Osteoradionecrosis has been first described by Regaud in 1922. Osteoradionecrosis of the jaws is most of the time developed on mandibula; maxillary localisation is rare. Osteoradionecrosis of the jaws is a priority considering that patients have recovered of their primary disease. Treating osteoradionecrosis is difficult because of the patient's health status and the poor healing capacities of the irradiated tissues. In addition, the quality and amount of vascular bed are reduced for microvascular reconstruction. Osteoradionecrosis physiopathology is not totally understood. Conservative treatment is possible at the very beginning of the disease, but as much as the osteoradionecrosis is growing and is ancient, the treatment must be radical. Concerning osteoradionecrosis, the main problem is to be able to perform radical treatment without lasting too much time with conservative treatment. Mandibular reconstruction using osteo-cutaneous free flaps is actually the best solution for concerned patients. Risks persist after the intervention, mainly haemorrhagic events and related hypovolaemia and respiratory distress, sometimes life-threatening. Peroperative haemostasis and postoperative aspiration drainage may not be discussed. The oral cancer incidence in France has been only evaluated: it appears to be one of the highest worldwide, with around 15400 new lip-oral-pharyngeal lesions registered in 2000. In our country, it's the 5<sup>th</sup> location in term of cancerous location and about the 8<sup>th</sup> cause of mortality. In over 90% of the cases, histology finds a squamous cell carcinoma. The average age of the patients is 60 years. Main risk factors are tobacco - in particular when the consumption exceeds 20 packages-years - and alcohol consumption. Women are increasingly exposed to these risk factors. These cancers appear mostly on a healthy mucosa. Squamous cell carcinomas appear as a granulated ulceration, resting on a hardened base. The functional signs are late, the pain prevails. Depending on the localization within the oral cavity, localized dental mobility, trismus, limitation of tongue mobility or a reflex ear-pain can be observed. The clinical examination combines the study of the tumour head and neck extension, the screening for a second cancer and for visceral metastases, and the evaluation of general health status. Radiological, endoscopic and biological assessments are carried out. The therapeutic management is collectively decided, during a multidisciplinary consultation: depending on the tumour size and location, the treatment is based on one or several techniques. The therapeutic step is systematically followed by a regular, clinical check-up aimed at establishing cancer continuation or a recurrence, a second cancer, or some therapeutic complications. The management of older patients induces frequent and constraining problems due to the repercussions of ageing on orofacial tissues; these repercussions consist of various intricate diseases that more or less directly interfere with the general disease. Besides, “normal senescence” may not or hardly be distinguished from a pathological state. The frailty of older patients, their uncertain participation, the constraints and risks related to their poly-medication constitute a wide range of problems in their therapeutic management. The risk of under- or over-consumption of drugs is also to be considered, particularly in cutaneous or oral cancerology. Lastly, it should be reminded that in most cases, age-related functional and aesthetic disorders are the main reason for seeking treatment.
