The width of the proximal and distal articular surface and the height of the trapezium and second metacarpal facet were measured on 96 peritrapezial views reconstructed from computed tomography scans. The trapezoid articular surface of the trapezium (TRAST) angle was calculated, and four different cup designs were virtually positioned centrally in the trapezium and parallel to the proximal articular surface. Risk of perforation was defined as a cup that exceeds the ulnar border of the proximal articular surface. The mean TRAST angle in our study was 33°. Risk of perforation is higher when the second metacarpal facet is smaller than 5 mm and when the TRAST angle is 35° or more. {"title":"IFSSH Scientific Committee on Congenital Hand Conditions.","authors":"Wee Lam, Charles Goldfarb, Wiebke Huelsemann, David McCombe, Lindley Wall","doi":"10.1177/17531934241309467","DOIUrl":"10.1177/17531934241309467","url":null,"abstract":"<p><p>The Oberg, Manske and Tonkin (OMT) Classification of Congenital Hand and Upper Limb Anomalies was adopted as the official IFSSH classification in 2014 with recommendations for 3-yearly reviews and updates. This report examined the evidence and feedback from the scientific community to see whether changes to the 2020 OMT should be made. The Committee concluded that no current changes are required, but highlighted a number of areas where further research and discussions are needed. These areas include the conditions of symbrachydactyly vs transverse arrest, the ongoing challenge with classifying syndactyly, and the nomenclature of arthrogrypotic conditions. {"title":"Use of a remote assessment pathway for diagnosis of carpal tunnel syndrome.","authors":"Katie Macdonald, Hugh Pearson, Georgina H Frew, Philippa Rust","doi":"10.1177/17531934241270347","DOIUrl":"10.1177/17531934241270347","url":null,"abstract":"<p><p>We undertook a retrospective cohort study of a remote carpal tunnel syndrome assessment pathway created in response to limitations caused by the COVID-19 pandemic. Between July 2020 and September 2021, 702 patients referred from primary care (general practice) were assessed in a nurse-led telephone clinic using the carpal tunnel questionnaire of Kamath and Stothard (2003). Depending on their questionnaire score, patients were referred either for nerve conduction studies or a consultant hand surgeon review for diagnosis and treatment planning. Questionnaire scores of 3 and above accurately predicted a likely diagnosis of carpal tunnel syndrome in 83% of patients, and a diagnosis was unlikely in 90% of those with a score below 3. The pathway resulted in an estimated cost savings of £24,436 (€28,862, US$30,945) in comparison with the pre-pandemic service.
