The Implant is no more ankylosed in order to support occlusal forces, but integrated under the effect of these forces. This new therapeutic reasoning can appear disconcerting at first, and a good comprehension of the various types of early or late complications and their biomechanical and/or infectious origin is necessary before considering an enlightened reintervention. Finally, the management of therapeutic complications and failures, much more than the success of a treatment, highlights the theoretical and clinical competence level of a practitioner. The objective of this last article is thus to establish a retrospective study on all complete maxillary and/or mandibular rehabilitation with set sat back maxillofacial implants carried out in Milan Private Clinic (Paris) from 1994 to the early 2004, excluding from the field of this work all partial dental rehabilitations. Our results are classified in 4 distinct tables in order to determine the general statistics of our exercise, the influence of the occlusal and functional context and the rehabilitation type (strictly implantar or mixt dental-implantar) on the development of early or late complications, to evaluate their nature and frequency and to highlight the role of smoking and bruxism on their development. However, penumbra and diffusion, two X-ray use indissociable physical phenomena, can largely harm the precision of these radiological examinations. The objective of this article is to present these two phenomena and their consequences in oral implantology. On a toothless maxillary, their association will reveal the interfaces between bone structures and air cavities to the detriment of residual bone volume. On a facial cranium after large exeresis surgery, the increased X-ray diffusion within the cicatricial fibrous masses will induce an overvaluation of the residual bone structures. Every hand injury may lead to exposure to bloodborne pathogens. The G-VIR<sup>®</sup> glove is an innovative device against exposure risk. Its unique active barrier design allows to expel disinfectant on the sharp when it breaches, the viral load transmitted through the glove is therefore reduced. Furthermore this glove does not hinder the dexterity of the surgeon (as shown by a clinical study performed at Biomatech and Lyon Sud Hospital). This glove has to be considered as the standard for some regions of the world, or in some circumstances when performing mass surgery and conversely in well focused cases as human immunodeficiency virus or hepatitis C virus positive patient in our countries and hospitals. Its cost is low when compared to the infection risk and in order to prevent occupational infection it should be used each time it is considered as beneficial. Dohan","doi":"10.1016/j.implan.2004.07.002","DOIUrl":"10.1016/j.implan.2004.07.002","url":null,"abstract":"<div><p>The PRF (Platelet Rich Fibrin) belongs to a new generation of platelet concentrates, with simplified modes of production and without biochemical handling of blood (heparin, EDTA, bovine thrombin, Calcium chloride…). In this fourth article, we sought to correlate the previously evaluated biology of the PRF with first established clinical results, in order to determine the potential fields of application of this biomaterial. Our reasoning is structured around four fundamental events of the cicatrisation: angiogenesis, immune control, circulating stem cells trapping and wound covering epithelialisation. Indeed, all the known clinical applications of the PRF highlight an accelerated tissue cicatrisation because of the development of an effective neovascularization, an accelerated wound closing with fast cicatricial tissue remodelling and the quasi-total absence of infectious events. Our present work is intended to disclose a protocol supposed to perform such a task. Every dentist who follows closely such a protocol will be able to perform a very secure dental surgery with no risk for the patient. The most practical way to handle such a technique is to own two dentist’s chairs and to conduct the operating process with two personal assistants. The ward must be devoted to dental surgery only and the input should be protected by an airlock. Otherwise a dental surgery operation could be conducted under very secure patient's conditions even if the dentist could use only one dentist’s chair and be helped by only one personal assistant. To work under such restrictive conditions it is important to arrange very carefully the ward and to cleanse it right after the dental operation has been performed. It uses microvibrations of intermediate frequency generated by a piezoelectric transducer and applied to titanium nitride hardened and/or diamond-coated inserts. With its vast range of inserts, it finds many applications in oral and maxillofacial surgery, such as non-traumatic dental avulsions, root surfacing and bone defect debridement, or cyst removal. It also proposes a simplified protocol for the sinus lift surgery. Lastly, it offers a true revolution in the bone grafting surgery by allowing precise and non-traumatic graft harvesting. This article presents its physical, technological and clinical aspects and discusses its most promising applications.
