Numerous risk factors for suicide have been identified, not only at the level of the individual, but also at the level of the family, the community and society as a whole. These factors include the existence of a mental health problem, in particular depression and substance abuse. At the clinical level, it is important to try to spot the signs of impending suicide and organize an appropriate care strategy. But there are also protective factors against suicide at individual, societal and cultural levels. There are many strategies for preventing suicide. In this perspective, clinical metabolomics, which focuses on metabolites, emerges as a particularly interesting and promising approach. Indeed, this science reflects the internal and external stimuli received by an individual, thus capturing their physiological and/or pathological state. Close to the phenotype, it represents the interface between the patient, their genes, and their environment in the broadest sense. Its translational nature requires the conjunction of several expertise areas, both in analytical, biostatistical, and clinical levels. Combined with other data, it allows the generation of predictive or diagnostic models useful for early detection and monitoring of pathologies, taking into account notably the individual characteristics of patients. {"title":"[Prevention of contrast-induced nephropathy].","authors":"François Cousin, Martin Moïse, Cédric Ilbert, Paul Meunier, François Jouret","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Contrast-induced nephropathy (CIN) is a renal complication occurring after the administration of iodinated contrast agents routinely used in medical imaging. CIN causes acute renal failure of varying severity. The pathophysiology of CIN is probably multifactorial: it involves (i) renal vasoconstriction inducing tissue hypoxia, and (ii) a possible direct toxicity of iodine derivatives leading to tubular inflammation and necrosis. Several risk factors are associated with CIN, some related to the procedure itself, others to the patient's co-morbid profile. In particular, the pre-existence of chronic renal failure, dehydration, congestive heart failure, diabetes or hypotension has been associated with an increased risk of CIN, as summarized in the Mehran score. Prevention of CIN relies essentially on adequate i.v. hydration before and after the procedure, and on the administration of the lowest possible volumes of contrast. In patients at high risk of CIN, the use of metformin and non-steroidal anti-inflammatory drugs is contraindicated at the time of contrast medium i.v. injection. Several therapeutic modalities can effectively reduce the risk of fractures both vertebral and non-vertebral. While a significant part of bone strength and structure is genetically determined, it should be recalled that the environment also plays a significant role in these parameters and the risk of fracture, thus offering preventive opportunities thanks to lifestyle. We will then discuss how a poor appreciation of the risks, on the part of both patients and clinicians, seems to fuel the phenomenon. We will discuss the interest of placing quaternary prevention within the broader framework of Shared Decision Making. We will focus on one of the stages of Shared Decision Making process, that of risk communication. CKD leads to significant cardiovascular morbidity and mortality, with a negative impact on quality of life and significant societal repercussions. Several drugs are effective in preventing and curbing CKD, including blockers of the renin/angiotensin/aldosterone system and inhibitors of the SGLT2 co-transporter. New molecules are currently in clinical trials focusing on the nephro-protection, such as non-steroidal mineralocorticoid receptor antagonists and GPL-1 receptor agonists. In addition to this drug arsenal, CKD prevention also relies on non-pharmacological optimization of hygienic-dietary measures, including smoking avoidance, physical activity and dietetics. Prevention of preterm birth and of its complications is a major public health issue. From before conception to long term follow up, many health actors are engaged in this preventive strategy with the same goal : to give the best quality of life for these vulnerable young patients. We will review different preventive aspects during antenatal and perinatal period, during NICU (Neonatal Intensive Care Unit) stay and after discharge. Prevention of prematurity's complications requires a global approach including respiratory, nutritional and infectious aspects among others. A healthy environment and a peaceful and equitable society ensure a normal brain development and promote individual (physical, mental, social, and spiritual) well-being. However, brain health is never certain and can be deteriorated, from conception until death, by exposure to multiple physical, chemical, biological, psychological, and social agents - the so-called exposome. From an integrated perspective, this article highlights the need to value, promote and protect brain health.
