{"title":"Retrospective analysis of post-treatment apical periodontitis and quality of endodontic treatment and coronal restorations in an elderly Turkish population","authors":"Evren Sarıyılmaz PhD, DDS , Cangül Keskin DDS , Öznur Özcan DDS","doi":"10.1016/j.jcgg.2015.08.003","DOIUrl":"https://doi.org/10.1016/j.jcgg.2015.08.003","url":null,"abstract":"<div><h3>Purpose</h3><p>The aim of this cross-sectional study was to determine the prevalence of apical periodontitis in 1768 root canal-treated teeth in an elderly Turkish population and to evaluate correlations between several factors and periapical status.</p></div><div><h3>Methods</h3><p>Panoramic radiographs were collected of 818 patients who were older than 60 years and attended the Faculty of Ordu University Dentistry for dental care. Teeth were classified as healthy or diseased, according to the periradicular status. Root canal filling, coronal restoration quality, and presence of postretained restorations were also evaluated.</p></div><div><h3>Results</h3><p>The prevalence of apical periodontitis was 26.7% in the elderly population. A statistically significant association between root canal filling quality and periapical health was detected. Coronal restoration quality also exhibited a statistically significant relationship with periapical health. No significant difference was recorded between the periapical status of teeth restored with and without posts.</p></div><div><h3>Conclusion</h3><p>Both technical quality of root canal filling and coronal restoration play key roles in endodontic treatment outcome. Community-based organizations (CBOs) are important underutilized partners in activating and providing the resources necessary to deliver coordinated, culturally-tailored services to older individuals. This paper describes the process and preliminary results of creating and implementing a “Wellness Pathway” to improve the health of older individuals by coordinating care between health care providers and CBOs, such as senior centers. The Wellness Pathway provides a mechanism whereby health care providers refer patients to programs at the senior center, and senior center staff members refer patients for urgent or primary care services at the health care provider organization. The aim of the project was to determine whether the creation of the Wellness Pathway through the health care provider and CBO education and engagement is feasible and to identify challenges in scalability. During the 6 month study period, 25 patients were referred through the Wellness Pathway, and six patients acted on the referral. Stakeholders at both organizations were interviewed to obtain detailed information regarding their experiences with, and perceptions of, the Wellness Pathway. Patients and providers who participated in the pilot believe that it has the potential to positively impact the health of older patients and is worth continuing. Moreover, conflicting data exists regarding the prevalence of vitamin B12 deficiency among elderly diabetics. The aim of this study was to investigate the link between vitamin B12 levels, type 2 DM, and BMD in elderly patients.</p></div><div><h3>Methods</h3><p>A case–control study was conducted on 61 participants, ≥60 years of age, divided into 31 cases of patients with diabetes and 30 age-matched healthy controls. Patients receiving vitamin B12 supplements were excluded. The relationship between BMD and serum levels of vitamin B12 was examined.</p></div><div><h3>Results</h3><p>Borderline/deficient serum B12 status was more common in the control group; it was found in 53.33% of the controls and 25.80% of diabetic patients. The mean serum vitamin B12 concentration was 820.65 ± 544.77 pg/mL in patients with diabetes and 677.80 ± 619.89 pg/mL in healthy control participants (<em>p</em> = 0.34). Serum vitamin B12 concentration showed no significant difference between osteoporotic patients, osteopenic patients, and normal patients among the diabetic group.</p></div><div><h3>Conclusion</h3><p>The prevalence of vitamin B12 deficiency was higher in the control group than the diabetic group who did not receive oral B12 supplementation. Low serum vitamin B12 is commonly overlooked in the elderly, with or without diabetes. The presence of diabetes mellitus did not affect BMD in the elderly. The prevalence, awareness, treatment, and control of hypertension, along with their sociodemographic and health-related correlates were established in a national cross-sectional epidemiological study of the elderly in Singapore. The associations between hypertension and select medical risks were also investigated.</p></div><div><h3>Methods</h3><p>Data was collected from 2488 randomly selected elderly (≥ 60 years) as part of the Well-being of the Singapore Elderly (WiSE) study that involved face-to-face surveys and physical examination. Sitting blood pressure (BP) was measured twice with a 1 minute interval and averaged. Hypertension was defined by a systolic blood pressure (SBP) ≥ 140 mmHg, and/or a diastolic blood pressure (DBP) ≥ 90 mmHg, and/or current treatment with anti-hypertensive medication.</p></div><div><h3>Results</h3><p>The prevalence of hypertension in this population was 74.1%. The rate of hypertension was associated with older age, lower education, and higher body mass index (BMI). Those who were Malay, retired and very physically active were more likely to be unaware of being hypertensive. An age of ≥85 years, being unemployed, widowed or divorced/separated, and being a non-smoker were associated with uncontrolled hypertension. Those with hypertension were also more likely to report a diagnosis of diabetes.</p></div><div><h3>Conclusion</h3><p>The proportion of elderly with hypertension is high in this Asian population. What is of concern is the substantial proportion of undetected cases. This highlights the need for effective health screening among the elderly. His hand and finger movements were slow and clumsy. He had an isolated optic ataxia, a component of Balint's syndrome. The computed tomography scan showed enlargement of the posterior horns of the lateral ventricles. He had recurrent falls probably owing to visual attentional deficits, which may be present in patients with posterior cortical atrophy. The findings can be deemed to fall within the posterior cortical atrophy spectrum.
