The objectives of the study were: (1) to compare cardiac rehabilitation in settings: (a) actively receiving guidelines and additional facilitation; (b) actively receiving guidelines; and (c) not actively receiving guidelines; (2) to determine whether there are any differences in the three settings between those centres identified as either low or high providers of cardiac rehabilitation; (3) to identify particular problems associated with the implementation of guidelines in a multiprofessional setting; and (4) to gain feedback from health care workers about the credibility and feasibility of guidelines developed for cardiac rehabilitation. Six centres were randomized into the three groups, and a variety of tools were used, including questionnaires, facilitation, audit and semi-structured interviews. Results show that the national guidelines did not appear to have been utilized, other than in brief discussions. This was required because of the concerns about using existing age-related formulae, which were inappropriate for this clinical group.</p><p>Thirty male recent post-myocardial infarction patients with beta blockade therapy (aged 60±8.9 years) and 40 male recent post-myocardial infarction patients without beta blockade therapy (aged 63.4±8.2 years) performed a graded exercise test (modified Bruce protocol) on a motorized treadmill until volitional cessation or reaching any of the American College of Sports Medicine criteria.</p><p>Ratings of perceived exertion, blood lactate, oxygen uptake, angina score, dyspnoea score and blood pressure were recorded every 3 min during the exercise. Oxygen uptake, volume of expired gas and heart rate were recorded every 30 s during the exercise and a 12-lead electrocardiograph was monitored continuously.</p><p>The results showed that the patients did not achieve the age-related maximum heart rates predicted from 220-age, showing early signs of myocardial ischaemia at about 90% of their peak heart rate. The questions at issue were how these individuals assessed their QoL in general and also with respect to sex, age, civil status and occupation. A cross-sectional study was conducted in a health care area in southern Sweden with 310000 inhabitants. A sample of pacemaker patients (<em>n</em>=182) entered into the study through a random selection of every fourth person. The results show that the total QoL for these pacemaker patients was acceptable. A poorer QoL was found with regard to physical health, the possibility of influencing one's own situation, lifestyle, health care resources and sociocultural orientation. Men, those who were cohabiting, white-collar workers, managers and those 64 years of age or younger assessed their QoL as higher. By using Orem's self-care theory, information, support and education can be carried out in such a way that nursing care affects the entire QoL in pacemaker patients.
