Here we evaluated the effectiveness of neoadjuvant chemotherapy in downgrading the tumor combined with arthrodesis of the wrist by ulnar translocation. Neoadjuvant chemotherapy was administered to reduce tumor size and vascularity, resection was followed by arthrodesis of the wrist through ulnar translocation to stabilize the joint and restore function.</p></div><div><h3>Materials and methods</h3><p>A retrospective analysis was conducted on a cohort of patients with distal radius GCT who underwent this combined treatment approach. Clinical and radiographic assessments were performed preoperatively, post-chemotherapy, and at follow-up visits. Objectively, the effect of chemotherapy of tumor size was calculated and the resected length was calibrated intra-operatively. Functional outcomes were evaluated using revised MSTS score, grip strength, and functional range of motion.</p></div><div><h3>Results</h3><p>The study included 10 patients with a mean age of 26.6 years (18–32 years). Neoadjuvant chemotherapy resulted in a significant reduction in tumor size in all cases. The subsequent arthrodesis procedure using ulnar translocation achieved excellent stability and alignment of the wrist joint. The mean follow-up was 9.2 ± 4.21 months demonstrating reduced pain, improved grip strength of 65.77 ± 11.63 % (compared to the contralateral hand), and restored range of motion of 82.750 ± 1.440 of supination and 81.740 ± 2.340 of protonation. Of the 10 cases, one had soft tissue recurrence and one had delayed radio-ulnar union. The mean revised MSTS score was 24.9 ± 1.85 at the final follow-up.</p></div><div><h3>Conclusion</h3><p>Neoadjuvant chemotherapy is an effective approach for downgrading distal radius GCT and reduces the chances of recurrence. The complex network of vessels, nerves, and muscular structures further complicates surgical procedures, particularly in the context of tumor surgery. While the upper extremity is not a common site for tumors, it can occasionally be affected. Achieving negative surgical margins in this difficult area is particularly challenging. In this study, we discuss evidence-based strategies employed in the treatment of several rare upper extremity tumors and present the first reported case of an upper extremity-originated chordoma.</p></div><div><h3>Methods</h3><p>This retrospective case series adheres to the PROCESS guidelines. Patients with rare bone or soft tissue tumors treated in our orthopedic oncology unit between January 2022 and December 2023 were included. Data were collected from electronic medical records, including demographics, clinical presentation, imaging studies, surgical interventions, and postoperative outcomes. Diagnostic workup involved physical examination, radiographic imaging, and histopathological analysis. Surgical interventions were based on tumor type and location. Primary outcomes included symptom resolution, postoperative complications, and recurrence rates. Secondary outcomes focused on functional recovery.</p></div><div><h3>Results</h3><p>Case 1:A 21-year-old male with wide spread synovial chondromatosis of shoulder underwent sequential arthroscopic and open excision of lesions. Case 2: A 27-year-old female with intraosseous schwannoma and pathological fracture received curettage and intramedullary nailing after confirmation with pre operative biopsy. Case 3: A 37-year-old male with a paraganglioma had the mass excised completely after pre opeative diagnosis with biopsy. Case 4: A 60-year-old female with extra-axial chordoma of the proximal humerus underwent wide resection and prosthetic reconstruction. All cases achieved successful oncological and functional outcomes with no complications.</p></div><div><h3>Conclusion</h3><p>Evidence-based surgical strategies, multidisciplinary collaboration, and advanced techniques can achieve successful oncological and functional outcomes in the upper extremity. This review also aims to identify key risk factors, such as osteoporosis and undiagnosed vertebral fragility fractures, that contribute to these disparities and explore potential strategies for improving patient outcomes.</p></div><div><h3>Method</h3><p>A comprehensive literature search was conducted using electronic databases, including PubMed, MEDLINE, Embase, and Cochrane. The search focused on studies published between January 2010 and September 2021 that investigated health disparities in the management of fragility fractures in older adults within LMICs. Relevant studies were selected based on predefined inclusion criteria, resulting in a total of 14 studies being included in the review.</p></div><div><h3>Results</h3><p>The reviewed studies highlight significant health disparities in the management of fragility fractures among the geriatric population in LMICs. Key contributing factors to these disparities include limited access to healthcare, inadequate resources, poor infrastructure, insufficient training of healthcare professionals, and cultural beliefs. Additionally, the review identifies the high prevalence of osteoporosis and undiagnosed vertebral fragility fractures as critical risk factors that exacerbate these disparities. These factors collectively contribute to delayed diagnoses, suboptimal treatment, and poorer patient outcomes, including higher morbidity and mortality rates.</p></div><div><h3>Conclusion</h3><p>Health disparities in the management of fragility fractures in the geriatric population of LMICs present a significant public health challenge. Addressing these disparities requires a multifaceted approach, including improving healthcare infrastructure, increasing access to care, enhancing the training of healthcare professionals, and developing culturally-appropriate interventions. Orthopedic procedures, especially those involving implants, are highly susceptible to infections, which can be catastrophic when caused by resistant organisms.</p></div><div><h3>Causes</h3><p>Antimicrobial resistance (AMR) in orthopedic surgeries is driven by several causes and contributing factors. These include prolonged hospital stay and increased use of implants (which can serve as surfaces for biofilm formation by resistant bacteria). Inadequate sterilization of surgical instruments and poor infection control practices further exacerbate the risk of introducing resistant organisms.</p></div><div><h3>Conclusions</h3><p>Addressing AMR in orthopedics necessitates a multifaceted approach, including stringent antibiotic stewardship programs, robust infection control practices, and ongoing research into new antimicrobial agents and alternative treatments. This article provides detailed insights into AMR in the context of orthopedic surgeries. The aim of this review is to systematically evaluate existing literature on bilateral patella fractures, specifically traumatic fractures, with special focus to mechanism of injury, fracture type, management options, and outcomes.</p></div><div><h3>Methods</h3><p>A comprehensive search for literature was conducted in PubMed, Embase, The Cochrane Library, and Google Scholar, including the period from January 01, 1913 to December 31, 2023.</p></div><div><h3>Results</h3><p>The search for reviews resulted in a total of 18 studies. The review included a total of 19 patients with bilateral simultaneous fractures of the patellae. Out of the 19 patients included, 13 were male and six were female. The mean age was 42.1, with a range of 17 years to 85 years. The mean follow-up was 9.3 months (2–24 months).</p></div><div><h3>Conclusion</h3><p>Trauma represents the most common injury mechanism in patients with bilateral simultaneous patella fractures. In this method, bones are positioned and secured by pins of the external fixator system. Although the system provides a reliable solution for fixing the fracture site, the most common problem in clinical practice of this method is pin site failure due to pin micro motions. This problem leads to the loosening of the pins which results in infection at the pin-bone interface that's called pin tract inflammation. The aim of this study is to reduce pin motions by altering the design of conventional Schanz pin so, a new pin, called <strong>M</strong>icro-<strong>M</strong>otion <strong>D</strong>amping <strong>P</strong>in (MDP).</p></div><div><h3>Methods</h3><p>MDP was compared with the Schanz pin in two methods. The first method compared inter-pin motion behaviors on a same external fixator bar that was set on polyethylene rods. The second method compared bending behaviors. A universal axial test machine was used to apply force in the experiments, and a non-contact video extensometer was used to capture inter-pin motions between the first and the third pins on the proximal fragment.</p></div><div><h3>Results</h3><p>Although, the results showed that there was not a statistically significant differences between two pin designs in reducing the inter-pin motions (p = 0,240), the measured displacement of the MDP was less than the Schanz pin when compared. Displacements were 0,117 ± 0,02mm and 0,142 ± 0,065mm for the MDP and the Schanz pins respectively.</p></div><div><h3>Conclusion</h3><p>The results point out the MDP absorbed some amount of the inter-pin motion when compared to Schanz pins. Moreover, the MDP's mechanical results were superior in bending tests, and the difference was statistically significant (p = 0,002). Chippaux-Smirak index, Staheli index, and Clarke's angle was measured with ink footprint method and photographic footprint. The intra-class and inter-class correlation coefficient (ICC) was evaluated for reliability(r).</p></div><div><h3>Results</h3><p>The photographic glass box method and ink footprint method, showed the intra-rater and inter-rater reliability test values greater than 0.9 and the ICC range varies from 0.92 to 0.98, which is classified as excellent reliability.</p></div><div><h3>Conclusion</h3><p>Current study showed excellent intra-rater and inter-rater reliability of Photographic footprint glass box method evaluated by Chippaux- Smirak index, Staheli index and Clarke's angle in young healthy adults of 19–21 years. Various percutaneous treatment options have been explored, but incompletely evaluated in comparison to one another in terms of rigidity of fixation at the fracture site.</p></div><div><h3>Methods</h3><p>A biomechanical study was performed with 9 pairs of matched cadavers after creation of a suprasyndesmotic fracture model as occurs in association with a tibial pilon fracture. One group in each pair was assigned to threaded screw and the other to smooth pin fixation. Displacement at the fracture site was evaluated before and after cyclic axial and rotational loading with an MTS frame through video dimensional analysis.</p></div><div><h3>Results</h3><p>In vertical loading alone, smooth pin and threaded screw fixation demonstrated no significant difference in displacement and there was no change in performance before and after loading cycles. With external rotation combined with a modest vertical load, screw fixation demonstrated smaller displacements than pin fixation, but the difference was less than 1 mm.</p></div><div><h3>Conclusions</h3><p>The results suggest that both smooth pin fixation and threaded screw fixation can achieve stable fixation in a transverse fibular fracture. {"title":"The impact of nutritional status on the success of the ponseti method in clubfoot patients: A systematic review","authors":"Muhammad Muzzammil ,&nbsp;Muhammad Owais Minhas ,&nbsp;Amna Jamil","doi":"10.1016/j.jorep.2024.100461","DOIUrl":"10.1016/j.jorep.2024.100461","url":null,"abstract":"<div><h3>Purpose</h3><p>This literature review aims to assess the impact of malnutrition on the Ponseti method, a widely used non-surgical approach for treating clubfoot, and to evaluate the importance of nutritional status in achieving successful outcomes.</p></div><div><h3>Methods</h3><p>A systematic search was conducted on PubMed, Embase, and Scopus databases using the keywords \"malnutrition,\" \"Ponseti method,\" \"clubfoot,\" \"non-surgical treatment,\" \"surgical outcomes,\" and \"nutritional status.\" Studies published between 2010 and 2023 were considered for inclusion. A total of 12 relevant articles were reviewed.</p></div><div><h3>Results</h3><p>Malnutrition is a significant risk factor for poor outcomes in clubfoot patients treated with the Ponseti method. Malnourished patients are more likely to experience delayed wound healing, increased infection risk, impaired cognitive function, and decreased bone density, all of which can negatively impact the success of the treatment. Malnourished children also have a higher relapse rate and require a longer duration of treatment compared to well-nourished patients. Improving nutritional status through appropriate measures is crucial for achieving a successful outcome. Vitamin D deficiency is a prevalent issue in clubfoot patients, and addressing this through supplementation has been shown to improve success rates.</p></div><div><h3>Conclusions</h3><p>The results of this review highlight the importance of assessing nutritional status in clubfoot patients undergoing the Ponseti method. Addressing malnutrition and optimizing nutritional status can prevent complications, reduce treatment duration, and increase the success rate of the treatment.
