The most effective evaluation methods for frailty and malnutrition are yet to be delineated, despite being proposed as predictors of adverse outcomes. Consequently, this study aims to assess the significance of these factors in predicting in-hospital complications within this population.</p><p><strong>Methods and results: </strong>In this single-centre, intrahospital case-control study, 217 older patients (aged >70 years) with ACS were categorized into two groups based on the occurrence of net adverse clinical events (NACE) during hospitalization: the NACE (n = 59) and non-NACE (n = 158) groups. Demographic and clinical variables, including assessments of frailty using the FRAIL scale (FS), dependency using the Barthel index, and nutritional status using both the prognostic nutritional index (PNI) and geriatric nutritional risk index, were collected. Regression models and receiver operating characteristics curves were constructed to predict NACE. Patients in the NACE group exhibited higher frailty (P < 0.001), increased dependency (P < 0.001), and poorer nutritional status (P < 0.05) compared with those in the non-NACE group. The FS [odds ratios (OR) 95% confidence interval (CI) = 2.03 (1.39-2.95), P < 0.001] and PNI [OR (95% CI) = 0.64 (0.41-0.99), P < 0.05] scores emerged as independent predictors of NACE in a multivariable model including age, sex, and traditional cardiovascular risk factors. {"title":"How is digital technology used to facilitate shared decision-making between healthcare professionals and patients in cardiovascular care? A scoping review.","authors":"Dirk Leysen, Eva Propst, Hannah McGowan, Franziska Pfannerstill, Rik Crutzen, Stefan Tino Kulnik","doi":"10.1093/eurjcn/zvaf029","DOIUrl":"10.1093/eurjcn/zvaf029","url":null,"abstract":"<p><strong>Aims: </strong>To (i) identify characteristics of digital technologies (DTs) used to facilitate shared decision-making (SDM) within the field of cardiovascular care, (ii) examine conceptualizations of SDM underpinning these DTs, and (iii) summarize designs and outcomes of evaluation studies concerning these DTs.</p><p><strong>Methods and results: </strong>We conducted a scoping review following methodological guidelines by the Joanna Briggs Institute. Ten scientific databases were searched. We selected peer-reviewed articles in cardiovascular care, with SDM and DT in the title and/or abstract published between January 2000 and March 2023. We extracted data on the type of DT, intended user(s), time point and mode of use, purpose, decision topic, underlying SDM model, and designs and outcomes of evaluation studies. Data were analysed descriptively in a narrative synthesis. From 4432 search results, 48 articles reporting on 31 DTs were included. Half of the articles were published after 2018, with most published in 2022. Most DTs are web-based applications for interactive use before and during the encounter between patient and healthcare professional, to convey information on cardiovascular care, calculate personal risk, and/or recommend treatment options. Shared decision-making conceptualizations refer to the International Patient Decision Aid Standards and authors in the field. Thirty-seven evaluation studies report on 31 DTs, with heterogeneous results related to variation in intervention designs, measurement time points, and outcome measures.</p><p><strong>Conclusion: </strong>A recent increase in the reporting of DTs to facilitate SDM in cardiovascular care indicates their potential. Understanding these needs is essential for developing targeted interventions that enhance informal caregiver support and improve patient outcomes in CR. This study aimed to explore the educational needs of informal caregivers supporting individuals with CVD attending CR.</p><p><strong>Methods and results: </strong>A mixed-methods approach was used to explore the educational needs of informal caregivers. Quantitative data were collected through an online cross-sectional survey completed by 86 informal caregivers. The survey assessed sociodemographic characteristics, quality of life (WHOQOL-BREF), and educational needs. Most respondents had not received formal education or training related to caregiving, although their knowledge of CVD was rated as moderate to high. Many reported difficulties accessing reliable information and resources. Qualitative data were gathered through semi-structured interviews with 16 informal caregivers, and four key themes were determined: Helpful Resources, Empowerment, Lived Experience Network, and Psychological Support. Informal caregivers expressed a desire for practical resources, more direct access to healthcare professionals, and both emotional and peer support. They also highlighted the importance of managing stress and balancing their well-being alongside caregiving responsibilities.</p><p><strong>Conclusion: </strong>The findings suggest that educational interventions for informal caregivers should be comprehensive and flexible, incorporating practical caregiving strategies as well as social and emotional support. Screening to detect delirium in high-risk patients is important; yet, hypoactive delirium is often missed in nurse-reported screening. Polymorphic delta waves are associated with delirium, and can be detected with single-channel electroencephalography (SC-EEG). The aim of the study is to assess whether SC-EEG as a screening instrument for delirium will increase the detection rate of post-operative delirium and reduce hospital stay of delirious patients.</p><p><strong>Methods and results: </strong>A prospective quasi-experimental study compared Delirium Observation Screening Scale (DOSS) screening (442 patients) to SC-EEG screening (462 patients) to detect post-operative delirium in cardiac surgery patients aged over 70 years. Delirium was diagnosed using the Diagnostic and Statistical Manual of Mental Disorders 5th edition. Detection of delirium was higher in the SC-EEG group (20%) compared with DOSS group (14%), P = 0.016. A difference in length of stay for delirious patients was attributed to baseline differences as pneumonia and longer cardiopulmonary bypass time, but not delirium screening method. Length of stay for all patients was -0.11 (95% CI -0.18 to -0.04) night shorter for SC-EEG cohort patients compared with DOSS cohort patients, P = 0.002.</p><p><strong>Conclusion: </strong>Screening with SC-EEG increased delirium detection after cardiac surgery. Only length of stay for all patients was significantly reduced in the SC-EEG cohort.
