Yet, information about the association between DHEA level and knee OA is lacking. Objectives: To explore the link between serum dehydroepiandrosterone sulfate (DHEAs) levels and knee OA among elderly patients. Methods: A case control study was conducted on 80 elderly subjects (40 males, 40 females) aged 60 years and older attending the outpatient clinics in Ain Shams University Hospital. Participants underwent a standard clinical examination of the knee, assessment of physical difficulty and pain severity using WOMAC OA index. Weight-bearing anteroposterior radiographs of the knees in the semi-flexed position were performed. Serum levels of DHEAs were examined. Results: The serum level of DHEAs in males with knee OA was 0.29 ± 0.17 μg/mL compared to those without knee OA 0.59 ± 0.51 μg/mL (p=0.006), small but significant difference existed between the serum level of DHEAs in females with knee OA 0.25 ± 0.11 μg/mL compared to those without knee OA 0.39 ± 0.26 μg/mL (p=0.044*). Additionally, the serum DHEAs level negatively correlated with the severity of knee OA in both sexes. Conclusions: There was a sex difference in serum DHEAs level and occurrence of knee OA. Subsequently, although she did not show typical manifestations of systemic lupus erythematosus (SLE), she was diagnosed as SLE complicated with Sjögren's syndrome and Greves’ disease. While AIHA occurs in 5-10% of SLE patients and SLE is the most common underlying condition of mixed-type AIHA, complication of mixed-type AIHA among SLE patients has not been studied yet. SLE mainly affects women of child-bearing age and is seldom diagnosed in elderlies and the present case is the oldest SLE patient presented with mixed type-AIHA reported so far in the literature, indicating that mixed-type AIHA may complicate in elderly SLE patients and thorough evaluation of the possibility of the complication is justified. {"title":"Organ Toxicity from Benzene Exposure among Elderly Subjects after a Flaring Disaster at the BP Refinery Plant in Texas City","authors":"M. Methods: Elderly subjects aged 60 years and older who had been exposed and unexposed to benzene were included. We reviewed medical charts and compared measures of white blood cells (WBC), platelets, hemoglobin, hematocrit, blood urea nitrogen (BUN), creatinine, alkaline phosphatase (ALP), aspartate amino transferase (AST), and alanine amino transferase (ALT) in exposed and unexposed elderly subjects. Results: Records from 294 elderly subjects (benzene exposed, n=216 and unexposed, n=78) were reviewed. Benzene exposed subjects had significantly higher levels of WBC (X 103 per μL) (7.7±1.9 versus 6.3±1.5, P=0.0000) and platelet (X 103 per μL) counts (256.8 ± 51.6 versus 237.9 ± 41.9, P=0.0104) compared with the unexposed subjects. Serum creatinine levels (mg/dL) were also significantly increased in the exposed group compared with the unexposed group (1.1 ± 0.4 versus 0.9±0.2, P=0.000). Serum levels of ALP (IU/L) were significantly elevated in the exposed subjects compared with the unexposed subjects (87.5 ± 23.6 versus 72.5 ± 17.8, P=0.000). Similarly, benzene exposed subjects had significantly higher levels of AST (24.8 ± 6.2 versus 19.2 ± 5.1, IU/L, P=0.000) and ALT (24.2 ± 8.6 versus 19.1 ± 4.8, IU/L, P=0.000) compared with those unexposed to benzene. Non-pharmacological treatments of BPSD have gained increasing attention since the use of pharmacological methods has been found to be associated with negative consequences and risks. Behavioral management of BPSD is one of the non-pharmacological treatment approaches that has been utilized in psychosocial treatment programs. Behavioral strategies have also been used in the treatment of caregiver distress and depression. This paper is based on a comprehensive search of literature in PubMed/MEDLINE and Scopus. It reviews the literature to evaluate the efficacy and utility of behavior management in helping with reduction of problem behaviors along with psychological distress in caregivers of patients with dementia. In 2005, the Food and Drug Administration (FDA) issued alerts regarding their use as antipsychotics in elderly patients with dementia-related psychosis. Guidelines on psychotropic use were also recently established in Japan; however, few studies have reported on the actual usage of psychotropics among patients with dementia in clinical settings. The aim of this study was to estimate the frequency of psychotropic use in patients with Alzheimer's disease receiving donepezil.</p></div><div><h3>Methods</h3><p>Data from six prospective, postmarketing studies of donepezil in patients with Alzheimer's disease conducted between 1999 and 2011 were pooled for analysis. Patient and clinical characteristics at baseline were analyzed descriptively. The proportion of psychotropics used concomitantly with donepezil was calculated for all patients and for patients divided into types of psychotropics used, sex, age, and study time period. As for the study time period, studies were divided into two groups: the period before the FDA alert was issued (1994–2004) and the period after (2005–2011).</p></div><div><h3>Results</h3><p>A total of 14,726 patients were analyzed (67.0% women, aged 79.3 ± 7.2 years). The overall proportion of psychotropic use was 24.2%. The number of psychotropics used was one in 8.2% of patients, two in 8.1%, and ≥ 3 in 5.5%. The types of psychotropics used were as follows: typical antipsychotics, 7.8%; atypical antipsychotics, 4.4%; antidepressants, 5.7%; anxiolytics, 6.4%; and hypnotics, 11.0% (% does not sum up to 24.2% due to multiple use). With regards to the time periods, in studies conducted between 1999 and 2004 versus those between 2005 and 2011, the proportions were 31.7% and 21.3%, respectively.</p></div><div><h3>Conclusion</h3><p>Psychotropic use was seen in a quarter of patients with dementia receiving donepezil in Japan.
