Patients were classified as responsive to PPI (PPIREoE) or non-responsive to PPI (PPINREoE) according to response totreatment (&lt;15 eos/hpf) at second endoscopy after 8 weeks.</p></div><div><h3>Results</h3><p>Of the 231 patients (72.3% male), 64 (27.7%) were responsive to the proton pump inhibitors. Edema (77.3% <em>vs</em>. 62.5%, p<!--> <!-->=<!--> <!-->0.031) and vertical lines (69.5% <em>vs</em>. 51.6%, p<!--> <!-->=<!--> <!-->0.014) were more frequent in PPINREoE patients. An eosinophil count in the mid‐esophagus ≥ 35 eos/HPF (25.1% <em>vs</em>. 12.5%) was more frequent in these patients (p<!--> <!-->=<!--> <!-->0.001). Those with eosinophil count &lt;<!--> <!-->15 eos/HPF in the mid‐esophagus at the first endoscopy were more likely to respond to treatment with proton pump inhibitors compared to patients with 15‐34 eos/HPF (p<!--> <!-->=<!--> <!-->0.004, OR: 3.26, 95% CI: 1.46‐7.24) and to patients with ≥ 35 eos/HPF (p<!--> <!-->=<!--> <!-->0.006, OR: 3.20, 95% CI: 1.39‐7.41).</p></div><div><h3>Conclusion</h3><p>Edema and vertical lines at the endoscopy and a higher eosinophil count in the mid‐esophagus were more frequent in patients who were non‐responsive to proton pump inhibitors. Cases (<em>n</em> <!-->=<!--> <!-->40) were considered as the children with leprosy registered at the National Information System of Notifiable Diseases in 2014 and 2015, who were intradomiciliary contacts of at least one adult diagnosed with the disease in the family, and as a control group (<em>n</em> <!-->=<!--> <!-->164) of children living within a radius of up to 100<!--> <!-->m of the notified cases. Data were obtained through medical file analysis, interviews, and blood samples for anti‐PGL‐I serological test by the ELISA method. Mello","doi":"10.1016/j.jpedp.2019.05.026","DOIUrl":"https://doi.org/10.1016/j.jpedp.2019.05.026","url":null,"abstract":"<div><h3>Objective</h3><p>To evaluate the impact of a nutritional intervention on nutritional status, ultra‐processed food consumption, and quality of life of adolescents with excess weight.</p></div><div><h3>Methods</h3><p>Experimental study with adolescent students with excess weight, divided into two groups, an intervention group and a control group. The nutritional intervention lasted six months, with an educational and motivational approach. Nutritional status (waist circumference and body mass index), quality of life, and ultra‐processed food consumption were evaluated before and after the period.</p></div><div><h3>Results</h3><p>Sixty‐two adolescents with overweight or obesity participated in the study, 37 in the intervention group and 25 in the control group, aged 13.2<!--> <!-->±<!--> <!-->1.5 years in intervention group, and 13.0<!--> <!-->±<!--> <!-->1.8 years in control group, both of which had a higher female participation. There were changes in the body mass index (intervention group Δ: −0.81<!--> <!-->±<!--> <!-->2.28, control group Δ: −0.64<!--> <!-->±<!--> <!-->1.28) and in the waist circumference for intervention group (Δ:−3.31<!--> <!-->±<!--> <!-->5.47). For the pre‐ and post‐ultra‐processed food consumption, there was a significant reduction in the consumption of soft drinks in the intervention group (Δ: −0.07 [−0.27 to 0.00]), instant noodles (Δ: −0.03 [−0.07 to 0.00]), and sandwich cookies (Δ: −0.06 [−0.26 to 0.00]). The case group consisted of 378 obese individuals and the control group of 378 non‐obese individuals. Obesity was measured by percentage of body fat using the air displacement plethysmography technique. The study variables included data on socioeconomics, demographics, lifestyle, physical activity, waist circumference, waist‐to‐height ratio, and body mass index. To identify the rs9939609 polymorphism of the <em>FTO</em> gene, blood samples were obtained for genomic DNA extraction by the real‐time PCR (Polymerase Chain Reaction) technique. Categorical variables were compared between the groups by the chi‐squared test. The normality of the anthropometric measurements body mass index, waist circumference, waist‐to‐height ratio, and percentage of body fat was evaluated by the Shapiro‐Wilk test. Comparison of the anthropometric measurements, stratified by the polymorphism genotypes, was performed by the Kruskall‐Wallis test. The Hardy‐Weinberg equilibrium was calculated. The significance level was set at 5%.</p></div><div><h3>Results</h3><p>The variables gender, age, and physical activity showed significant differences between the groups (p &lt; 0.001). The samples of obese and non‐obese adolescents were in Hardy‐Weinberg equilibrium (p = 0.0515). There was no significant difference between the genotypic (p = 0.719) and allelic frequencies (p = 0.812) regarding the case and control groups. {"title":"Epidemiology of pediatric cardiopulmonary resuscitation","authors":"Tania Miyuki Shimoda‐Sakano ,&nbsp;Cláudio Schvartsman ,&nbsp;Amélia Gorete Reis","doi":"10.1016/j.jpedp.2019.08.004","DOIUrl":"https://doi.org/10.1016/j.jpedp.2019.08.004","url":null,"abstract":"<div><h3>Objective</h3><p>To analyze the main epidemiological aspects of prehospital and hospital pediatric cardiopulmonary resuscitation and the impact of scientific evidence on survival.</p></div><div><h3>Source of data</h3><p>This was a narrative review of the literature published at PubMed/MEDLINE until January 2019 including original and review articles, systematic reviews, meta‐analyses, annals of congresses, and manual search of selected articles.</p></div><div><h3>Synthesis of data</h3><p>The prehospital and hospital settings have different characteristics and prognoses. Pediatric prehospital cardiopulmonary arrest has a three‐fold lower survival rate than cardiopulmonary arrest in the hospital setting, occurring mostly at home and in children under 1 year. Higher survival appears to be associated with age progression, shockable rhythm, emergency medical care, use of automatic external defibrillator, high‐quality early life support, telephone dispatcher‐assisted cardiopulmonary resuscitation, and is strongly associated with witnessed cardiopulmonary arrest. In the hospital setting, a higher incidence was observed in children under 1 year of age, and mortality increased with age. Higher survival was observed with shorter cardiopulmonary resuscitation duration, occurrence on weekdays and during daytime, initial shockable rhythm, and previous monitoring. Despite the poor prognosis of pediatric cardiopulmonary resuscitation, an increase in survival has been observed in recent years, with good neurological prognosis in the hospital setting.</p></div><div><h3>Conclusions</h3><p>A great progress in the science of pediatric cardiopulmonary resuscitation has been observed, especially in developed countries. Oliveira ,&nbsp;Carlos Henrique M. da Silva","doi":"10.1016/j.jpedp.2019.05.004","DOIUrl":"https://doi.org/10.1016/j.jpedp.2019.05.004","url":null,"abstract":"<div><h3>Objective</h3><p>To cross‐culturally adapt and validate the universal Portuguese version of the Pediatric Functional Assessment of Chronic Illness Therapy – Fatigue (pedsFACIT‐F).</p></div><div><h3>Method</h3><p>The universal Portuguese version of the pedsFACIT‐F was cross‐culturally adapted and validated in 323 children and adolescents aged 8–18 years, 173 healthy individuals, and 150 with chronic diseases (cancer, juvenile idiopathic arthritis, and diabetes). Reliability (internal consistency and test–retest reliability) was assessed. Item response theory model assumptions were evaluated using confirmatory and exploratory factor analyses. Items were calibrated using a graded response model. Differential item functioning was assessed regarding age, gender, and clinical condition (healthy <em>vs.</em> chronic diseases).</p></div><div><h3>Results</h3><p>No major cultural adaptations were needed. Internal consistency (Cronbach's alpha<!--> <!-->=<!--> <!-->0.84) and test–retest reliability (intraclass correlation coefficient<!--> <!-->=<!--> <!-->0.92) were good. CFA (CFI<!--> <!-->=<!--> <!-->0.92, TLI<!--> <!-->=<!--> <!-->0.90, RMSEA<!--> <!-->=<!--> <!-->0.097) and CFE analysis confirmed sufficient unidimensionality. The data also fit the GRM and demonstrated good coverage of the fatigue construct (threshold parameters range: −1.42 to 4.56). No items demonstrated significant differential item functioning.</p></div><div><h3>Conclusion</h3><p>The universal Portuguese version of the pedsFACIT‐F provides a reliable, precise, and valid measure after being assessed by robust psychometric properties. {"title":"Prevalence and impact of asthma in schoolchildren in the city of Caxias do Sul‐RS","authors":"Daniel Zacaron ,&nbsp;Cristian Roncada ,&nbsp;Rossano Sartori Dal Molin ,&nbsp;Marcus Herbert Jones ,&nbsp;Paulo Condessa Pitrez","doi":"10.1016/j.jpedp.2019.04.002","DOIUrl":"https://doi.org/10.1016/j.jpedp.2019.04.002","url":null,"abstract":"<div><h3>Objective</h3><p>To analyze the prevalence and impact of asthma in schoolchildren from the city of Caxias do Sul, RS, Brazil.</p></div><div><h3>Methods</h3><p>Cross‐sectional observational and case‐control study with children and adolescents between 7 and 15 years old, from public schools in Caxias do Sul/RS. The study is composed of two phases: Phase I analyzed the prevalence of asthma in the delimited population investigating 1915 schoolchildren; Phase II quality of life questionnaires, asthma control and classification (for the asthmatic group), physical activity, school performance, pulmonary function tests and anthropometric measures were applied to 266 asthmatics and 288 controls.</p></div><div><h3>Results</h3><p>The estimated prevalence of asthma was 16.1%. In the comparison between asthmatics and nonasthmatics premature birth (p &lt;<!--> <!-->0.001) and diagnosis of another chronic disease at birth (p &lt;<!--> <!-->0.001) were found. Regarding pulmonary function, significant differences were found in the values between groups in FEV<sub>1</sub>, FEV<sub>1</sub>/FVC and forced expiratory flow in the 25 and 75% moments (FEF<sub>25‐75%</sub>), being that asthmatics presented lower values. Among asthmatics, 133 (50.8%) did not have the disease controlled. In the anthropometric variables, significant differences were observed, with higher values in controls, in the waist‐to‐height ratio (p<!--> <!-->=<!--> <!-->0.009) and in the perception of health (p &lt;<!--> <!-->0.001). Quality of life is lower in asthmatics in the physical well‐being domain (p<!--> <!-->=<!--> <!-->0.001) and in the total score (p<!--> <!-->=<!--> <!-->0.016). The obese groups (13.2<!--> <!-->±<!--> <!-->1.4 years, 40.5%<!--> <!-->±<!--> <!-->5.8% fat by DXA) differed in their aerobic fitness (<span><math><mrow><mover><mi>V</mi><mo>˙</mo></mover><msub><mi>O</mi><mrow><mn>2</mn><mi>p</mi><mi>e</mi><mi>a</mi><mi>k</mi></mrow></msub></mrow></math></span> 76.0<!--> <!-->±<!--> <!-->8.1 vs. 56.6<!--> <!-->±<!--> <!-->5.8<!--> <!-->mL.kg<!--> <!-->muscle<!--> <!-->mass<sup>‐1</sup>.min<sup>−1</sup>), as well as the lean groups (13.1<!--> <!-->±<!--> <!-->1.6 years, 24.0%<!--> <!-->±<!--> <!-->4.8% fat) (<span><math><mrow><mover><mi>V</mi><mo>˙</mo></mover><msub><mi>O</mi><mrow><mn>2</mn><mi>p</mi><mi>e</mi><mi>a</mi><mi>k</mi></mrow></msub></mrow></math></span> 74.5<!--> <!-->±<!--> <!-->2.9 vs. 56.2<!--> <!-->±<!--> <!-->5.0<!--> <!-->mL.kg<!--> <!-->muscle mass<sup>−1</sup> <!-->min<sup>−1</sup>). Girls cycled two bouts of 25<!--> <!-->min with a 10<!--> <!-->min rest in between, at ∼5.4<!--> <!-->W.kg<sup>−1</sup> in the heat (36<!--> <!-->°C and 40% relative humidity) and they were kept euhydrated. Rectal and skin temperatures and heart rate were measured every 5<!--> <!-->min. Perceptual responses were evaluated throughout the exercise.</p></div><div><h3>Results</h3><p>Initial rectal temperature was higher in the obese subjects compared to the lean subjects (37.5<!--> <!-->±<!--> <!-->0.3 and 37.2<!--> <!-->±<!--> <!-->0.3<!--> <!-->°C). No difference was observed among the girls whom were obese (eight fit or unfit) and lean (also fit or unfit) throughout the exercise in rectal temperature (37.6<!--> <!-->±<!--> <!-->0.2, 37.5<!--> <!-->±<!--> <!-->0.3, 37.5<!--> <!-->±<!--> <!-->0.3, 37.4<!--> <!-->±<!--> <!-->0.3<!--> <!-->°C, respectively), skin temperature (34.8<!--> <!-->±<!--> <!-->0.8, 35.1<!--> <!-->±<!--> <!-->1.0, 34.4<!--> <!-->±<!--> <!-->0.9, 35.2<!--> <!-->±<!--> <!-->0.9<!--> <!-->°C), and heart rate (128<!--> <!-->±<!--> <!-->18; 118<!--> <!-->±<!--> <!-->12, 130<!--> <!-->±<!--> <!-->16, 119<!--> <!-->±<!--> <!-->16<!--> <!-->beats<!--> <!-->min<sup>−1</sup>). This study aimed to investigate the effect of <em>MBL2</em> gene polymorphism on short‐term outcomes in preterm infants.</p></div><div><h3>Method</h3><p>Infants of &lt;37 gestational weeks who were admitted to the neonatal intensive care unit during a two‐year period were enrolled in this prospective study. The neonates were categorized into two groups according to their <em>MBL2</em> genotypes. Normal <em>MBL2</em> genotype was defined as <em>MBL2</em> wild‐type (AA genotype), whereas mutant <em>MBL2</em> genotype was defined as <em>MBL2</em> variant‐type (AO/OO genotype). The relationship between <em>MBL2</em> genotype and short‐term morbidity and mortality was evaluated.</p></div><div><h3>Results</h3><p>During the two‐year study period, 116 preterm infants were enrolled in this study. In <em>MBL2</em> variant‐type, mannose‐binding lectin levels were significantly lower and incidences of mannose‐binding lectin deficiency (MBL level<!--> <!-->&lt;<!--> <!-->700<!--> <!-->ng/mL) were higher (<em>p</em> <!-->&lt;<!--> <!-->0.001). In this group, the prevalence of respiratory distress syndrome and mortality was significantly higher (<em>p</em> <!-->&lt;<!--> <!-->0.001, <em>p</em> <!-->=<!--> <!-->0.03 respectively). In the <em>MBL2</em> wild‐type group, the prevalence of necrotizing enterocolitis (NEC) was higher (<em>p</em> <!-->=<!--> <!-->0.01). Logistic regression analyses revealed that <em>MBL2</em> variant‐type had a significant effect on respiratory distress syndrome development (odds ratio, 5.1; 95% confidence interval, 2.2–11.9; <em>p</em> <!-->&lt;<!--> <!-->0.001).</p></div><div><h3>Conclusions</h3><p><em>MBL2</em> variant‐type and mannose‐binding lectin deficiency are important risk factors for respiratory distress syndrome development in preterm infants. Additionally, there is an association between <em>MBL2</em> wild‐type and NEC.
