{"title":"The influence of further consultation adherence on myopia progression after children wear glasses","authors":"Chang-feng Guo","doi":"10.3760/CMA.J.ISSN.1006-4443.2017.05.006","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1006-4443.2017.05.006","url":null,"abstract":"Objective \u0000To explore the influence of further consultation adherence on the increase of degree of myopia after myopia children wear glasses. \u0000 \u0000 \u0000Methods \u0000A total of 170 cases of myopia children were collected (spectacle lens power: 340) who had received treatment in our hospital and worn glasses during June 2013 to June 2015; and were divided into three groups according to regular further consultation period, among which there were 53 children in the group with three-month regular further consultation period, 60 children in the group with six-month regular further consultation period, 57 children in the group with one-year regular further consultation period; finally analyzed the changes of refractive status for patients after wearing glasses in 6 months and one year. \u0000 \u0000 \u0000Results \u0000Since there was no significant statistic difference (F=0.194, P=0.842) in ages when the patients in three groups were original diagnosed, and there was also no statistic difference (P >0.05) in diopter of spherical power, astigmatism degree, and spherical equivalent degree, the data collected had clinical comparability. In addition, there was no clear statistic difference in the increase of spherical equivalent degree for patients, with 3-month further consultation period and 6-month further consultation period, visiting the doctor in the sixth month. As for the three groups, the spherical equivalent degree increased respectively -0.79±0.37, -0.81±0.36 and -0.92±0.40 one year later. There were statistic differences in the comparison among three groups (F=4.066, P=0.018); for the comparison between 3-month group and 6-month group, there was no statistic differences (P >0.05); however there were statistic differences (P=0.009, P=0.022) between one-year group and 3-month group, one-year group and 6-month group. Among them 19 males (19 eyes) and 9 females (9 eyes) with age ranging from 24 to 63, averages (44.00±0.00) years old. The average onset time was from 3 to 18 days, averaged (9.00±0.00) days. And hypopyon were observed in 9 cases (9 eyes). Besides topical anti-fungal Fluconazole Eye Drops and Natamycin Eye Drops, all patients also received fluorescein staining and laser photocoagulation under topical anesthesia for their cornea. Meanwhile, in vivo confocal laser-scanning microscopy were used to detect the positioning location, scope and structure changes after laser surgery of the fungal hyphae. \u0000 \u0000 \u0000Results \u0000Laser photocoagulation on the hyphal structure of fungal corneal ulcer detected by using fluorescein staining. Laser photocoagulation on the hyphal structure of fungal corneal ulcer detected by using fluorescein staining. Fungal corneal ulcer in the 28 patients (28 eyes) were all under control after the laser photocoagulation detected by using fluorescein staining. And hypopyon were decreased gradually in the 9 patients. After laser photocoagulation, the fungal mycelium structure in the ulcer was broken and melted detected by Confocal laser corneal microscopy. Noulcer recurrence was observed in 3 to 6 months after the follow-up. \u0000 \u0000 \u0000Conclusions \u0000Laser photocoagulation detected by using fluorescein staining leads to structural breakdown of the fungal hyphae of Fungal Keratitis. Confocal laser corneal microscopy is helpful to detect the positioning location, scope and structure changes after laser surgery of the fungal hyphae. From October 2006 to December 2013, totally 24 patients (24 eyes) of entire or greater than or equal to fifty percent blepharocoloboma after malignant palpebrae superior tumorectomy, admitted to Department of Ophthalmology in the Affiliated Hospital of Binzhou Medical College, were repaired with autogenous ear cartilage transplantation for the eyelid reconstruction. The autogenous ear cartilage was utilized for a substitude of tarsal plate. Eyelid tumors were removed completely, took the contralateral ear cartilage, made the length of 2 mm larger than the defect meibomian and equal width. The internal surface of ear cartilage was covered with remaining fornical conjunctiva and the external surface with orbicularis oculi and skin. \u0000 \u0000 \u0000Results \u0000All patients were followed up after operation for average 32.6 months. All the patients received refractive correction, occlusion of the sound eye and red scintillation. In addition, patients with eccentric fixation received Haidinger brush and afterimage therapy. Corrected visual acuity was considered improved as visual acuity was elevated by ≥2 lines. \u0000 \u0000 \u0000Results \u0000The best corrected visual acuity of the amblyopic eye was 0.28±0.18, 0.49±0.28, 0.60±0.32, 0.70±0.35 before treatment, after 3 months’, 6 months’ and 1year treatment. Among all the 72 patients, 55 patients’ visual acuity improved after 1-year treatment (76.4%). Only 8 patients’ visual acuity improved in 21 patients with eccentric fixation (38.1%), while 47 patients’ visual acuity improved in 51 patients with centric fixation (92.2%). 13, 33, 9 patients’ visual acuity improved in 26 severe, 37 moderate, 9 mild anisometropic amblyopia patients respectively. No one could get improved visual acuity without compliance. \u0000 \u0000 \u0000Conclusions \u0000Good results can be attained through combined therapy in older children with anisometropic amblyopia. The results are related to the initial severity of amblyopia, fixation of the amblyopic eye and compliance. During the operation, the anterior and posterior nasal mucosa flaps of \"\" type was sutured to that of lacrimal sac mucosa flaps of \"H\" type. During 6 months, the state of mucosa flaps, tear washing, and tearing symptom was followed up. Patients with median canthoal tendon interruption received tendon repairing simultaneously. Patients with obvious orbital fracture received orbital reconstruction 3 to 4 months later. \u0000 \u0000 \u0000Results \u0000Among 26 patients with traumatic dacryocystitis, 20 male, 6 female, aged 5 to 63. During the operation of all patients, the distance between the nasal mucosa and the lacrimal sac became widen. All had nasal bone, lacrimal bone fracture at different degree; 18 patients with median canthoal tendon interruption, and 14 patients with obvious orbital fracture. Tear washing was smooth in all cases at 2 weeks follow up. MiR-204 mimic and inhibitor were transiently transfected into SRA01/04 cells using Lipofectamine 2000 to regulate the expression level of miR-204. Flow cytometry was used to detect the cell apoptosis rate in each group SRA01/04 cells. By microRNA databases and recent study researches, we predicted that SIRT1 maybe a potential target gene of miR-204. Quantitative real-time PCR (qRT-PCR) were used to detect the expression level of miR-204 and SIRT1 gene. \u0000 \u0000 \u0000Results \u0000Quantitative real-time PCR results showed that the expression level of miR-204 in cell apoptosis model of SRA01/04 was significantly higher than the normal SRA01/04 cells. Overexpression of miR-204 in SRA01/04 promoted H2O2-induced cell apoptosis, and down-regulated the expression level of SIRT1 gene. While downexpression of miR-204 in SRA01/04 reduced H2O2-induced cell apoptosis, and up-regulated the expression level of SIRT1 gene. Each result was statistically significant (P <0.01). \u0000 \u0000 \u0000Conclusions \u0000The expression level of miR-204 in cell apoptosis model of SRA01/04 is significantly higher than the normal SRA01/04 cells. MiR-204 can regulate the apoptosis of the human lens epithelial cells by targeting SIRT1 gene. The changes of patients were compared before and after intervention. \u0000 \u0000 \u0000Results \u0000Compared with pre intervention, serum SOD, middle-low frequency contrast sensitivity and CLVQOL score before and after the intervention were statistically significant (P 0.05) in treated group. CLVQOL scores were positively correlated with contrast sensitivity at low spatial frequency (3 cyc/degree) in LA-treated group. \u0000 \u0000 \u0000Conclusion \u0000LA capsules can improve the antioxidant capacity of dry AMD patients and improve the quality of life of patients. The treatment group underwent lateral canthal level pull operation, the control group underwent resection under the eyelid level, lower eyelid surgery according to the position of shortened lateral canthal ligament length. The location of the lower lacrimal gland, the location of the lower eyelid, the overflow of tears, foreign body sensation and conjunctival congestion after surgery were observed. \u0000 \u0000 \u0000Results \u0000The treatment group of 23 cases (34 eyes) of lower eyelid ectropion correction in 21 days, 2 cases (2 eyes) on the seventh day after surgery for lacrimal punctum ectropion underwent medial fusiform conjunctival resection, postoperative lower eyelid function improved, foreign body sensation, epiphora symptoms subsided. The control group of 23 cases (34 eyes) of lower eyelid ectropion in 21 days was corrected in 14 cases (21 eyes), 5 cases (7 eyes) of lower eyelid function improved, palpebral margin irregular, corneal irritation and affect the appearance of 4 cases (6 eyes) to open and close the lower eyelid could not close to the ocular surface. There was no significant difference between the two groups (P >0.05). There was no significant difference in the lower lacrimal spot and eyelid position between the two groups after surgery: P >0.05. {"title":"The effects of refractive status and central corneal thickness on applanation tonometry measurements","authors":"Ning Li, Qi-Wen Zhong","doi":"10.3760/CMA.J.ISSN.1006-4443.2017.05.009","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1006-4443.2017.05.009","url":null,"abstract":"Objective \u0000To investigate the effect of the refractive status and central cornea thickness (CCT) on Goldmann applanation tonometry (GAT). \u0000 \u0000 \u0000Methods \u0000A total of 119 cases of normal subjects (195 eyes) and 74 cases of myopia (148 eyes) were included in this study. The CCT was measured by Pentacam and intraocular pressure (IOP) by using GAT. SPSS17.0 was used to process the data. \u0000 \u0000 \u0000Results \u0000The average CCT of the normal group was (525.07±38.58)μm. The mean IOP was (12.96±2.64)mmHg. The difference on both IOP and the mean CCT between low myopia group and normal group had statistically significant. The difference in IOP between high myopia group and normal group had statistically significant. The IOP of myopia group was negatively correlated with diopter (r=-0.396, P <0.001). IOP increased 1mmHg with per -3.86D increase.
