Nearly half the studies did not report any definition or measurement of fracture. Among those that did, 29 out of 33 studies (88%) described a linear fracture shape, 13 out of 33 studies (39%) reported the extent of the fracture across the scaphoid and none specified the fracture line orientation. Of the 25 studies describing fractures using at least one of the shape and extent groupings, 12 (48%) regarded a linear signal through the cortex as a fracture, five (20%) regarded a linear intramedullary signal in isolation as a fracture and eight (32%) regarded a diffuse intramedullary signal in isolation as a fracture. Eighty-one patients were included with a median age of 25 years (interquartile range 15 to 35; range 6-71). The left-right laterality of limb absence was 27:54. Eighty-one per cent of left-hand-dominant (absent right hand) patients were left-eye dominant (compared with 57% of normal left-handers); 83% of right-hand-dominant (absent left hand) patients were right-eye dominant (compared with 66% normal right-handers). Patients were more likely to be right-eye dominant than left-eye dominant (odds ratio 22.4; 95% confidence interval: 7.0 to 85.8; <i>p < </i>0.001). However, concern remains about postoperative recurrence of the spasticity. In a previous prospective study of 42 patients, we reported significant improvement in spasticity with minor recurrence but overall tone reduction at an average of 31 months post-surgery. In this follow-up study we reassessed a subpopulation of patients from the same cohort to evaluate the long-term efficacy of hyperselective neurectomy. At an average of 53 months post-surgery, the results of 39 patients showed a similar trend to the previous study with significant improvement in the modified Ashworth and Tardieu scale within a year, and this improvement remained stable at the final follow-up. There was also significant improvement in spontaneous posture (mean 75-40° elbow flexion), with no decline in the range of motion and strength of the operated muscles. Antagonist muscle groups and functional scores demonstrated a general trend towards improvement (House score from 2.2 to 3.4). However, as with every tool, its use must be executed by the right hands with the right training and right instrumentation. In this article, four papers on arthroscopy were examined for their contribution to the literature and to reflect on the evolving role of arthroscopy in hand surgery. The dorsal sensory branch of the ulnar nerve remains vulnerable with the placements of ulnar portals. Building from previous work, a 'nerve density' concept was presented, as it relates to wrist pronation and supination. The advent of arthroscopy has allowed the closed repair of triangular fibrocartilage complex injuries with better healing potential. Well-defined anatomical landmarks allow more precise placement of suture materials which need to be used judiciously to avoid further injuries. Likewise, scaphoid non-union can be successfully treated as a closed procedure using arthroscopy. A surgeon's learning journey was revealed as a change in practice to obtain the best results. Perilunate injuries can also be treated arthroscopically, but the problems related to bone-ligament interface healing remain unsolved. These publications reflect the increasing usage of this valuable tool and the lessons learnt would further optimize results and avoid pitfalls. Major breakthroughs in wrist arthroscopy in the near future may be obtained through better training and education. This systematic review examined the prevalence of DD after surgical and non-surgical hand trauma by conducting thorough searches of the PubMed, Embase, Cochrane and Scopus databases. Qualitative methods were used to synthesise the data and summarize the findings that were unsuitable for meta-analysis. The findings revealed an increased risk of DD associated with exposure to hand-transmitted vibration, sports-related trauma and distal radial fractures. Surgical procedures such as trigger finger release were also found to be associated with an increased risk, particularly in individuals who are predisposed to the condition. Key risk factors included age, male sex, diabetes and smoking. Hand trauma, particularly surgical trauma and repetitive mechanical stress, is associated with the onset of DD. These findings highlight the need to consider the risk of developing DD in patients undergoing surgery or sustaining an injury. Three-dimensional printing techniques can be used to produce patient specific implants (PSIs). Theoretically, this technique improves accuracy of reconstruction and therefore patient-related outcomes. The aim of this retrospective cohort study was to compare pre- and postoperative patient-rated functional and radiological outcomes following corrective osteotomy of 47 malunited DRFs with PSIs. The primary outcome was the difference in Patient-Rated Wrist Hand Evaluation score (PRWHE). The median PRWHE improved from 68 (IQR: 60 to 77) to 17 (IQR: 7 to 30) after a median follow-up of 15 months in 32 patients. The mean preoperative dorsal tilt of -15° (SD: -8), improved to 7° (SD: 7) palmar tilt postoperatively in 46 patients. Preoperatively, 28 patients exhibited radiocarpal malalignment, which decreased to five postoperatively.
