Secondairement, il incombe au soignant de choisir les pansements favorisant la cicatrisation en répondant à l’objectif de soin. Les pansements secondaires et les moyens de fixation possèdent un rôle primordial en cicatrisation dirigée et ne se résument pas à de simples systèmes de maintien. Leur diversité garantit une individualisation du soin. Comme tout dispositif médical, ils sont soumis à des règles d’utilisation et de prescription que les soignants doivent connaître.</p></div><div><p>Whatever the wound, its treatment is primarily that of its etiology. Secondarily, it is the responsibility of the caregiver to choose dressings that promote healing in a humid environment by meeting the goal of care. The secondary dressings and the attachment means have a primary role in directed healing and are not limited to simple holding systems. Their diversity guarantees individualization of the treatment. Des évaluations précliniques et cliniques, selon des critères exigeants, sont nécessaires pour obtenir un marquage de conformité européenne avant commercialisation. À l’hôpital, le tarif du pansement est inclus dans le forfait du séjour. En ambulatoire, le tarif est standardisé si le pansement est inscrit sur une ligne générique, ou négocié s’il présente des revendications spécifiques reconnues.</p></div><div><p>Dressings are constantly evolving thanks to technological advances and leading to modern devices for targeted action. Preclinical and clinical evaluations, according to demanding criteria, are necessary to obtain a CE mark before marketing. In the hospital, the dressing is financed in the hospitalization benefits. Des premiers soins protocolisés, pratiqués en leur temps par les Égyptiens, aux pansements dits “intelligents”, son histoire est jalonnée de découvertes associées pour certaines à d’illustres noms : Hippocrate, Galien ou Ambroise Paré pour ne citer qu’eux. Les progrès de la médecine ont permis, à la faveur de certains événements, de donner naissance à une gamme étendue de pansements. Les hydrocolloïdes et les hydrocellulaires ont ouvert la voie. Ainsi, depuis la fin du xx<sup>e</sup> siècle, les progrès de la médecine apportent régulièrement de nouveaux pansements qui agissent sur les mécanismes intimes de la plaie.</p></div><div><p>Since the Egyptians, who already practiced protocolized care through Hippocrates, Galen and Ambroise Pare among others, we have gone back to the present day. We have discovered according to the times, a wide range of dressings, sometimes folkloric but also not without common sense. Hydrocolloids and hydrocellulars have opened the way. La liste des produits et prestations remboursables (LPPR) fixe les performances minimales que doivent réaliser ces pansements in vitro en matière d’absorption libre, de rétention et d’absorption sous charge et décrit les méthodes permettant cette évaluation. L’objectif de ce travail a été de comparer, in vitro, les performances de 11 pansements superabsorbants de taille 10 cm × 10 cm environ, disponibles sur le marché français. Les résultats montrent que tous les pansements testés répondent aux spécifications de la LPPR mais que les différences entre pansements sont surtout liées à des différences de surface absorbante.</p></div><div><p>Superabsorbent dressings are characterized by their high capacity for absorption and retention of exudates, including during associated compressive treatment. The List of Reimbursable Products and Services (LPPR) sets the minimum performance that these in vitro dressings must perform in terms of free absorption, retention and absorption under load and describes the methods allowing this evaluation. The objective of this work was to compare, in vitro, the performance of 11 superabsorbent dressings about 10cm x 10cm, available on the French market. Aussi, une détersion mécanique, effectuée au lit du malade (par un médecin ou un infirmier) ou chirurgicale, réalisée au bloc opératoire, s’impose le plus souvent ; elle est complétée par une détersion autolytique faisant appel à des pansements techniques. Parmi les nombreux pansements modernes conçus sur le principe de la cicatrisation en milieu humide, certains sont plus adaptés à la phase détersive. Tous ne sont pas disponibles et remboursés en France en 2019. État des lieux.</p></div><div><p>For any wound, the existence of fibrin or necrosis is an obstacle to the formation of the granulation tissue and creates an environment conducive to bacterial proliferation.
