We present the case of a 70-year-old male with a history of mitral valve replacement and prior transcatheter edge-to-edge repair (TV-TEER), who underwent a successful redo TV-TEER due to torrential TR. Post-procedural evaluation revealed a significant reduction in TR severity and clinical improvement. Unfortunately, the patient succumbed to infectious complications and sepsis in the third week of follow-up. This study aims to determine the prevalence of anxiety and depression in patients with non-valvular AF and to investigate the relationship between these psychological conditions and the treatment regimens administered.</p><p><strong>Method: </strong>This cross-sectional study included 255 individuals diagnosed with non-valvular AF who were treated between 2021 and 2022. Psychiatric evaluation was conducted using the Beck Depression Inventory and Beck Anxiety Inventory. Multivariate regression analysis was performed to identify predictors of depression and anxiety.</p><p><strong>Results: </strong>Of the patients included, 62 were on warfarin, 124 were on novel oral anticoagulants (NOACs), and 69 were not receiving any oral anticoagulant (OAC) therapy. Overall, 68.6% had depression and 64.7% had anxiety at a moderate or higher severity level. Although there was no notable variation in anxiety and depression scores between patients on NOACs and those not undergoing OAC treatment, the warfarin group had significantly higher scores than the other two groups. Age, anxiety, C-reactive protein (CRP) levels, and CHA2DS2-VASc scores (Congestive heart failure, Hypertension, Age ≥ 75 years, Diabetes mellitus, prior Stroke/transient ischemic attack-Vascular disease, Age 65-74 years, Sex category) all positively correlated with the severity of depression. Anxiety, in turn, was positively associated with age, depression, and CHA2DS2-VASc scores, and negatively associated with ejection fraction. Regression analysis revealed a strong correlation between warfarin treatment and anxiety severity.</p><p><strong>Conclusion: </strong>The findings of this study suggest that warfarin treatment is associated with significant psychological effects in patients with AF. {"title":"The Outcomes of Concomitant Carotid Endarterectomy and Off-Pump Coronary Artery Bypass Grafting.","authors":"Mehmet Şanser Ateş, Eray Aksoy, Zümrüt Tuba Demirözü, Sami Gürkahraman","doi":"10.5543/tkda.2025.99650","DOIUrl":"https://doi.org/10.5543/tkda.2025.99650","url":null,"abstract":"<p><strong>Objective: </strong>Limited data exist on the combined use of carotid endarterectomy (CEA) and off-pump coronary artery bypass grafting (OPCABG).</p><p><strong>Method: </strong>This retrospective study collected data from 42 patients who underwent simultaneous carotid endarterectomy and off-pump coronary artery bypass grafting between November 2015 and June 2023 at two affiliated institutions. CEA was performed first via eversion endarterectomy, followed by OPCABG using mostly arterial grafts on a beating heart, primarily with a \"no-touch\" aortic technique. Patient data were obtained from digital hospital records, and follow-up was completed through electronic systems or phone contact.</p><p><strong>Results: </strong>Among 1,154 OPCABG patients, 42 (3.6%) underwent simultaneous CEA, with a median age of 72 (range: 59-84); 35 patients (83.3%) were male and seven (16.7%) female. All patients were asymptomatic for carotid disease and diagnosed preoperatively via routine Doppler ultrasound. Complete arterial revascularization without aortic manipulation was achieved in 83.3% of cases, with a mean of 3.66 +- 1.22 distal anastomoses. Early mortality occurred in one critically ill patient (2.4%). One patient (2.4%) experienced a postoperative transient ischemic attack and recovered without neurologic sequelae. Seventeen patients (40.4%) were extubated in the operating room. During follow-up, no patients experienced cerebrovascular events; two patients died due to non-cardiac disease. As all events occurred within the first year, the Kaplan-Meier one-, three-, and five-year stroke-free survival rates were identical at 92.6 +- 4.1%.</p><p><strong>Conclusion: </strong>Concomitant CEA and OPCABG surgery is considered the optimal strategy for patients with extensive carotid and coronary artery stenosis at experienced centers. {"title":"A Case of Double-Inlet Left Ventricle Reaching Adulthood Without Surgery.","authors":"Emrah Kaya, Mehmet Ali Astarcıoğlu, Taner Şen, Emre Berk Erkip","doi":"10.5543/tkda.2025.67916","DOIUrl":"https://doi.org/10.5543/tkda.2025.67916","url":null,"abstract":"<p><p>Double-inlet left ventricle (DILV) are rarely encountered congenital heart defects which are also known as single-ventricle defects and have a complex structure in which the blood exiting both atriums flows into a single ventricle, and they constitute approximately 1.5% of all congenital heart diseases. The 37-year-old female patient who did not have a history of cardiac disease visited our outpatient clinic for routine cardiological assessments. Transthoracic echocardiography (TTE) was performed upon hearing a 3/6 pansystolic murmur at the mesocardiac focus and a 3/6 systolic ejection murmur at the pulmonary focus on cardiac auscultation. The TTE of the patient revealed that the atrioventricular valves opened into one ventricular chamber in four-chamber apical imaging. There was no noticeable interventricular septum, while a rudimentary right ventricle and a ventricular septal defect (VSD) were observed. There was a gradient of 38 mmHg in the pulmonary valve. Mild-to-moderate pulmonary stenosis was present. In the transesophageal echocardiography (TEE), a rudimentary interventricular septum and the right ventricle were observed. Because the patient did not have any symptoms or cyanosis, the patient was given information about a potential need for occasional infective endocarditis prophylaxis and phlebotomy, and close medical follow-up was decided. A double-inlet left ventricle is known as a severe congenital heart anomaly that is typically symptomatically diagnosed in childhood and requires surgical intervention. However, the extended duration and pain associated with the procedure may cause patient movements, potentially leading to disruptions in the electro-anatomical mapping systems. Sedative and analgesic agents are used to prevent body movements and manage pain. This study aimed to conduct a comparison between the safety and effects of ketamine and propofol for deep sedation on outcomes in AF patients undergoing radiofrequency ablation.</p><p><strong>Methods: </strong>This retrospective and single-center study included 108 patients who underwent radiofrequency AF ablation under deep sedation (without intubation) in our hospital. The patients were categorized into two groups based on the anesthetic agent administered for deep sedation: the propofol group and the ketamine group. Procedure duration, success rates, and recovery times were compared.</p><p><strong>Results: </strong>Of the 108 patients, 54 were in the propofol group and 54 were in the ketamine group. The procedure durations were similar in both groups (propofol group: 135 min (120 - 145) vs. ketamine group: 140 min (120 - 155), p=0.803). The eye-opening time after the procedure was 275 seconds in the propofol group and 266 seconds in the ketamine group (p=0.530). Additionally, no significant variation was detected in the initial measurements of systolic and diastolic blood pressure or heart rate.</p><p><strong>Conclusion: </strong>In conclusion, there was no significant difference between the propofol group and the ketamine group in terms of outcomes.
