The therapeutic calendars, as well as the surgical techniques used, vary from an operational centre to another, just like the resort to presurgical orthopaedic treatment. Today, palatal plates are recommended by some authors and disparaged by others. Several kinds of plates exist: fixed or removable, active or passive, combined or not to an extraoral strapping. After reminding the principal cleft lip and palate classifications, we describe the different kinds of palatal plates and their realisation techniques. Preoperative orthopaedic treatment is of course much debated, but short-, mean- and long-term expected beneficial effects are not the same for all teams. It remains difficult to make allowance between the combined action of surgery, orthopaedic treatment, the extent of dysfunctions, the initial polymorphism of cleft, the growth potentialities different from an individual to another. Usually, it refers to the sampling of a living tissue and preservation of its morphology by immediate fixation for future histological analysis. In any case, the clinician has to synthesize the anatomopathological results and clinical observations, in order to establish the most adapted therapeutic strategy. These last years, several complementary techniques have been developed in addition to standard biopsy: extemporaneous examination during surgery for the verification of the integrity of the tumour peripheral resection limits, or for the assessment of potential ganglionic metastasis; cytologic methods for fluid puncture or onlay smears performed on a mucous lesion or a ganglion section. Investigation techniques such as electronic microscopy, immunohistochemistry, and now molecular biology, may be used for biopsy samplings or cytology products. Most of them necessitate specific conservation conditions (freezing for some immunohistochemical reactions and for molecular biology, specific fixation agents for electronic microscopy). The corticosteroid agents which are steroidal anti-inflammatory drugs have all a hormonal activity, concerning principally the metabolic regulations, and they cause pituitary-adrenal suppression. Some pathological states require particular attention but they are not necessarily contra-indicated for short duration prescriptions. Unerupted third molar extractions and oral dermatology are among their indications, with specific administration modalities. The non-steroidal anti-inflammatory drugs (NSAIDs) regroup various chemical classes but without any steroidal structure. The prostanoids biosynthesis inhibition by the NSAIDs is responsible for many of their pharmacological properties but also of their side-effects (essentially at the digestive, renal and cutaneous level). They can be prescribed in odontostomatology either for their analgesic properties, either for their anti-inflammatory and anti-oedematous properties, after careful analysis of the benefit/risk ratio. The new anti-inflammatory drugs (coxibs, selective inhibitors of the cyclo-oxygenase 2) have their main practical application in rheumatology, but they do not really provide notable advantages with regard to the incidence of side effects compared to conventional NSAIDs.
