Lors de la prise en charge des patients, après brûlure grave notamment, la gestion de l’environnement mécanique de la plaie mais également l’exposition au rayonnement ultraviolet font partie des facteurs à maîtriser pour promouvoir une cicatrisation de qualité.</p></div><div><p>During the skin repair process, myofibroblasts play a key role in remodeling granulation tissue and scar maturation but may also participate in the formation of hypertrophic scars. L’objectif de cet article est d’aborder dans un premier temps, le vieillissement cutané et ses conséquences sur la cicatrisation, puis les troubles de la cicatrisation consécutive au vieillissement cutané et enfin les comorbidités fréquentes dans cette population pouvant entrainer des troubles de la cicatrisation. Du fait de ces spécificités, la cicatrisation chez le sujet âgé est souvent complexe, et doit être pluridisciplinaire pour permettre une prise en charge optimum.</p></div><div><p>Proportion of elderly individuals in the population has steadily increased worldwide: practitioners are now often confronted to wound healing and its specificities in elderly patients. The objective of this article is to address skin aging and its consequences on skin healing, disorders of skin healing due to dermatoporosis and specific comorbidities of the elderly that can cause wound healing perturbation. Les incisions chirurgicales ont été fermées à l’aide d’agrafes chirurgicales, de sutures ou de Steri-StripsTM, et recouvertes avec le nouveau pansement adhésif hydrocellulaire. La performance du pansement a été observée quotidiennement jusqu’à la cicatrisation de la plaie jusqu’à deux semaines maximum. Les résultats cliniques confirment que le nouveau pansement hydrocellulaire adhésif est sûr et efficace dans le traitement post-chirurgical des plaies ; il permet d’optimiser la gestion des plaies, d’améliorer la qualité de la vie des patients et d’obtenir une cicatrisation de première intention.</p></div><div><p>The objective of this prospective clinical study, carried out on a sample of 20 patients, was to evaluate the adherence of a new hydrocellular adhesive dressing on surgical incisions and the absence of irritation of perilesional skin. The surgical incisions were closed with surgical staples, sutures or Steri-StripsTM and covered with the new hydrocellular adhesive dressing. The performance of the dressing was observed daily until healing of the wound for up to two weeks maximum. The skin is one of the organs most affected by addictions. We can distinguish many drugs and many ways to consume them which are the origin of acute and chronic cutaneous complications. Les mécanismes responsables d’une hyperpression veineuse, d’une insuffisance artérielle ou d’une atteinte microcirculatoire sont différents mais aboutissent à une diminution de la perfusion tissulaire. Le dépistage de ces pathologies doit être réalisé pour toutes les plaies et un traitement instauré le plus tôt possible pour permettre une cicatrisation rapide.</p></div><div><p>Vascular pathologies can cause trophic disorders or be responsible for a delay in healing. The mechanisms responsible for venous hypertension, arterial insufficiency or microcirculatory involvement are different but result in a decrease in tissue perfusion.
