Anatomic limits of the floor of the mouth are described, followed by the description of the most common diseases affecting this anatomic region, namely: diseases of the mucosa; diseases of the salivary glands; benign tumours; malignant tumours; malformative diseases; infectious diseases; traumatology. The high diversity of the lesions that may be encountered in the mouth reflects its semiological importance. Specific diseases may be located in the velopalatal zone, while other diseases are specific of the oral cavity, and others are only signs of a general disease. The aesthetic access route reduces dramatically all these drawbacks. Despite its proven safety and its relevance in the cosmetic field, the aesthetic access route (SMAS flap and lifting incision) is not a routine procedure for many surgeons. The current paper describes this access route. Tricks are pointed out. Oral candidoses have, most of the time, a benign course and are well controlled by local antimycotic treatments. However, in local or general immunosuppressive contexts, the infection may evolve and become extensive, chronic, and some of the times systemic. Some exotic mycoses may develop on oral mucous membranes in some situations of immunosuppression; some others are imported mycoses that have to be recognized. In any case, ascertained diagnosis is based on evidence of the germ and its identification. The treatment is most of the time pharmacological; it is then specifically adapted to the type of oral mycosis. A first biopsy decision is a mistake because of the dissemination risk and the induced swelling reaction which could interfere with the future ablation and the definitive histological analysis. Of course, a reliable histological result takes advantage of a macroscopic, microscopic and ultrastructural analysis on the complete surgical piece. Also, the discovery of such a tumour will lead the clinician and the surgeon to a rigorous diagnosis estimation, taking into account both clinical and radiographic argues: the patient age, the lesion localisation, the evolution, the relation of the tumour with the teeth and cortical bone. This estimation will allow the surgeon to decide the best surgical preserving treatment before receiving the definitive histological result. In most of the cases, a benign tumour, even though voluminous, should be surgically removed taking care of the strong mucous and periosseous membrane. It is a prismatic triangle of which inferior-lateral wall constitutes the surgical approach. The mandible constitutes its superior-lateral wall and the medial muscular wall features neurovascular elements. Anterior and posterior walls correspond to both poles of the submandibular gland. The submandibular fossa includes essentially the submandibular gland and neurovascular elements (facial vessels, lingual veins, lingual and hypoglossus nerves, lingual artery).
