To achieve this goal, a study was conducted and empirical information was obtained from a representative sample of 411 patients. The study used general scientific methods of analysis, synthesis, generalization, interpretation of scientific data, as well as a systematic approach.Results. As a result of the study, the attitude of patients of the cardiac surgery clinics to organ donation and transplantation was determined. It was found that at the level of patients of the cardiac surgery clinics the cultural understanding of society in relation to organ donation and transplantation has not been formed.Conclusions. The existing cultural views of the patients in Ukraine doesn’t provide positive precedents for development of organ donation based on the system of «organ testament». {"title":"Analysis of early cardiac complications after coronary artery bypass grafting under two different regimens of anesthesia","authors":"S. Maruniak, O. Loskutov, I. Malysh","doi":"10.31928/2305-3127-2022.3-4.3642","DOIUrl":"https://doi.org/10.31928/2305-3127-2022.3-4.3642","url":null,"abstract":"The aim – to analyze the effect of two different schemes of anesthesia on early cardiac complications in patients with coronary artery bypass grafting (CABG) with cardiopulmonary bypass.Materials and methods. The study included 120 patients who underwent CABG with cardiopulmonary bypass (CPB). The median surgery risk according to EuroSCORE II was 3.45 % (2.15 %; 4.05 %). According to the scheme of anesthesia, all patients were divided into two groups: the first group (60 patients) – low-opioid scheme of anesthesia; the second group (60 patients) – a standard scheme of anesthesia.Results. Patients in the first group were more than twice as likely to develop postoperative atrial fibrillation compared to the second group (9 (15.0 %) vs. 19 (31.7 %), p = 0.031). In addition, patients in the first group were 2.3 times significantly less likely to have low cardiac output syndrome (LCOS) compared to the second group (11.7 % vs. 26.7 %, p = 0.037). The duration of CPB (p = 0.032) and the level of interleukin-6 after CPB (p = 0.004) were reliable indicators for predicting LCOS. The final statistical model [F (4, N = 120) = 12.52, p < 0.001, R2 = 0.304] covers almost a third of all factors in the development of LCOS. Only the level of interleukin-6 after CPB (the final statistical model (F (4, N = 120) = 11.54, p < 0.001, R2 = 0.286) was a reliable indicator for predicting postoperative atrial fibrillation.Conclusions. Recently, the investigations cause-and-effect relationships of PTSD with the main risk factors, the course and prognosis of cardiovascular diseases has become more relevant. Many studies have shown that patients with PTSD have higher risk of developing cardiovascular diseases in general and adverse events such as myocardial infarction, stroke, and heart failure. Individual clinical factors, pathophysiological and behavioral mechanisms of these connections require further investigation. Although the prognosis of HCM is relatively favorable, the annual overall mortality is about 0.7 %. The most common complication of HCM is obstruction of the left ventricular outflow tract (LVOT). Careful diagnosis and timely identification of high-risk patients by confirming the presence and degree of obstruction of the left ventricular outflow tract may reduce morbidity and mortality due to timely and correctly selected treatment. One-third of patients with HCM have echocardiographically confirmed LVOT obstruction at rest, and another third may have LVOT obstruction with provocation. Our clinical case demonstrates the presence of a small group of patients with HCM, whose LVOT obstruction is underestimated by transthoracic echocardiography (TTE) neither at rest nor when using provocative tests (Valsalva tests, exercise), and its hemodynamic significance is revealed only with invasive hemodynamic catheterization of the heart. Below, we illustrate the utility of hemodynamic catheterization as an adjunctive tool to echocardiography to help establish the presence, magnitude, and location of obstruction in patients with HCM in the absence of such data on TTE. Nevertheless, restenosis of stented coronary arteries, which significantly worsens the course of CAD, remains a significant clinical problem of this technology. Prevention of this complication requires a modern understanding of the complex pathogenetic mechanisms of restenosis. Based on the literature data, the frequency of restenosis development in stents with medical coatings is in the range from 3 to 20 %. The mechanism of restenosis is multifactorial and includes biological, mechanical, and genetic factors. The main mechanical causes are insufficient expansion of the stent or its destruction, and biological factors include local inflammation, leading to aggressive neointimal proliferation and late neoatherosclerosis. The search for genetic factors and possibilities of influence on the development of this complication is still ongoing. The cohort single-center study analyzed clinical, laboratory and instrumental data from 115 persistent AF patients with duration of its episode ≥ 90 days, who underwent an elective direct current cardioversion. Patients were subdivided into the groups depending on SR restoration effectiveness: 59 (51.3 %) and 56 (48.7 %) patients with restored and non-restored SR, respectively.Results and discussion. The group of patients with non-effective DC cardioversion was characterized more severe heart failure (HF) and more frequent percutaneous coronary intervention (PCI) cases, as opposed to the group with restored SR. The studied groups differed significantly by the certain echo parameters, characterizing the structure and function of left heart chambers. The transthoracic echocardiography (TTE) revealed a higher left ventricular (LV) dilatation and more frequent pulmonary hypertension (PH) cases in non-restored SR group. According to transesophageal echocardiography (TEE) data, both groups demonstrated the deterioration of left atrial (LA) function, namely the decrease of LA appendage flow velocity ≤ 40 cm/s in more than 60 % of patients. Additionally, the LA spontaneous echo contrast (SEC) was visualized in vast majority of non-restored SR patients (94.6 %). At the same time, only 2 (3.6 %) patients with non-effective DC cardioversion were free from SEC, as compared to 44 (74.6 %) restored SR patients (р < 0.001). The multivariable logistic regression analysis revealed SEC as an independent predictor of SR restoration.Conclusions. Persistent AF patients with duration of its episode ≤ 90 days and non-effective elective DC cardioversion demonstrated higher stage of HF, more frequent demand for PCI, the higher LV dilatation, and presence of PH (TTE) and LA SEC (TEE). The cascade of pathological processes can eventually lead to damage of different organs and systems including cardiovascular system. However, due to polymorphism of clinical manifestations, this disease often remains undiagnosed or diagnosed at terminal multiorgan failure stage. We performed search of the data in electronic databases PubMed, SCOPUS, Embase, Google Scholar OVID, related to the use of TEVAR. Currently, there is a limited amount of large cohort size studies outlining the use of TEVAR in patients with CTD. Endovascular prosthetics of the thoracic aorta in patients with accidents is dangerous because of the progressive dilatation of the aorta and the high probability of further re-interventions in later life. The analysis showed that open reconstruction still remains a gold standard of intervention in young patients with progressive CTDs, especially in acute type of aortic dissection. The overall risk of SCD in patients with HCMP is not high, but it remain the main cause of death in young patients. Implantation of cardioverter-defibrillator, surgical treatment of HCMP allow to prevent SCD and other negative cardiovascular events (heart failure and stroke) in patients with HCMP. Thus identification of predictors of negative cardiovascular events is essential in HCMP. Imaging methods (echocardiography and magnetic resonance imaging) enable to assess risk factors: left ventricular hypertrophy, left ventricular outflow tract obstruction, left atrial dimension, apical aneurysm, midventricular obstruction and extent of the late gadolinium enhancement.
