The number of procedures has increased exponentially in recent years, but because it is a surgery, complications are inherent, with perioperative bleeding being one of those that generates the most interest in the profession due to its high associated morbidity and mortality. The objective of this research was to describe the clinical factors involved in major perioperative bleeding and the need for blood transfusion in TKA.</p></div><div><h3>Material and methods</h3><p>Descriptive observational study based on retrospective data in patients undergoing TKA. An analysis of the variables was made, taking into account perioperative blood loss and the need for allogeneic red blood cell transfusion.</p></div><div><h3>Results</h3><p>777 patients were recruited, with a mean age of 69 years (SD: 9.7). Four patients were transfused with cumulative incidence of 0.5% (95% CI: 0.14 - 1.3), 3 of them were revision surgeries. The blood loss of the patients who required transfusion was on average 1590<!--> <!-->ml vs 730<!--> <!-->ml of those who did not require it, the surgical time was 115<!--> <!-->minutes vs 74<!--> <!-->minutes and the preoperative hemoglobin 14.8<!--> <!-->g/dl vs 12.7<!--> <!-->g/dl respectively.</p></div><div><h3>Conclusions</h3><p>Allogeneic transfusions in TKA occurred more frequently in surgeries with prolonged surgical time and in revision surgeries. Osteoarticular extra pulmonary involvement corresponds to 1-3% of cases; 50% correspond to the spine and the minority to small joints.</p></div><div><h3>Case report</h3><p>We present the case of a 21-year-old patient who is seen in clinic for pain and functional limitation of the ankle that started 14 months prior. The patient had no respiratory symptoms. After 18 months, and with reactivation of pulmonary symptoms, a diagnosis of septic arthritis due to tuberculosis was confirmed with culture and real-time polymerase chain reaction (PCR).</p></div><div><h3>Results</h3><p>Due to chronic septic arthritis, the patient presents severe and irreversible joint damage of the ankle. There are no episodes of relapse or recurrence of joint infection after 2 years of follow-up.</p></div><div><h3>Discussion</h3><p>The extra pulmonary presentation of tuberculosis infection is infrequent, especially in small joints. It is important to consider differential diagnoses when managing it. Chronic septic arthritis due to TBC can mimic any other type of disease, hence, microbiological diagnostic aids must be available. More frequent in the lumbar spine, then dorsal and finally cervical. From 2 to 7% of bone infections, mortality rate from 1 to 20%. Hematogenous spread (common).</p><p>The most common germ is <em>Staphylococcus aureus</em>, followed by <em>Streptococcus</em>, <em>Mycobacterium tuberculosis</em>, and <em>Enterobacteriaceae</em>. <em>Chryseobacteria</em> are aerobic gram-negative rods <em>Chryseobacterium indologenes</em> is the most frequent, common in immunocompromised patients.</p></div><div><h3>Clinical case</h3><p>55-year-old male with a history of lumbar surgery in 2014, due to degenerative pathology. He presented a surgical site infection and a surgical toilet was performed, without obtaining germ, empirical antibiotic treatment (ciprofloxacin and TMS).</p><p>Years later he presented progressive lumbar and radicular pain. Loosening of the radiological implant, and spondylodiscitis in MRI. Biopsy under CT, <em>Staphylococcus epidermidis</em>, sensitivity (ciprofloxacin and rifampicin).</p><p>Abscess is drained and prosthesis removed. Positive cultures for <em>C. indologenes</em>, susceptibility (levofloxacin and rifampicin).</p></div><div><h3>Conclusion</h3><p>Epidural abscess is a rare but potentially devastating condition. An angle in the coronal plane of 63–76° has been reported in both healthy and post-surgical patients.</p></div><div><h3>Objective</h3><p>To determine the coronal plane angle of the ACL <em>in vivo</em> in Colombian adult population through arthroscopy.</p></div><div><h3>Materials and methods</h3><p>Study case series, where 47 adult patients were included who underwent knee arthroscopy between October 2018 and March 2019, obtaining intra-surgical images of the ACL with a lenses of 0° and 30° through transpatellar portal and anterolateral portal, ACL angle was measured in each arthroscopy. Functionality and pain were obtained using the Knee Society Score (KSS) scale, with a different score for each variable. Finally, survival was defined as those prostheses that had not required revision as primary outcome. Other clinical and surgical variables were determined according to their nature by means, medians or frequencies.</p></div><div><h3>Results</h3><p>Patient satisfaction was 88.2, KSS was 83 for knee component and 76 for functionality. The HRQOL SF-36 scores grouped in two main components – physical function and mental health – were 53.3 and 60, respectively. Survival specified in this study was 77.3% in a 9.05 years average follow-up.</p></div><div><h3>Conclusions</h3><p>Unicompartmental knee arthroplasty is a valid treatment option for osteoarthritis of the medial compartment. In this series of patients, the surgery satisfaction was 88.2%, the average survival was 77.3% at the end of a follow up of 9.05 years, in which the main causes of revision were infection and osteolysis. Our objective is to evaluate the intraobserver and interobserver variability of the Gilula lines in perilunate dislocations and the agreement between general orthopaedic surgeons, hand surgeons and residents of orthopaedic surgery, who participated in the study.</p></div><div><h3>Materials and methods</h3><p>Six observers evaluated 30 PA carpal X-rays, including 15 with a diagnosis of perilunate dislocation and 15 without carpal lesions. These observers were the residents of orthopaedic surgery; orthopaedic surgeons not specialised in hand pathology and hand surgeons. All had to classify the X-rays as normal or pathological attending only to Gilula's lines. The purpose of this research is to describe the risk factors and prevalence for DDH in children born between 01/01/2016 and 12/31/2019 in a Children's Hospital in Bogotá, Colombia.</p></div><div><h3>Materials and methods</h3><p>Descriptive cross-sectional study. The Developmental Hip Dislocation (DHD) was clinically evaluated through Ortolani, Barlow and Galeazzi's signs, exploring the possible association with family history in the first and second degree of consanguinity. 2<!--> <!-->×<!--> <!-->2 contingency tables were created and the <em>X</em><sup>2</sup> test or Fisher's exact test was applied, considering a statistical significance of (<em>p</em> <!-->&lt;<!--> <!-->0.05). The information was taken from the DHD screening program database. The descriptive analysis was performed with Stata15 and SPSS20.</p></div><div><h3>Results</h3><p>3193 neonates meet the inclusion criteria. 49.5% are female. 7.1% had a family history of DHD in the first degree of consanguinity, 5% in the second degree of consanguinity, and 9.1% had a breech presentation. Clinical evaluation for the detection of DHD was positive in 0.41% (<em>n</em> <!-->=<!--> <!-->13). A prevalence of 2.2× every 1000 live births was documented.</p></div><div><h3>Conclusions</h3><p>No association between sex and history of first degree of consanguinity was documented, but and an association with second degree history was founded. The estimated prevalence in a single hospital highlights the importance of strengthening and expanding screening programs.</p><p>Level of evidence: III.
