Clinical ethics support helps healthcare providers in managing moral conflicts and moral distress. CURA is a low-threshold clinical ethics support instrument. The feasibility for nurses and nurse-assistants in palliative care has already been investigated. However, the acceptability and feasibility of CURA within medical practitioners is unclear.

Methods: Prospective cohort study, using mixed methods. Participants are medical practitioners working in geriatric or palliative care, who attended a workshop on CURA. They received a baseline questionnaire (n=41) after the workshop regarding acceptability and a follow-up questionnaire (n=22) after three months regarding feasibility. Additionally, in-depth interviews (n=7) were conducted.

Results: 68% of participants expressed intention to use CURA at baseline; 32% were uncertain. 14% has actually used CURA after three months. Feasibility is dependent of several factors: (1) recognizing a moral situation; (2) familiarity with the methodology; (3) scheduling a CURA moment; (4) the role of colleagues and the healthcare organization.

Conclusion: Whereas CURA is an acceptable tool for medical practitioners, feasibility is found to be low if practitioners do not receive additional training and support.
","PeriodicalId":101340,"journal":{"name":"Tijdschrift voor gerontologie en geriatrie","volume":"55 3","pages":"2-16"},"PeriodicalIF":0.0,"publicationDate":"2024-12-17","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144791213","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

All translation discrepancies were discussed and resolved through consensus. Important measures were stricter admission requirements for nursing homes and decentralization of homecare to municipalities. The consequences of these changes for the perceived quality of care are still unknown. We examined to which extent the sufficiency of care as experienced by older people has changed during this period. We also examined the relationship with individual characteristics, such as care needs, care use, and wishes regarding care. Data from older care recipients at four observations from the Longitudinal Aging Study Amsterdam (LASA) between 2012 and 2022 (N=1316) were used. Multilevel logistic regression analysis were performed. Perceived care sufficiency decreased a little after the healthcare reform in 2015/16 and 2018/19, but recovered in 2022. There appeared no difference based on living region, but care sufficiency was higher in less urban areas (OR=0,89). In addition, functional limitations (OR=0,94), chronic diseases (OR=0,80), use of public care (OR=0,67), and higher education level (OR=1,53) were strongly associated with care sufficiency. However, use of publicly paid care appeared to be the type of care that contributed least to care sufficiency; a lack of perceived control over care may be an explanatory factor.
","PeriodicalId":101340,"journal":{"name":"Tijdschrift voor gerontologie en geriatrie","volume":"55 3","pages":"20-37"},"PeriodicalIF":0.0,"publicationDate":"2024-09-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144791214","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Sometimes the blisters are preceded by years of itching and eczematous patches, which can cause a delay in recognition of BP. The disease is caused by production of autoantibodies against the hemidesmosomal proteins BP180 and BP230 at the dermal-epidermal junction. An association between BP and neurodegenerative disorders, such as Parkinson's disease, is known. However, this association is not commonly known amongst health care professionals, but is of clinical significance. Neurodegenerative disorders (including Parkinson's disease) are thought to disrupt the blood brain barrier, resulting in exposure of neuronally located BP180 and BP230 to the immune system. In most studies that have examined the association between BP and Parkinson's disease (and other neurodegenerative disease), BP develops several years after the diagnosis of Parkinson's. We present a case of a patient with proven bullous pemphigoid who developed clinical features of Parkinson's disease during follow-up at the dermatology outpatient clinic. Mainly anticholinergic side effects can lead to central and peripheral side effects on somatic and cognitive levels. In this study, we determined the anticholinergic load of the medication regimens of residents of a residential care centre using two validated measurement scales. We also examined to which extent the class(es) of medications influence this anticholinergic load. Finally, we analysed the association of a high anticholinergic load with incidents (urinary tract infections, falls, delirium).In this pilot study, we see a high degree of polymedication and use of psychotropic drugs with an anticholinergic effect. On average, we established a score of 1.67 on the ACB and a score of 0.87 on the DBI. The anticholinergic load in the ACB is mainly determined by antipsychotics (β = 0,647, p ‹ 0.001), in the DBI an influence of all medications with total anticholinergic and sedative load (F = 6.764, p ‹ 0.001). A risk score on the DBI has a statistically significant correlation with experiencing a urinary tract infection (F = 5.877, p = 0.018). This influence remains significant after adjusting for covariates.The anticholinergic load of medication regimens in residents is high. An increased anticholinergic load is also associated with urinary tract infection. Reducing the anticholinergic load within residential care centres therefore represents an important challenge for the future.
","PeriodicalId":101340,"journal":{"name":"Tijdschrift voor gerontologie en geriatrie","volume":"55 2","pages":"2-15"},"PeriodicalIF":0.0,"publicationDate":"2024-06-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144791209","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
