depended mainly on pelvic pain analysis, detailed history taking, and clinical examination and was confirmed by transcatheter venographic findings. TCFS by 5% ethanolamine oleate was performed in all patients. Follow-up was done by estimation of pain at 1, 3, 6, and 12 months after the maneuver. Results The percentage of technical success was 100%. A statistically significant clinical improvement in each category of specific symptoms was noticed at 1, 3, 6, and 12 months after the procedure. Recurrence was observed in one case. Minor adverse effects were noticed after the procedure, and they were mild and responsive to medical treatment. Conclusion We found out that TCFS of female varicocele using a 5% ethanolamine is safe and effective to treat pelvic congestion syndrome. Iron deficiency may lead to reactive thrombocytosis and rarely thrombocytopenia. Participants and methods This study included 115 children with iron-deficiency anemia attending Assiut University Children Hospital, Hematology Unit, from the June 1, 2017, to the June 1, 2018. Their age ranges from 2 to 5 years. Moreover, 55 apparently healthy children with the same age group were included as a control group. All patients and controls were subjected to meticulous history taking, thorough clinical examination, hematological study, as well as stool and urine examination. Results The mean platelet count in our patients was significantly higher than in controls. Thrombocytopenia was noticed in one patient. Results were discussed in the light of available literatures. Conclusion Reactive thrombocytosis is the most frequent platelet abnormality associated with iron-deficiency anemia. The authors also aimed to study the possible correlation between the occurrence of pulmonary hypertension and fluid overload, arteriovenous fistula (AVF) blood flow, valvular calcification, and other laboratory variables. Patients and methods This cross-sectional descriptive study was conducted on 80 patients with end-stage kidney disease on regular HD for more than one year in Al Agoza Hospital in Cairo, Egypt. General data were collected. A transthoracic 2D echocardiography was done for all studied patients within one hour after HD. Pulmonary hypertension's definition was an estimation of pulmonary artery systolic pressure greater than 35 mmHg by echocardiography. Results Pulmonary hypertension (pulmonary artery systolic pressure >35 mmHg) presented in 21 (26.2%) patients, whereas mild (35–50 mmHg), moderate (51–70 mmHg), and severe pulmonary hypertension (>70 mmHg) presented in seven (8.8%), 10 (12.5%), and four (5%) patients, respectively. Pulmonary hypertension presented in 13 (62%) female patients vs eight (38%) male patients, which is of statistical significance (P<0.001). Mean duration of dialysis was significantly higher in patients with pulmonary hypertension (PHTN) than those without (8.54±3.53 years vs 3.88±1.01 years; P<0.001). Valvular calcification, fluid overload, and blood flow through AVF were significantly correlated to the development of PHTN. Conclusion Pulmonary hypertension in patients with end stage kidney disease (ESKD) on regular HD is frequent.
