A cross-sectional, community-based, family-approach study was carried out with 434 families in Salvaterra, Marajó Island, Pará, Brazil, from June to July 2021. A household survey was carried out with a standardized questionnaire on sociodemographic data and the Brazilian Food Insecurity Scale. A multivariate logistic regression was used to analyze associated factors. Most quilombola families (88.9%) lied in food insecurity, 33.1% (n = 144) of whom in severe insecurity; 21.4% (n = 93), in moderate insecurity; and 34.3% (n = 149), in mild insecurity. The regression model showed that receiving no pension doubles the chance of mild food insecurity and multiplies moderate+severe insecurity by 5 times. Per capita family income below 1/5 or even 1/3 of the minimum wage increase the chance of moderate+severe insecurity by 9 and 9 times, respectively. Households in the B/C economic classification were 21% less likely to have moderate+severe insecurity than those with the D/E classification. Food insecurity had a high prevalence in quilombola families, with a preponderance of moderate+severe insecurity. Family income and its components configured the main factors associated with food insecurity.
","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"42 ","pages":"e00171825"},"PeriodicalIF":2.1,"publicationDate":"2026-07-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13404604/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148618437","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Using data from the Unified Registry and the finances of 5,570 Brazilian municipalities, we estimated the cost of these programs as a proportion of total and social expenditures. The results show that, for most municipalities, the implementation of programs along the lines of the studied models would be significantly more costly than for the reference municipalities. This study observed a marked regional inequality in implementation capacity, with the Brazilian North and Northeast facing an excessive fiscal burden, whereas its South and Southeast showed greater viability, especially for the Niterói model. Simulating scenarios with fiscal efforts resembling those in Maricá and Niterói found a trade-off between coverage and benefit and very large regional inequalities in transfer value. The universalization of basic income programs in Brazil based exclusively on municipal resources is unlikely due to deep regional disparities and high fiscal cost. This study suggests the need for a financing model that goes beyond the municipal sphere, enjoying substantial federal support and a link to a broader debate on fiscal justice and income redistribution to ensure equal benefit distribution and the population's well-being.
","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"42 ","pages":"e00157325"},"PeriodicalIF":2.1,"publicationDate":"2026-07-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13440318/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148618429","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This cross-sectional study was carried out in 2023 with 290 users of primary care units in Niterói (distributed among Polyclinics, the Family Medical Program, and Psychosocial Care Centers). Financial sacrifice was defined as the need to postpone essential purchases, take out loans, or sell goods to pay for health expenses. Structured questionnaires were applied based on validated methods to evaluate the pharmaceutical sector. Descriptive analyses and binary logistic regression were performed with a 5% significance level. The prevalence of financial sacrifice totaled 35.5% (95%CI: 30.2-41.0), with no significant difference between types of service (p = 0.416). Hospital admission was the only factor independently associated with the outcome (OR = 2.89; 95%CI: 1.05-7.94; p = 0.04). This study found that 6.2% of interviewees reported expenses with medication, with no significant association with financial sacrifice. The most frequent coping strategies referred to bill postponement (14.8%) and formal or informal indebtedness (12.7%), followed by reduction in food purchases (5.2%). Financial sacrifice affects more than a third of primary care users, with hospital admissions being the main associated factor. These findings highlight financial protection gaps in the Brazilian Unified National Health System and reinforce the need to strengthen policies for the public provision of medication and protection against catastrophic expenditures.
","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"42 ","pages":"e00231825"},"PeriodicalIF":2.1,"publicationDate":"2026-07-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13440317/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148618428","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The annual cases reported in the Morbidity Yearbooks of the Ministry of Health (2005-2023) and age-adjusted rates (population ≥ 20 years) were used. Analyses were performed stratified by sex and age groups (20-44, 45-59, ≥ 60 years). After the ENPCSOD, an immediate decrease of 55.20 cases per 100,000 adults was observed (95%CI: -86.24; -24.15; p = 0.004), with no evidence of a sustained downward trend in the post-strategy period (post-ENPCSOD slope, β = 5.52; p = 0.180). The COVID-19 pandemic was associated with a sharp drop in the level (β = -109.04; p < 0,001). A scoping review was carried out according to the methodological guidelines of the JBI, registered on the Open Science Framework. The retrieved scientific studies included articles, theses, dissertations, academic papers, and various official regulations. The findings show that, despite important legal frameworks, these regulations emerged in historical contexts in which the identity categories and specific demands of trans men and transmasculine people were not yet consolidated in the public and legal debate. Notably, by remaining without substantive updates, these provisions produce effects of exclusion and invisibility, showing the persistence of a cisheteronormative logic in the structuring of access to reproductive health. Recent advances include the recognition of trans parenting in official documents and the creation of specific materials, such as the Transgesta prenatal booklet. However, critical gaps persist in access to fertilization, gamete preservation, and family planning, as do symbolic and institutional barriers to health care. This study shows that the fulfilment of sexual and reproductive rights of this group depends on more inclusive normative frameworks and on the transformation of institutional practices and the training of healthcare providers. The historical and critical analysis of policies in this study evince advances, setbacks, and omissions, contributing to expand knowledge about the reproductive health of trans men and transmasculine people in Brazil.
","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"42 ","pages":"e00200525"},"PeriodicalIF":2.1,"publicationDate":"2026-07-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13411463/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148618400","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This mortality projection study was based on lung cancer deaths (ICD-10: C33-C34) registered in the Brazilian Mortality Information System between 2001 and 2020, with redistribution of ill-defined causes. Crude and age-adjusted rates were calculated according to the 1960 world standard population. The projections adopted the Nordpred model in the R program, in 5-year periods from 2021 to 2045. There was a projected increase in the absolute number of deaths in both sexes, especially in the South and Southeast regions. The age-adjusted rates indicate a downward trend among men in all regions, despite remaining higher in the South. Among women, there was growth by 2025, followed by stability or a slight reduction by 2045, with the highest rates in the South Region. Population aging contributes to the increase in crude rates, even with the decline in adjusted rates. The projections suggest persistent regional inequalities, which may be associated with historical smoking patterns and unequal access to lung cancer diagnosis and treatment. The findings reinforce the need for regional prevention and control policies, including addressing smoking and considering the new forms of nicotine consumption, such as electronic cigarettes.
","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"42 ","pages":"e00199725"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13411256/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148618487","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Data were obtained from 144,046 adolescents in the 2019 Brazilian National Survey of School Health (PeNSE, acronym in Portuguese). Self-reported indicators included the frequency of five emotional states, weekly minutes of physical activity in leisure time, physical education, and commuting, and daily TV hours. Latent class analysis classified adolescents based on their mental health profiles, and multinomial logistic regression estimated associations between physical activity domains and TV viewing time with mental health classes, adjusting for covariates. Six mental health profiles were identified: (1) school-related concern; (2) anxious and stressed; (3) subthreshold internalizing; (4) anxious, sad, and angry; (5) low symptom; (6) severe internalizing. Adolescents who engaged in physical activity in their leisure time and at school were more likely to belong to the low symptom class, while those who engaged in physical activity during commuting were more likely to be in class 6 (severe internalizing). Adolescents who watched two to four hours of TV per day were more likely to belong to class 1 (school-related concern), class 4 (anxious, sad, and angry), and class 5 (low symptom), whereas those who watched TV more than four hours/day were less likely to belong to class 3 (subthreshold internalizing) and class 6 (severe internalizing).
