Ihde (Professeur associé)","doi":"10.1016/j.implan.2005.05.005","DOIUrl":"10.1016/j.implan.2005.05.005","url":null,"abstract":"<div><p>L'usage des implants latéraux permet aujourd'hui de doter presque tous les patients de dentiers fixes à supports endo-osseux sans nécessiter d'augmentation des crêtes [1]. Dans la plupart des cas, les restaurations peuvent être assorties de protocoles de traitement permettant la mise en charge immédiate des implants insérés. Les patients présentant une réduction importante de l'os maxillaire et ceux suspectés d'exercer des forces masticatoires élevées posent toutefois des problèmes particuliers à l'implantologue et au chirurgien–dentiste. Il est particulièrement important de contrôler de manière sûre les forces exercées sur l'interface entre l'os et l'implant lorsque la réserve osseuse est insuffisante, les dimensions verticales et sagittales défavorables ou les muscles masticateurs très actifs. L'administration à titre prophylactique de toxine botulique peu de temps avant l'implantation facilite la réduction de la force du masséter et du temporal après l'implantation en abolissant les mouvements volontaires ou involontaires du système masticatoire agissant sur l'interface os–implant et susceptibles d'empêcher l'intégration des implants. Ce premier article décrit certains aspects de la physiologie osseuse en vue de justifier l'utilisation de la toxine botulique dans la mise en charge immédiate en implantologie et l'illustre par deux cas cliniques. This technique, which seems less invasive at first sight, meets a large success and is a real option with classic maxillary sinus floor bone grafting through lateral antrostomy. To reduce the failure rate due to mucous membrane perforation during osteotomy, authors have imagined stop sinus osteotomes. The Easy Abutment<sup>®</sup> system associates an abutment, a screw, an impression cap, a plug filing and a healing cap. There's no temporary prosthesis, neither temporary abutment.
