Predictors of different physical activity intensities have not been sufficiently analyzed. This study aimed to identify clusters of trajectories of physical activity intensity in leisure time, their predictors and the profile of the participants in the clusters. Baseline data and two follow-up visits of 11,262 participants from the Brazilian Longitudinal Study of Adult Health (ELSA-Brazil) were included. physical activity was assessed at three moments using the International Physical Activity Questionnaire (IPAQ). Clusters of physical activity trajectories according to intensity (weak, moderate and strong) were identified via longitudinal K-means. The number of clusters was based on the within-clusters sum-of-squares (WCSS) measure and the classification was based on scientific recommendations. Machine learning was used to verify predictors importance. Five clusters were identified for men and four for women. Men in the adequate cluster with a strong increase in physical activity had higher income, schooling level, and daily consumption of fruits and vegetables; they were younger; had never smoked and had a normal nutritional status. On the other hand, women in the adequate cluster with moderate physical activity increase had higher income and schooling level; had never smoked and had a normal nutritional status. In both sexes, age and schooling level were the most important predictors for classification in clusters. Actions to promote physical activity should be implemented over time, and be adapted to sociodemographic and behavioral factors.</p>","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"41 4","pages":"e00132924"},"PeriodicalIF":1.9,"publicationDate":"2025-05-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12091857/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144109749","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In Cubatão, a municipality in the state of São Paulo, official data from the Information System of the Brazilian Immunization Program indicate a sharp decline in children's vaccination coverage. The study aimed to estimate the actual vaccination coverage in children under 2 years old, assess whether doses are administered on time, and identify factors associated with vaccination delays using a household survey with Cubatão residents born in 2019 and 2020. The municipality was divided into five socioeconomic strata, and the sample included 450 children. Vaccination coverage at 12 months was 86.9%, and 82.6% at 24 months. Only 25.7% of children received the full vaccination schedule without delays. Factors associated with the delay in vaccination at 24 months included the mother's educational level and receiving Bolsa Família benefits. Although vaccination coverage is higher than that in national surveys, delayed vaccination is a public health issue in Cubatão, requiring better education of healthcare professionals and the population on the importance of timely vaccination.</p>","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"41 4","pages":"e00089524"},"PeriodicalIF":1.9,"publicationDate":"2025-05-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12091858/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144109755","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This article analyzes the association between PAs focused on SUS and the reelection of municipal mayors in 2024. The research used public secondary data, involving 2,818 municipalities. The reelection of mayors was related to the per capita average of amendments transferred from the Brazilian National Health Fund, from January 2021 to October 2024. Hierarchical logistic regression was used, controlling individual and political factors of candidates, and socioeconomic and demographic municipal factors. The results indicate a positive and statistically significant association between the increase in resources for health amendments and the probability of reelection of mayors in 2024 in all models and cutoffs. Note that the values of amendments were considerably higher in municipalities with up to 20,000 inhabitants compared to the other municipalities. Possibly, the amendments consolidate the image of city mayors as capable of attracting resources, strengthening their electoral support in a context of quota of federal expenses. The political use of the amendments contrasts with the criteria adopted for the transfer of resources from SUS programs and may weaken the coordination of the Brazilian Ministry of Health.</p>","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"41 4","pages":"e00220924"},"PeriodicalIF":1.9,"publicationDate":"2025-05-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12091854/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144109718","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This is a qualitative study that used ethnographic observation, semi-structured interviews, and focus groups. It assessed 139 male and female participants aged 15 to 24 years living in five low-income Brazilian communities located in Porto Alegre, São Paulo, Rio de Janeiro, Manaus, and Salvador. When analyzing the experiences and demands of young people regarding sexual and reproductive health, including HIV testing, despite regional specificities, the interviewees share similar perspectives regarding the use of local public health services. Complaints about long waiting times for care and insufficient professionals were emphasized by the interviewees. At an individual level, young people have trouble accessing health information and services due to barriers such as poor knowledge about their rights, shame, and stigma associated with sexual and reproductive health issues, as well as fear of testing positive for HIV. The lack of dialogue and guidance on sexuality and health in the family also contributes to this vulnerability. At a programmatic level, discrepancies between the working conditions and dynamics of health units and the demands of adolescents and young people, associated with poor preparation of health professionals to address specific issues of this population, are highlighted as obstacles. This study concluded that there is a lack of regular spaces for learning and dialogue about sexuality, prevention, and care for the youth due to absent investment in public health care and professional training.</p>","PeriodicalId":9398,"journal":{"name":"Cadernos de saude publica","volume":"41 4","pages":"e00047824"},"PeriodicalIF":1.9,"publicationDate":"2025-05-19","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC12091859/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"144109720","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Particularly effective in preparing for syndemic cycles within the context of environmental crises, the concept of comprehensive prevention contributed to understanding processes of vulnerability to COVID-19, mpox, sexually transmitted infections/AIDS, violence, unwanted pregnancy and psychosocial distress. It provided guidance for addressing the syndemic nature of pandemics intensified by social crises, and encouraged young people to develop creative responses to political polarization fueled by the far right, which aggressively inhibits approaches based on human rights, especially in the field of sexuality, enhancing the infodemic that downplays the severity of ongoing pandemics. We underline the productivity of the concept of comprehensive care in developing initiatives related to human rights and prevention which dynamically anticipate and address events that synergistically overlap in marginalized territories. In total, 9,901 adults aged 50 years or older were studied in the second wave of ELSI-Brazil. A 53-item FI was created according to a standardized protocol. Logistic regression was used to determine the association between frailty levels and disability/health status, whereas the relationship between frailty level, disabilities, and healthcare use was analyzed by a negative binomial regression. Frailty was high, with a 0.19 weighted mean FI score and 0.19 median. Frailty distribution was right-skewed, with higher levels in women and increased exponentially with age. Widow(er)s, black and mixed-race individuals, and those living in rural areas had higher levels of frailty. Regression models showed that higher frailty was associated with poorer self-assessment of health, higher disability, and greater use of healthcare services. The sample was composed of 2,384 adolescents, aged 12 to 17, resident in Brazilian capitals and participants in the Study of Cardiovascular Risk in Adolescents (ERICA), a school-based cross-sectional study undertaken in 2013 and 2014. The outcome variable was alcoholic beverage consumption characterized as having drunk an alcoholic beverage at least once in the previous 30 days. The exposure variable was urban physical disorder measured according to the urban features in the 2010 Demographic Census. The total effect of the indicators of exposure to urban physical disorder on the consumption of alcohol among adolescents was estimated using Poisson regression models with robust variance. Crude and adjusted prevalence ratios (PR) were calculated with 95% confidence intervals (95%CI) for each exposure, controlling for possible confounding variables. Adolescents living in areas with paved roads and manholes presented higher prevalence of alcohol consumption (adjusted PR = 1.24, 95%CI: 1.02; 1.50 and adjusted PR = 1.36, 95%CI: 1.01; 1.84, respectively). Conversely, the presence of wheelchair ramps and open sewers was associated with a lower prevalence of alcohol consumption (adjusted PR = 0.79, 95%CI: 0.62; 0.99 and adjusted PR = 0.80, 95%CI: 0.66; 0.97, respectively).
