Despite a deficit of research concerning the relationships between tobacco and implantology, it remains possible to lean on the numerous studies achieved in the periodontics field to formulate pathogenic hypotheses that permit to explain the frequent implant failures observed in smoking patient. Among these, we may mention nicotinic affinity for some oral tissues, post-surgical healing delay or local inflammatory aftermaths of previous acute periodontal history. An innovating security system is proposed here to counter this problem. The DRAVINO-1<sup>®</sup> system was developed to limit nosocomial infections of endogenous or exogenous origin when using surgical aspiration tubing and single-use drape sheets in buccal surgery. The drape sheet and the tubing are installed simultaneously (in less than 1 minute), without coming into contact with the non-sterile environment. The tubing that passes through the drape sheet is industrially pre-sealed to the drape (Industrial Quality Control for sealing according to the European Standard EN 13 795-1) at a length sufficient to allow for vacuuming all around the operative fenestration. During the act of unfolding the drape over the patient, the non-sterile part of the tubing, which is found both under and outside of the drape and which linked to the vacuum socket, is thus automatically isolated by the drape. Il peut toutefois arriver que des implants initialement bien intégrés présentent une mobilité inattendue lors du fonctionnement effectif du système os–implant–restauration. De nombreux développements peuvent être impliqués ici et peuvent tous aboutir, isolément ou en combinaison, à une surcharge de l'interface entre l'os et l'implant endo-osseux [2]. Il en résulte dans un premier temps une mobilité accrue, sans que l'ostéo-intégration des implants au sens strict soit immédiatement compromise. Dans certains cas, la toxine botulique peut être employée à titre thérapeutique pour favoriser la stabilisation de l'interface entre l'os et les implants et la reminéralisation. In the field of dentistry and orthopedics, the biomechanical requirements for the implants have lead to an extensive use of biocompatible alloys. However, in a solid form these alloys have always exhibited inadequate fixative properties in the surrounding bone. Recent technologies allow the manufacturing of highly porous metallic open cell structures, biologically and biomechanically compliant. Two of these porous metals appeared particularly interesting for the design of novel dental implants: a porous titanium-nickel alloy (NiTi) and a porous tantalum structure. The purpose of the present study was to assess in vitro the invasion level of these porous materials by human osteoblasts, by the use of a three-dimensional cell culture system. Bone cell growth was measured by the MTT test, whereas cell infiltration and morphology throughout the porous scaffolds were examined by confocal microscopy. Our results indicate that porous NiTi and porous tantalum behave as suitable substrates for the attachment and ingrowth of human osteoblasts. Furthermore, the tantalum porous structure appeared more favorable to bone cell morphology and infiltration.
