Concern over the quality of life (QL) of survivors thus becomes increasingly important. This study attempts to define the concept of QL in a small sample of BMT survivors, 1–3 years after transplant, and to identify the factors influencing the quality of survival.</p><p>Interpretative phenomenological analysis of in-depth interviews indicated that survivors, despite lingering side-effects, viewed their QL as the same or better than before BMT, thus challenging the long-standing impression that they experience a less than optimal QL. However, whilst many similarities between survivors have been identified, and the global impression was that they were ‘doing well’, this must not overshadow the fact that one survivor described a life of ‘misery … disability … aloneness … and steady decline’. It is evident from the research literature that health care professionals still avoid discussing sexual issues with patients and their partners (<span>Jenkins 1988</span>, <span>Gamel et al 1993</span>). To identify the educational needs of health care professionals, a multi-disciplinary questionnaire was developed to assess the understanding and requirements within two health-care trusts. From the results, a pilot sexuality study day was developed which was based on the needs identified. The day included education about sexuality, the impact of cancer and its treatments, how to assess and discuss sexual concerns and the effects of crises on relationships. It also allowed open discussion about difficult or embarrassing situations, communication strategies and appropriate referral services. Evaluation of the day was positive with regard to the knowledge gained and implications for clinical practice. This system has been named shared care. A philosophy of shared care has been identified whereby the concept of holistic care is promoted across health-care boundaries, moving away from the traditional medical model of care where the patient is delivered to the care system, to a holistic one, where the system is delivered to the patient. This paper discusses the development of such a practice within a regional paediatric oncology centre where 36 DGHs currently purchase oncology services. It focuses on the changes in practice and attitudes that were required by all the staff at the regional oncology centre and the dissemination of specialist skills and knowledge to the health-care teams participating in shared care. The study involved interviewing 5 parents and 5 nurses and the use of questionnaires to obtain further information from 3 parents and 7 nurses.</p><p>The main themes emerging from the data are: (a) parents have a need to stay with their children, (b) negotiation of care between parents and nurses is a transparent process, and (c) the parents reported experiencing an overwhelming boredom on the unit and found their exclusion from the kitchen facilities frustrating and a threat to their responsibility as a parent. Implications of the study take the form of proposed changes in current practice and recommendations are made for further study.</p><p>This study was the focus of a self-development module at Masters level, rather than a dissertation. However, what the study lacked in research rigour was compensated for by the immense learning which took place for myself and colleagues on the paediatric oncology unit.
