{"title":"Effects of SSRI antidepressant treatment upon mood status and blood pressure reactivity to demand in coronary patients","authors":"C.N. Hallas , E.W. Thornton","doi":"10.1054/chec.1999.0059","DOIUrl":"10.1054/chec.1999.0059","url":null,"abstract":"<div><p>This pilot study investigated the effect of selective serotonin re-uptake inhibitor antidepressants (SSRI) as prescribed in clinical practice upon anxiety, depression, and cardiovascular responses to challenge, in post myocardial infarction patients (<em>n</em> = 30). Primary episode patients were psychologically assessed 4 weeks following discharge from hospital and re-assessed 18 months later. Data were collated at re-assessment on reactive blood pressure (BP) responses to four laboratory demand test procedures, a 12 hour period of normal daily activity, and responses to episodes of self-reported stress during this last period. Negative affect and BP reactivity, especially to self-reported stress, were reduced in patients who were taking SSRIs compared with non-users. Design: Two hundred male patients who took part in a randomized controlled trial of community based post-infarct rehabilitation were followed-up with an annual questionnaire. Results: The questionnaire response rate fell from about 90% at year 1 to 72% at year 11. The level of exercise, the prevalence of angina pectoris and `energy' scores of the treatment group and the controls converged over several years and met between years 6 and 8. There was no significant difference between the two groups for non-fatal reinfarction nor for long-term mortality. Significant predictors of mortality included pulmonary oedema on the admission chest X-ray, the development of complications during admission, one or more previous infarcts, increasing age and low initial fitness. Mood, coping and behaviour at follow-up were measured concurrently. While in hospital, participants were asked to report their behaviour during a typical week prior to their hospitalization. Analyses reported here concentrate on the relationship between coping and mood and behaviour. After partialling out pre-morbid measures, hierarchical stepwise regression analyses showed both measures of mood and behaviour were predicted by the coping strategies reported in hospital. Anxiety was associated with measures of both problem- and emotion-focused coping. In contrast, depression was predominantly associated with emotion-focused coping. Cigarette smoking, mild exercise and alcohol consumption were each associated with coping strategies: only diet was not predicted by coping variables. Baker","doi":"10.1016/S1362-3265(99)80045-5","DOIUrl":"10.1016/S1362-3265(99)80045-5","url":null,"abstract":"<div><p>The cardiac rehabilitation unit of the North Hampshire Hospital, Basingstoke provides an education session (‘Heartlearn’) for the partners and relatives of patients in the coronary care unit with the provisional diagnosis of acute myocardial infarction. This paper reports the results of the session on the self-rated confidence levels of the session attenders, on their knowledge about coronary disease and their attitudes to learning cardiopulmonary resuscitation (CPR). Partners or relatives of 36.5% of 368 CCU patients admitted over 22 months attended a ‘Heartlearn’ session. Following the session there was a significant increase in levels of confidence for understanding the nature of a heart attack, for knowledge of causes of a heart attack and for feeling confident as a carer. Levels of knowledge about heart disease were relatively high before the session and these levels were increased after the session.
