These strategies are: denial, normalizing and togetherness. Carers are not fixed in one particular behaviour pattern but can move within and between strategies. Denial is the most destructive of the strategies because, although in the short term it enables carers to cope with the situation, eventually failure to face reality results in increased stress and can be a cause of estrangement in families. Normalizing the situation by attempting to live life in as ordinary a way as possible is a valuable means of coping because it makes the prospect of dying and death seem less threatening. However, the most positive coping strategy is seen when couples work through the situation together. Most people did not achieve such complete togetherness but the case of a man and woman who did is cited and the reasons why they were able to form a partnership to deal with her dying are discussed. The main findings arising from the literature are that these groups have poorer experiences and care in the health-care system than the white community. This has resulted in under-use of services by minority groups in an attempt to avoid inappropriate and insensitive care, as well as institutional racism. In analysing these problems closely, it appears that difficulties arise essentially from communication failures rather than through deliberate neglect or malevolence. This paper focuses on culturally-based variables that are known to impair the communication process. A primary objective was to document the incidence and severity of side-effects at each cycle of chemotherapy treatment.</p><p>Thirteen patients participated. Data collection was via a self-report questionnaire listing 61 possible side-effects. Participants completed a questionnaire following each cycle of chemotherapy. Participants graded the severity of each side-effect using an ordinal scale of 0–4. A score for each side-effect was calculated, based on the product of incidence and severity values (‘C score’). Patients also stated which had been the worst side-effect at each cycle.</p><p>The response rate was 70%. Seventy-one side-effects were reported by the sample. Hair loss was the most common side-effect (mean incidence = 98%). However, patients ranked nausea as the worst side-effect overall. The increasing incidence of the disease and the little change there has been in survival rates over the past 50 years, despite progress with treatment options, makes breast cancer an important health problem for women (<span>Nettles-Carlson 1989</span>, <span>Cancer Research Campaign 1991</span>, <span>American Cancer Society 1991</span>). According to <span>Blamey et al (1994)</span>:</p><p>Lack of knowledge of the pathogenesis of breast cancer means that primary prevention is currently a distant prospect. Screening represents an alternative approach to reduce mortality from breast cancer.</p><p>The <span>Department of Health (1992)</span> has targeted a reduction in breast cancer related mortality as one of its major targets. It predicts that there will be 1 250 less deaths per annum as a result of the National Breast Screening Programme in the UK. However, this programme is only offered to women aged between 50 and 64. Although increase in any knowledge or behaviour change is notoriously difficult to measure (<span>Chavasse 1994</span>), the importance of evaluation of such programmes is widely recognized (<span>Chambers 1988</span>, <span>Ferguson 1994</span>). An assessment of post-basic educational activities in the area of training in oncology for nurses was funded by Europe Against Cancer in 1996. As part of this project, a number of interviews took place with educational course organizers and participants from different European countries. In addition to information regarding the value of educational courses established, more general issues arose as a result of the semi-structured interviews reflecting some of the wider concerns of nurses involved in cancer care.
