A cancer diagnosis during the years of adolescence poses particular challenges if developmental tasks are to be completed in addition to coping with diagnosis and treatment. For this exploratory pilot study, a card-sort instrument was developed to elicit the information priorities of teenagers with cancer and the beliefs of the professionals involved in their care. The teenagers' priorities were information about their diagnosis, prognosis and treatment. This was accurately perceived by the professionals. The professionals overestimated the importance of information regarding the impact of illness upon the teenagers' lifestyle, particularly concern for their appearance, and underrated their need for information about the future. The card-sort game developed for this study was positively evaluated, and enabled teenagers to consider their priorities and make their needs known. There is an important role for the nurse in maintaining good oral hygiene, either directly by providing oral care or indirectly by providing advice and opportunities to provide self-care (<span>Torrance 1990</span>), thus contributing to the child's overall comfort.</p><p>The nurse has a pivotal role in deciding which form of oral care to provide, this includes which implement or cleansing agent to introduce and the frequency with which it is used. Performing indiscriminate oral care not only causes physiological problems but it may also have psychological ramifications for the child. Consequently, oral care procedures must be based on sound scientific evidence, and not on tradition, anecdote or subjective evaluation. This is a cancer of increasing incidence worldwide, with over 92 000 cases reported in 1985, representing 1.2% of the total cancer burden (<span>Boyle et al 1995</span>). Although there have been advances in treatment with innovative approaches such as immunotherapy and gene therapy, metastatic melanoma remains largely incurable and accounts for 5000 deaths every year (<span>Jensen et al 1990</span>). On rare occasions, our duty to act in the child's interests may conflict with accepting parents' decisions. The ideology of Family Centred Care makes such disagreement with parents very problematic for professionals and, it is suggested, can lead to an abdication of the professional's responsibility towards the child. In this paper, a fictitious case study is used to examine the consequences of a disequilibrium in these factors and the effect that this has on nurse/parent relationships. In this case, the mother reversed the balance of power, empowered by 10 years exposure to health-care providers and her confidence in expressing her needs. However, it was her anger and aggression in expressing herself that resulted in conflict with the staff and prevented negotiation and partnership with nurses. Such conflict is still uncommon in paediatric BMT and thus, for many staff, a cycle of alienation, conflict, withdrawal and poor communication evolved.
