Le diagnostic a été chaque fois évoqué devant la survenue d'un accès fébrile, une hémolyse brutale, une hémoglobinurie, un ictère cutanéomuqueux et une insuffisance rénale aiguë consécutive à une prise d'antipaludéens (sulfadoxine-pyrimétamine [Fansidar<sup>®</sup>], acide acétylique et sels de quinine). Les investigations paracliniques ont permis d'objectiver un déficit enzymatique en glucose 6 phosphate déshydrogénase (G6PD). Au cours de la seconde hospitalisation, la prise en charge efficace a été possible grâce à de nombreuses transfusions sanguines et à l'épuration extrarénale. La morbidité qui en découle reste préoccupante, notamment le risque de complications infectieuses nosocomiales et transfusionnelles.</p></div><div><p>We report a case of blackwater fever after an interval of six years in Ivory Coast. On each occasion, the presentation was fever, acute haemolysis, heamoglobinuria, jaundice, and acute renal failure following antimalarial treatment on the first occasion with sulfadoxine-pyrimetamine and aspirin, the second time with quinine salts. Laboratory investigations at the time of the second hospital admission revealed G6PD deficiency. The patient was successfully treated with multiple transfusions and heamodialysis. Cependant, de telles découvertes ne peuvent être systématiquement retenues comme cause de l'accident cérébral. Des critères de franche perméabilité sont ainsi nécessaires, d'autant que les mécanismes physiopathologiques restent encore non résolus. Les agrégants plaquettaires et les antivitamines K sont généralement prescrits, mais leurs indications respectives et la durée du traitement, de même que la fermeture chirurgicale des pathologies, ne font pas encore l'objet d'un consensus.</p></div><div><p>Following ischemia in young patients, trans-œsophageal echocardiography may discover anomalies such as patent foramen ovale or/and atrial septal aneurysm. We describe the observation of transitory stroke in a 32-year old man in which the etiology can be reported as paradoxical embolism. To incriminate patent foramen ovale or atrial septal aneurysm in stoke, massive passage contrast through the abnormal structures, peripheric phlebitis or concomitant pulmonary embolism physiopathology remains uncertain. Nous décrivons ici le cas d'une femme de 39 ans, sans antécédents médicaux, ayant volontairement absorbé une dose supposée équivalente à 6 g de Flécaïne<sup>®</sup> (flécaïnide, classe lc de Vaughan-Williams) associée à plusieurs comprimés de Corgard<sup>®</sup> (nadolol, bêtabloquant). Malgré une prise en charge intensive et précoce en réanimation, l'évolution de cette intoxication va devenir défavorable, marquée par un état de choc cardiogénique sévère, réfractaire à toute thérapeutique, avec des troubles graves du rythme cardiaque et de la conduction. Le taux de Flécaïne<sup>®</sup> retrouvé dans le sang (1,9 mg/L) va alors démontrer un taux plasmatique certes toxique, mais en fait peu en rapport, d'une part avec la sévérité clinique de l'intoxication et, d'autre part, avec la dose de 6 g supposée ingérée par la patiente. C'est pourquoi, l'ingestion associée d'un bêtabloquant pourrait, en tant que facteur aggravant, avoir précipité l'évolution de cette intoxication vers une issue malheureusement fatale.</p></div><div><p>Some poisonings with antiarrythmic drugs still have a poor prognosis, especially in cases of voluntary massive ingestion, despite progress in intensive care facilities and procedures. This case report concerns the suicidal intoxication of a 39-year old female without any medical history who ingested six grams of flecainide (Vaughan-Williams class Ic), probably associated with nadolol (beta-adrenergic blocking agent). Despite rapid intensive care, the outcome was unsuccessful, featuring a cardiac shock without response to drugs and severe cardiac rhythm and conduction problems. Flecainide blood level (1.9 mg/L) showed a toxic concentration, but was uncorrelated with the clinical severity and the ingested dose of six grams.
